Özet

Bilgi ve İletişim Teknolojileri günümüzde hızlı bir gelişim süreci içerisindedir. Diğer sektörlerde olduğu gibi Sağlık sektörü de bilgi teknolojilerinde ki bu gelişmeden olumlu yönde etkilenmektedir. Sağlık hizmeti sunan kuruluşlar karmaşık yapıda olup, sunulan hizmeti niteliğinden dolayı bilgi sistemlerine daha çok ihtiyaç duyan kuruluşlardır. Sağlık hizmet sunucuları teknoloji - emek yoğun ve karmaşık olarak sunulan sağlık hizmetlerinin yönetilebilirliğini kolaylaştırmak için kendi bünyelerinde Hastane Bilgi Yönetim Sistemlerini(HBYS) kullanmaktadırlar. HBYS'ler kullanıldıkları hastanelerin niteliğine cevap verecek şekilde zaman içinde gelişimini sürekli olarak sürdürmektedirler. Hastane bilgi yönetim sistemlerinin bilgi teknolojilerine paralel olarak hızlı gelişmesi ve hastanelerin bu sistemleri aktif olarak kullanıyor olması E-Sağlık uygulamalarının geliştirilmesine ve uygulanabilir olmasına zemin hazırlamıştır. Elektronik Reçete uygulaması Sosyal Güvenlik Kurumun geliştirmiş olduğu bir E-Sağlık uygulaması olup, sağlık hizmet sunucuları bu E-Sağlık uygulamasını kendi HBYS'leri vasıtasıyla entegre olarak kullanmaktadırlar. Buradan hareketle birinci bölümde Hastane Bilgi Yönetim Sistemleri ve E-Sağlık uygulamalarından bahsedilmiştir. İkinci bölümde ise E-Reçete uygulamasının içerisinde yer aldığı MEDULA sistemi ve E-Reçete uygulamasının genel yapısı ve işleyişine yer verilmiştir. E-Reçete uygulaması MEDULA hastane uygulamasında yer alan tüm hekimleri ve Sosyal Güvenlik Kurumu ile sözleşmeli tüm eczaneleri sürece dâhil ettiği için kapsamı oldukça geniş ve en aktif kullanılan E-Sağlık uygulamalarından birisidir. Bundan dolayı Tez çalışmamda E-Reçete Uygulaması Hekim ve Eczacı açısından ayrı ayrı değerlendirilmiştir. Uygulamanın sağlık hizmet sunucularına sağladığı kolaylıklar ve uygulamada yaşanan problemler alan araştırması kısmında ele alınmıştır. Alan araştırmasında betimleme tekniği kullanılmış olup, uygulama her açıdan incelenip analiz yapıldıktan sonra mülakat soruları hazırlanarak bir hekim ve bir eczacı ile mülakat çalışması yapılmıştır. Hekim açısından değerlendirilme kısmında uygulamanın reçete tanzim sürecine olumlu ve olumsuz etkileri ele alınmış, matbu reçeteye göre kıyaslamalar yapılmıştır. E-Reçete uygulamasında tüm işlemlerin Hastane bilgi sistemi üzerinden yapılıyor olmasının hekimlere neler getirdiği konusunun üzerinde durulmuştur. Bilgi sistemine bağlı olunmasının hekimleri rahatız ediyor olması ancak burada esas problemi reçete tanzim sürecinde sistemden kaynaklanan teknik problemler, sistemde yaşanan kesintiler oluşturmaktadır. Yani hasta bakım ve reçete tanzim sürecinde bilgi sistemlerinde yaşanan kesinti ve teknik problemler hekimleri rahatsız etmektedir. Uygulamanın hekimlere sağlamış olduğu kolaylıklar ise detaylı şekilde alan araştırma kısmında verilmiştir. Eczacı açısından değerlendirilme kısmında uygulamanın reçete karşılama ve eczane iç süreçlerine etkisi değerlendirilmiştir. Matbu reçetede yaşanan tüm problemlerin ortadan kalkması ve reçeteye ilişkin tüm verinin sisteme otomatik olarak geliyor olması eczaneler için büyük kolaylık oluşturmuştur. Kısmen de olsa bilgi sistemine bağlılığın getirdiği olumsuz durumlar yaşansa da eczacı açısından değerlendirmede önemli ölçüde memnuniyet ve hizmet sunumuna olumlu katkı söz konusudur. Information and Communication Technologies are currently in the process of rapid development. Like it is in other sectors the health sector is also positively affected by the development of information technologies. The structures of organizations offering health care is very complex and because of the nature of services they offer, they require more information systems. In order for health care providers to ease the management of technology, labor intensive and complex healthcare services offered, they are using the Hospital Information Management System (HBYS) within their own structure. The HBYS systems maintain their continuous improvement in order to respond to the nature of their use in the hospitals. The fact that the hospital information management systems are developed parallel to the rapid development of technology and the fact that these systems are actively used in hospitals, laid the groundwork for the development of practicable e-Health applications. The electronic prescription application is an e-Health application created by the Social Security Administration and health service providers use this e-Health application within their own HBYS system. Hence in the first part the Hospital Information Management Systems and e-Health applications and in the second part the MEDULA system in which the e-Prescription application is included and the general structure and funtions of e-Prescription application were mentioned. Because the e-Prescription application is used within the MEDULA hospital application, it is included in all processes regarding the physician and contracted pharmacies, which makes it one of the broadest and most actively used e-Health applications.Within the thesis the E-Prescription Application was evaluated from the point of the Physician and Pharmacist. Conveniences and experienced problems that were brought to health providers by the application were evaluated in the part of field survey.In the field study, descriptive technique was used and after the examination of the application from every angle and the necessary analyzes interview questions were prepared and an interview was conducted with a physician and a pharmacist. In terms of evaluation of the Physicians, the positive and negative effects of the application on the prescribing process were handled and comparisons were made according to the printed prescription. The results to the physicians of the E-Recipe application which is done through the hospital information system were emphasized. That physicians are uncomfortable for being dependent on the information system and it was observed that the main problem is the technical problems arising from the system during the prescription process system interruptions. In other words, downtime and technical problems in information systems during patient care and prescription disturb the physicians. The facilities that the application has provided to physicians were given in detail in the research section. In terms of evaluation of the Pharmacists, any impacts on the pharmacy internal processes were evaluated. Elimination of all the problems experienced in preprinted prescriptions and that the entire prescription data are taken automatically to the system created great convenience for pharmacies. Partially, even though there exist some adverse conditions brought by the dependence to the system, in terms of pharmacists significant satisfaction and positive contribution to the provision of services is concerned.
