Özet

43 6. ÖZET Çalışmamızda, yeni bir anestezik ajan olan ve klasik preparatlara göre çeşitli avantajları öne sürülen propofolün hücresel ve humoral immun sistem üzerine etkisinin eski ve daha yaygın olarak kullanılan thiopentone ile karşılaştırılması ve sonuçlara göre anestezide hastalara uygun preparat kullanılınıı önerilmesi amaçlanmıştır. Genel cerrahi operasyonu geçirecek ASA I,H sınıflamasına giren 40 erişkin birey 20'şer kişiden oluşan iki gruba ayrıldı. Total intravenöz anestezi (TIVA) tekniğiyle birinci gruptaki hastalara propofol infuzyonu (İndüksiyon için 2 mg/kg, idame için 6-12 mg/kg/saat), ikinci gruptakilere ise thiopentone (İndüksiyon için 3-5 mg/kg, idame için 7 mg/kg/saat) uygulandı. Anestezik gaz verilmedi. Hastalardan operasyondan 30 dk önce, 30 dk ve 24 saat sonra alınan kanlarda immünolojik (IgG, IgM, IgA, C3, C4, CD4 ve CD8), biyokimyasal (SGOT, SGPT, fosfolipid, kolesterol, trigliserid, alkalen fosfataz, üre ve kreatinin) ve hematolojik tetkikler (Lökosit, Hct, Hb, periferik yayma, lenfosit sayımı) yapıldı. İmmunglobülinler ve kompleman, Radyal İmmunodiffuzyon tekniği ile, CD4-CD8 ise monoklonal antikorlar kullanılarak, immunofloresans yöntemiyle araştınldı. Aynca hastalara anestezi indüksiyonundan 2 gün önce ve sonra tüberkülin testi uygulandı. Çalışmamız sonucunda, propofol uygulanan grupta (1. grup) postop. 30. dk'da IgG, IgM, IgA ve C3, thiopentone uygulanan grupta ise IgM, IgA ve C4 düzeylerindeki azalma anlamlı bulundu (p

SUMMARY THE COMPARISON OF THE EFFECT OF PROPOFOL AND THIOPENTONE ON CELLULAR AND HUMORAL IMMUNITY This study was designed to compare the effect of thiopentone which was formerly and more commonly used, with propofol, a new anaesthetic agent, on cellular and humoral immunity and to choose the suitable one to patients according to the discovered results. 40 adult patients classified on ASA I, II scheduled for surgical operation were divided into equal two groups by using TIVA technique first group patients were infused with propofol (2 mg/kg for induction, 6-12 mg/kg/hr for maintenance) and second group infused with thiopentone (3-5 mg/kg for induction, 7 mg/kg/hr for maintenance). Anaesthetic gas was not given. Many immunological (IgG, IgM, IgA, C3-C4, CD4 and CD8), biochemistrical (ALT, AST, phosfolipids, cholesterol, trigliserid, Alk-P, urea and creatinin) and haematological (leucocyte, net, Hb, blood smear and lymphocyte counts) testes were studied on samples obtained 30 min. before, 30 min. and 24 hr after the operation. The immunoglobulin and complement were studied by radial immunodiffusion technique and CD4-CD8 were studied by immunoflorescence + monoclonal antibodies technique.
