Özet

Genel yoğun bakım hastalarında pleth variability indeks ile vena cava inferior distensibility indeks değerlerinin sıvı tedavi yanıtlılığının öngörülmesindeki yeri Amaç: Yoğun bakım hastalarında sıvı dengesinin doğru ve hızlı bir şekilde belirlenmesi çok önemlidir. Sıvı dengesinin en iyi nasıl belirleneceği konusunda fikir birliği bulunmamakta ve literatürde farklı yöntemlerin kullanıldığı görülmektedir. Bu nedenle çalışmamızda genel yoğun bakım ünitesinde yatan hastalarda noninvaziv yöntemlerden pleth variability index(PVI), inferior vena cava distensibility index(dİVC), pasif bacak kaldırma (PBKT,passive leg raising test) ve ekspiryum sonu oklüzyon testi (EEOT) ölçümlerinin invaziv picco kardiyak index(CI) ve global end-diyastolik volüm index(GEDI) ölçümleriyle korelasyonuna bakmayı amaçladık. Böylelikle yoğun bakımda noninvaziv yöntemlerle en hızlı şekilde sıvı tedavi yanıtlılığını öngörmeyi hedefledik. Gereç ve Yöntem: YBÜ'de rastgele seçilen 18-90 yaş aralığında 68 hastada supin pozisyonda EKG, SpO2, PİCCO femoral intraarteriyel nabız basınç monitörizasyonu, ısı monitörizasyonu yapıldı. Seldinger tekniği ile 3 yollu santral kateter takıldı. Hastalar supin nötr pozisyonda iken CVP monitörizasyonu ile CVP ölçüldü, CVP'nin <8 mmHg değeri hipovolemi lehine değerlendirildi.Yine aynı pozisyonda ultrasonografi cihazının ECHO probu ile İVC görüntülemesi yapılıp dİVC hesaplandı, dİVC >%18 değeri hipovolemi lehine değerlendirildi. Aynı zamanda hastaya massimo tekniği ile ölçüm yapan parmak probu ile PVI monitorizasyonu yapıldı. PVI>%14 hipovolemi lehine değerlendirildi. Hastaya pulse contour cardiac index(PCCİ) ve nabız basınç kontrollü PBKT uygulanıp PCCİ >%12 ve sistolik tansiyonda >%12 artış pozitif kabul edildi. PBKT yapılırken dİVC, PVI ve CVP ölçümleri tekrarlandı. Daha sonra hasta supin nötral pozisyona alındı ve EEOT yapıldı ve PCCİ>%5 ve sistolik tansiyonda>%5 artış pozitif kabul edildi, eş zamanlı CVP ve PVI değerleri kaydedildi. Bulgular: Yaptığımız çalışmada, istatistiksel olarak Cİ ve GEDİ ile PBKT arasında güçlü derece pozitif korelasyon(p:0.0001), Cİ ve GEDİ ile EEOT arasında güçlü derece pozitif korelasyon ve Cİ ve GEDİ ile CVP, dİVC, PVI arasında orta derece pozitif korelasyon(p:0.0001) tespit edildi. Noninvaziv ölçümlerden PBKT %90 sensivite, %89 spesifite ile en yüksek korelasyonu gösterirken, CVP %87 sensivite, %65 spesifite ile en düşük korelasyonu gösterdi. Aynı zamanda PBKT ve EEOT sonrası sıvı cevaplı grupta PVI' de istatiksel olarak anlamlı olarak azalma(p:0,001/PBKT,P:0,003/EEOT), CVP'de istatiksel olarak anlamlı artma (p:0,0001/PBKT,p:0,001/EEOT) ve dİVC'da istatiksel olarak azalma tespit edilmiştir(p:0,0001). Sıvı cevapsız grupta parametreler arasında anlamlı fark bulunamamıştır. Sonuç: Dinamik parametreler olan dİVC ve PVI' ın non- invaziv, kolay uygulanılabilir olması, sıvı cevabını öngörmede ve hedefe yönelik sıvı tedavisini yönetmede invaziv ölçümlerle korele sonuçlar vermesi nedeniyle rutin pratikte daha sık kullanılması gerektiği kanaatine vardık. Ayrıca gündelik pratikte çok sık uygulanmayan pasif bacak kaldırma ve ekspiryum sonu oklüzyon testi yöntemiyle yeterli miktardaki sıvının hızla ve geri dönüşümlü olarak santral kompartmana geçirilebildiğini ve ciddi volüm kaybı durumlarında ilk basamak olarak düşünülen sıvı replasmanı yerine bu testlerin daha sıklıkla klinik rutine alınmasını önermekteyiz.

Objective: Accurate and rapid determination of fluid balance is of great importance in intensive care patients. There is no consensus on how to determine fluid balance best and it is seen that different methods have been used in the literature. Therefore, our study aimed to investigate the correlation of noninvasive methods of pleth variability index (PVI), inferior vena cava distensibility index (dIVC), passive leg raising (PLR) test and end-expiratory occlusion test (EEOT) measurements with invasive PiCCO cardiac index (CI) and global end-diastolic volume index (GEDI) measurements in patients hospitalized in general intensive care unit. Thus, it was aimed to predict responsiveness to fluid therapy in the fastest way using noninvasive methods in intensive care unit. Materials and Methods: In the supine position, ECG, SpO2, PiCCO femoral intraarterial pulse pressure monitoring and temperature monitoring were performed on 68 patients aged between 18-90 years who were randomly selected in ICU. A three-way central catheter was inserted using the Seldinger technique. CVP was measured via CVP monitoring; a CVP value of <8 mmHg was evaluated in favor of hypovolemia. In the same position, IVC imaging was performed using a ECHO probe of ultrasonography device and dIVC was calculated, and a dIVC value of >18% was evaluated in favor of hypovolemia. At the same time, PVI monitoring was performed on the patient using a finger probe taking measurement with the Massimo technique. A PVI value of >14% was evaluated in favor of hypovolemia. Pulse contour cardiac index (PCCI) and pulse pressure-controlled PLR test were applied to the patient, and a PCCI of 12% and a 12% increase in systolic pressure were considered positive. While carrying out PLR test, dIVC, PVI and CVP measurements were repeated. The patient was then placed in the supine neutral position and EEOT was performed, and a PCCI of >5% and a >5% increase in systolic blood pressure were considered positive, and simultaneous CVP and PVI values were recorded. Results: In our study, it was found that there was a statistically strong positive correlation between CI and GEDI and PLR test (p:0.0001), a strong positive correlation between CI and GEDI and EEOT, and a moderate positive correlation between CI and GEDI and CVP, dIVC, PVI (p:0.0001). Noninvasive measurements of PLR test showed the highest correlation with a sensitivity of 90% and a specificity of 89%, while CVP showed the lowest correlation with a sensitivity of 87% and a specificity of 65%. At the same time, the group responsive to fluid therapy had a statistically significant decrease in PVI (p:0.001/PLR test, p:0.003/EEOT), a statistically significant increase in CVP (p:0.0001/PLR test, p:0.001/EEOT), and a statistically significant decrease in dIVC (p:0.0001) after PLR test and EEOT. There was no significant difference between the parameters in the group irresponsive to fluid therapy. Conclusion: We concluded that dynamic parameters of dIVC and PVI should be used more frequently in routine practice because of being non-invasive, easy-to-use, and producing correlated results with invasive measurements in predicting fluid response and managing goal-directed fluid therapy. In addition, we suggest that the methods of passive leg raising and end-expiratory occlusion test , which are not frequently used in daily practice, allow a sufficient amount of fluid to be rapidly and reversibly transferred to the central compartment, and that these tests should be included in clinical routine practice instead of fluid replacement considered as the first-line therapy in cases of severe volume loss .
