Özet
Çalışmanın amacı, demansı olan bireylerde, Roy'un Adaptasyon Modeli'ne temellendirilmiş Bilişsel Uyarım Terapisinin, başetme-uyum, bilişsel işlevler ve yaşam kalitesine olan etkisini saptamaktır. Araştırmanın evrenini Akdeniz Üniversitesi Nöroloji polikliniğine kayıtlı demans tanılı hastalar oluşturmuştur. Araştırma deneysel tipte, ön test son test düzeninde tasarlanmış randomize kontrollü bir çalışmadır. Bireyler girişim ve kontrol gruplarına basit randomizasyon yöntemi ile atanmıştır. Çalışma, 30 girişim, 30 kontrol grubunda olmak üzere toplam 60 birey ile yürütülmüştür. Araştırmanın uygulama aşaması Antalya Sosyal ve Sağlıklı Yaşam Derneği'nde yapılmıştır. Veriler, sosyodemografik ve hastalık öyküsü bilgi formu, Başetme ve Uyum Süreci Ölçeği, Standardize Mini Mental Test ve Alzheimer Hastalarında Yaşam Kalitesi Ölçeği kullanılarak toplanmıştır. Girişim grubunda yer alan bireylere Roy'un Adaptasyon Modeli'ne temellendirilmiş Bilişsel Uyarım Terapisi uygulanmıştır. Veriler t testi, Mann Whitney U testi ve Wilcoxon işaret testi kullanılarak değerlendirilmiştir. Çalışmanın sonunda, Roy'un Adaptasyon Modeli'ne temellendirilmiş Bilişsel Uyarım Terapisi uygulanan girişim grubundaki bireylerin başetme-uyum, bilişsel işlev ve yaşam kalitesi düzeylerinin, kontrol grubundaki bireylerin başetme-uyum, bilişsel işlev ve yaşam kalitesi düzeylerinden yüksek olduğu saptanmıştır. Roy'un Adaptasyon Modeli'ne temellendirilerek uygulanan Bilişsel Uyarım Terapisi'nin hafif ve orta evre demanslı bireylerin, başetme-uyum, bilişsel işlev ve yaşam kalitesi düzeyleri üzerine olumlu yönde etkili olduğu görülmüştür. Bu sonuçlara dayalı olarak,Roy'un Adaptasyon Modeli'ne temellendirilmiş Bilişsel Uyarım Terapisi'nin demanslı bireylerin bilişsel işlevler, başetme-uyum ve yaşam kalitesini artırmada hemşirelik uygulaması olarak kullanılması önerilmektedir.

The objective of the study was to determine the effect of“Cognitive Stimulation Therapy”based on Roy's Adaptation Model on the coping-adjustment, cognitive functions and quality of life of individuals with dementia. The study population consists of dementia patients at the Akdeniz University Neurology polyclinic. The study is a randomized controlled study designed in the experimental type using a single blind pre-test, post-test pattern. Individuals are assigned to intervention and control groups with simple randomization method. The study was carried out with 60 dementia patients with 30 in the intervention and 30 in the control groups. The application stage of the study was carried out at the Social and Healthy Life Association. Data was acquired via socio-demographic and disease history information form, Coping and Adjustment Period Scale, Standardized Mini Mental Test and Quality of Life in Alzheimer's Disease Scale. Cognitive Stimulation Therapy based on Roy's Adaptation Model was applied to the intervention group. The data were evaluated using t test, Mann Whitney U test and Wilcoxon signed rank test. It was determined as a result of the study that the coping-adjustment, cognitive function and quality of life levels of the intervention group patients subject to Cognitive Stimulation Therapy based on Roy's Adaptation Model were higher in comparison with the coping-adjustment, cognitive function and quality of life levels of the individuals in the control group. It was observed that the Cognitive Stimulation Therapy applied based on Roy's Adaptation Model had a positive effect on the coping-adjustment, cognitive function and quality of life of light and moderate stage dementia patients. Cognitive Stimulation Therapy applied based on Roy's Adaptation Model dementia cognitive functions, the quality of life and coping-compliance of individuals, to be used as nursing practices in improving is recommended.
