Özet
Amaç: Prematüre bebeklerde metabolik kemik hastalığı sıklığı gebelik haftasının düşüklüğüne paralel olarak artar. Metabolik kemik hastalığı etiyolojisinde pek çok neden rol oynasa da, en önemli nedenler protein ve minerallerin eksikliği olarak bilinmektedir Kemik mineralizasyonunda rol oynayan D vitamini, kalsiyum, fosfor, magnezyum ve çinko gibi mineraller gebeliğin özellikle son trimesterinde plasental yolla fetusa geçer ve depolanır. Son trimestiri tamamlamadan doğan geç preterm bebeklerde (340/7- 366/7 gebelik haftası) MKH gelişebileceği öngörülebilir. Özellikle stabil ve gestasyon haftası 35 haftanın üzerinde olan bebekler anne yanında izlenmekte, metabolik kemik hastalığına yönelik taranmadan taburcu edilmektedirler. Bu bebeklerde MKH sıklığı bilinmemektedir. Bu nedenle çalışmamızın amacı geç preterm bebeklerde kemik mineralizasyonunu ortaya koymak ve metabolik kemik hastalığı sıklığını belirlemektir. İkincil hedefimiz ise geç pretermlerdeki D vitamini ve mineral eksikliğinin annenin D vitamini ve mineral eksikliği ile ilişkisini ortaya koymaktır. Gereç ve Yöntem: Haziran 2018- Mayıs 2019 tarihleri arasında doğan geç preterm (gebelik haftası:34 0/7-36 6/7) AGA bebekler ve anneleri (n:31) ile term AGA (gebelik haftası: 38 0/7- 40 6/7) bebekler (n:39) çalışmaya dahil edildi. Bilinen doğumsal kalp hastalığı, kromozom anomalisi, asfiktik doğum hikayesi, intrauterin gelişme kısıtlılığı olan bebekler, pre-eklamptik, koryoamniyonitli anne bebekleri ve herhangi bir kronik hastalığı olan annelerin bebekleri çalışma dışında bırakıldı. Çalışmaya dahil edilen bebeklere ve annelerine ait bilgileri içeren form dolduruldu ve tüm bebeklerin fizik muayeneleri yapılarak, antropometrik ölçümleri kayıt edildi. Term ve geç preterm bebeklerden doğum anında umbilikal kord kanından kan örneği alındı. Geç preterm bebeklerin annelerinden de doğum öncesi serum örnekleri alındı. Geç preterm bebeklerden posnatal 4. ayda serum örneği alındı ve tüm örneklerden Ca, P ve Mg düzeyleri, ALP aktivitesi, 25(OH)D vitamin düzeyleri, osteokalsin ve PTH düzeyleri çalışıldı. Geç pretermlerde kord kanı, term bebeklerde ve geç pretermler posnatal 4. ayda sırasıyla serum Ca <6,2 mg/dl, <7,6 mg/dl, <9 mg/ dl olması hipokalsemi, P<4,5 mg/dl, <4,5 mg/dl, <4 mg/dl olaması hipofosfatemi, tüm bebekler için Mg düzeyinin <1,5 mg/dl olması hipomagnezemi olarak belirlendi. İstatiksel değerlendirmede IBM SPSS 17.0 istatistik programı kullanıldı. Bulgular: Çalışmaya dahil edilen geç pretermlerin ortalama gebelik haftası ve doğum ağırlığı sırasıyla 35,8±0,7 hafta ve 2704,0 ± 444,8 gram bulunurken, term bebeklerin ortalama gebelik haftası ve doğum ağırlığı sırasıyla 39,0 ± 0,8 hafta ve 3280,1 ± 331,4 gram olarak saptandı. Geç preterm ve term bebeklerin gestasyon haftaları ve doğum ağırlıkları ortalamaları karşılaştırıldığında, aralarında anlamlı fark saptandı (p<0,001, p<0,001) Geç pretermlerin %64,5 kız iken, term bebeklerde bu oran %36 idi ve aralarındaki fark istatistiksel olarak anlamlı idi (p=0,017). Sezaryen (C/S) doğum oranı term bebeklerde %67,7 iken, geç pretermlerde bu oran %82 olarak saptandı. Geç preterm bebeklerin postnatal dördüncü ay ortalama vücut ağırlığı, boy ve baş çevreleri sırasıyla 6518,0 ± 752,3 gram, 62,7 ± 2,7 cm ve 41,2 ± 1,3 cm olarak saptandı. Bu değerler Prof. Dr. Olcay Neyzi tarafından Türk çocukları için hazırlanan persantil eğrilerinde değerlendirildiğinde, hepsinin büyümeyi yakaladığı saptandı. Geç pretermlerin ilk 4 ay boyunca anne sütü ağırlıklı beslendikleri öğrenildi. Geç preterm bebeklerin umbilikal kord kanı Ca, P ve Mg düzeylerinin ortalaması sırasıyla geç pretermlerde 9,40±0,98 mg/dL, 5,20±1,00 mg/dl, 1,95±0,64 mg/dl, termlerde ise 9,72±,69 mg/dl, 5,12±0,55 mg/dl, 1,95±0,64 mg/dl saptandı. Her iki grupta hipokalsemi saptanmaz iken, geç pretrem bebeklerin %19'unda (n:6) hipofosfatemi, %16'sında (n:4) hipomagnezemi, term bebeklerin ise %11'inde (n:4) hipofosfatemi, %26'sında (n:10) hipomagnezemi saptanmış olup, gruplar arasında istatistiksel anlamlı fark bulunmadı. Geç preterm ve term bebeklerin umbilikal kord kanı Ca, P, Mg, ALP, D vitamini, PTH ve osteokalsin değerleri karşılaştırıldığında Ca, P, Mg, PTH ve osteokalsin düzeylerinde istatistiksel olarak anlamlı fark bulunmadı. Geç preterm bebeklerin ALP değeri (150,1 ± 46,1 U/L) term bebeklere göre (124,1 ± 32,6 U/L) istatistiksel olarak anlamlı düzeyde yüksek bulunsa da, normal sınırlar içinde idi. D vitamini düzeyleri karşılaştırıldığında, geç preterm bebeklerde (18,4 ± 10,2) term bebeklere göre (27,1 ± 8,3) anlamlı daha düşük saptandı (p<0,001). Geç preterm bebeklerin annelerinde doğumdan hemen önce bakılan D vitamini değeri ortalaması 13,74 ± 16,49 µg/L bulundu. Annelerin yaklaşık %87'sinde (n:27) D vitamini eksikliği (<20 µg/L) bulunurken, %3'ünde (n:1) D vitamini yetersizliği (20-30 µg/L) ve %10'unda (n:3) ise D vitamini düzeylerinin yeterli olduğu saptandı. Tüm annelerin serum kalsiyum düzeyleri normal bulunurken, annelerin %45'inde (n: 14) hipofosfatemi saptandı. Ortalama Mg düzeyi 1,83±,73 mg/dl saptanmış olup, 10 annede (%32,26) hipomagnezemi olduğu bulundu. Geç preterm bebeklerin 4.ay Ca, P, Mg ortalaması sırayla 10,3 ± 0,68 mg/dl, 5,5 ± 0,60 mg/dl, 2,14±,020 mg /dl olup değerlerinin tümü normal sınırlar içinde idi; sadece bir olguda sınırda ALP yüksekliği (ALP: 467 U/L) saptandı. Geç preterm bebeklerin umbilikal kord ve 4. ay Ca, P, Mg, ALP, D vitamini, PTH ve osteokalsin düzeyleri karşılaştırıldığında Ca, Mg, Alkalen fosfataz ve D vitamini düzeylerinin postnatal 4. ayda umbilikal kord kanındaki değerlere göre anlamlı düzeyde artmakla birlikte, bu değerlerin normal referans aralığında olduğu görüldü (p=0,001, p=0,010, p<0,001). Kordon kanında D vitamini yeterli olan geç pretermlerin sayısının (n:5), postnatal 4. ayda (n:14) istatistiksel olarak anlamlı ölçüde arttığı saptandı (p: 0,003). Tüm bebeklerin düzenli olarak D vitamini kullandığı öğrenildi. Sonuç: Çalışmamızda geç preterm bebeklerde kemik mineralizasyonu ve MKH tanısında kullanılan biyokimyasal belirteçleri değerlendirdiğimizde, doğumda term bebeklerdekine yakın değerler elde edilmiştir. Geç pretermlerin 4.ay değerlendirmesinde de term bebekler için literatürde bildirilen değerlere yakın değerler saptanmıştır. Bu sonuçların ışığında, vaka sayımız kısıtlı olmakla birlikte, ek risk faktörleri olmayan geç pretermlerin rutin olarak kemik mineralizasyonlarının değerlendirilmesine yönelik taranmaları gerektiği söylenemese de, kesin bir öneride bulunmak için radyolojik yöntemlerin de kullanıldığı ve daha fazla sayıda olgu içeren, uzun dönem izlemin yapıldığı çok merkezli çalışmalara halen gereksinim vardır.

Objective: The incidence of metabolic bone disease (MBD) of prematurity increases at younger gestational ages. Although many factors are involved in the etiology of metabolic bone disease, deficiencies of protein and minerals are known as the most important causes. Vitamin D, calcium, phosphorus, magnesium and zinc which are involved in bone mineralization, are transmitted by the placenta to the fetus and stored, especially in the last trimester of pregnancy. It can be predicted that MBD may develop in late preterm babies who are born before completing the last trimester (34 0/7- 36 6/7 gestational week). Especially, stable babies with a gestational age above 35 weeks are being followed up at the side of their mothers and being discharged without screening for metabolic bone disease. The frequency of MBD is not known in these babies. Therefore, in this study, we primarily aimed to demonstrate bone mineralization and to determine the incidence of metabolic bone disease in late preterm babies. Our secondary objective was to demonstrate the relationship of vitamin D and mineral levels in late preterm babies and their mothers. Material and Method: Late preterm AGA babies (34 0/7-36 6/7 gestational weeks) born between June 2018 and May 2019 and their mothers (n:31), and term AGA (38 0/7- 40 6/7 gestational weeks) babies (n:39) were included in the study. Babies who had known congenital heart disease, chromosomal anomaly, history of birth asphyxia and intrauterine growth retardation, and babies of the mothers who had pre-eclampsia, chorioamnionitis and any chronic disease were excluded from the study. The forms including the data of the babies and mothers who were included in the study, were filled in, physical examination was performed in all babies and the anthropometric measurements were recorded. Umbilical cord blood samples were obtained at the time of delivery from the babies who were included in the study. Serum samples were obtained before delivery from the mothers of late preterm babies. Serum samples were obtained from the late preterm babies also at the postnatal 4th month and Ca, P and Mg levels, ALP activity, 25(OH) D vitamin, osteocalcin and PTH (parathyroid hormone) levels were measured in all samples. Hypocalcemia and hypophosphatemia were defined as a calcium and phosphor level below 6,2 / 4.5 mg/dl and 7,6 / 4.5mg/dL in late preterm and term babies, respectively. At the postnatal 4th month calcium level below 9 and apd a phosphor level below 4mg/dl was considered low for late preterms. In all babies, a Mg level less than1,5 mg/dl was considered hypomagnesaemia SPSS 17.0 statistical program was used in statistical analysis of the data. Results: The mean gestational week and birth weight in the late preterm babies who were included in the study were found to be 35,8±0,7 weeks and 2704,0 ± 444,8 grams, respectively, whereas the same values were found to be 39,0 ± 0,8 weeks and 3280,1 ± 331,4 grams, respectively, in the term babies. A significant difference was found between the late preterm babies and term babies in terms of mean gestational week and mean birth weight (P<0,001, P<0,001). In the late preterm group, 64,5% of the babies were female, while this rate was found to be 36% in the term babies; the difference was statistically significant (p=0,017). The rate of cesarean section (C/S) delivery was found to be 67,7% in the term babies and 82% in the late preterm babies. The mean body weight, height and head circumference at the postnatal 4th month in the late preterm babies were found to be 6518,0 ± 752,3 grams, 62,7 ± 2,7 cm and 41,2 ± 1,3 cm, respectively. When these values were evaluated using the percentile curves prepared by Professor Olcay Neyzi for the Turkish children, it was found that they all fulfilled catch-up growth. It was learned that the late preterm babies were predominantly fed with breast milk in the first 4 months. When the umbilical cord bloods of the late preterm babies were evaluated, the mean Ca, P and Mg values were found to be 9,40±0,98 mg/dL, 5,20±1,00 mg/dl and 1,95±0,64 mg/dl, respectively. In the term babies, the mean Ca, P and Mg values were found to be 9,72±,69 mg/dl, 5,12±0,55 mg/dl and 1,95±0,64 mg/dl, respectively. Hypocalcaemia was not found in either group. Hypophosphatemia was found in 19,35% (n:6) of the late preterm babies and hypomagnesaemia (n:4) was found in 16,13%. In the term babies, hypophosphatemia was found with a rate of 10,53% (n: 4) and hypomagnesaemia was found with a rate of 25,64% (n:10). A statistically significant difference was not found between the groups. When the umbilical cord blood Ca, P, Mg, ALP, vitamin D, PTH and osteocalcin values in the late preterm and term babies were compared, a statistically significant difference was not found in terms of Ca, P, Mg, PTH and osteocalcin levels. Although the ALP value in the late preterm babies was found to be statistically significantly higher (150,1 ± 46,1 U/L) compared to the term babies (124,1 ± 32,6 U/L), it was within the normal limits. When vitamin D level was evaluated, it was found to be significantly lower in the late preterm babies (18,4 ± 10,2) compared to the term babies (27,1 ± 8,3) (p<0,001). The mean vitamin D value in the mothers of late preterm babies measured immediately before delivery was found to be 13,74 ± 16,49 µg/L. Vitamin D deficiency (<20 µg/L) was found in approximately 87% of the mothers (n:27), vitamin D insufficiency (20-30 µg/L) was found in 3% (n:1) and sufficient vitamin D level was found in 10% (n:3). The serum calcium levels were found to be normal in all mothers, were as hypophosphatemia was found in 45,16% (n:14). The mean Mg level was found to be 1,83±,73 mg/dl and hypomagnesaemia was found in 10 mothers (32,26%). The mean Ca, P and Mg values at the 4th month in the late preterm babies were found to be 10,3 ± 0,68 mg/dl, 5,5 ± 0,60 mg/dl and 2,14±,020 mg /dl, respectively, and all were within the normal limits; borderline ALP elevation (ALP: 467 U/L) was found only in one subject. When the umbilical cord and 4th month Ca, P, Mg, ALP, vitamin D, PTH and osteocalcin levels were compared in the late preterm babies, it was observed that the Ca, Mg, ALP and vitamin D levels at the postnatal 4th month were significantly higher compared to the values found in the umbilical cord blood, but they were within the normal reference range (p=0,001, p=0,010, p<0,001). It was found that the number of late preterm babies with sufficient vitamin D in the cord blood (n:5) statistically significantly increased at the postnatal 4th month (n:14) (p: 0,003). Conclusion: When we evaluated bone mineralization and the biochemical markers that are used in the diagnosis of MBD in our study, the values we obtained at birth were found to be close to the ones found in term babies. In the 4th month assessment of the late preterm babies, we found values close to the ones reported in the literature for term babies. These results revelaed that late preterm babies who are breastfed and receive appropriate prophylaxis for vitamin D, did not display the signs of MBD or mineral deficiency. Unless they have other risk factors such as prolonged hospitalization and the use of certain drugs, such as diuretics, steroids and aminoglycoside, they do not have to be screened routinely for MBD of prematurity. The limitation of our study is that we were unable to use Dexa because of ethical reasons due to X-ray exposure, which is the gold standart for the diagnosis of MBD. In the light of these results that we obtained in our study which had a limited number of subjects, multi-center studies with long-term follow-up periods including a higher number of subjects and using also radiological methods, still need to be conducted to make definite recommendations, though we can not state that routine screenings directed to assessment of bone mineralization are required in late preterm babies who do not have additional risk factors.
