Özet

Introduction and background: The inverse relationship between iron status and obesity, contributes to the global double burden of malnutrition. The aim of this research was to investigate the potential links between obesity and iron deficiency (ID), particularly in women of child-bearing age. Objectives: To assess the effectiveness of interventions designed to address ID in women who are either overweight or obese, to explore the effectiveness of interventions designed to reduce inflammation, to identify any links between BMI, iron status, fatigue level and diet and to increase depth of information available regarding the fatigue experience. Methods: A mixed methodological approach was undertaken with two systematic style reviews, two quantitative studies and one qualitative study. The reviews were designed to fulfil the first two objectives with data reported using the PRISMA framework for transparency. In the first quantitative study new Slimming World (SW) female members were invited to take part in an online survey including validated tools to determine levels of iron intake and fatigue levels. The second quantitative study was modelled on the first study, but with cultural/practical adjustments and the availability of iron-related blood parameters. Turkish women, who consulted a registered dietitian to lose weight, were invited to take part in a paper-based survey. A qualitative study compromised of two methodological phases with a focus group and telephone interviews conducted. Results: The first review found that although intervention trials which assess iron status are available, to date, no study has been undertaken where the primary aim is to improve iron status of women who are obese. Effective interventions included additional dietary iron in a very low energy diet, higher protein and energy restriction, breakfast cereals as part of a weight loss intervention and exercise training. The second review found that weight loss may be associated with a decline in some biomarkers of inflammation, such CRP, in adult women who are obese. Also, a higher protein, brown rice, aerobic exercise, histidine supplementation, and tomato juice were found to be effective dietary strategies in reducing inflammation with or without weight loss. The first quantitative study found that SW is an effective weight management programme in helping to achieve reductions in BMI, fatigue levels and consumption of energy-dense foods, and increasing PA levels and consumption of high-fibre/antioxidant foods. The second quantitative study further supported Study 1 and additionally the relationship between iron status and low-grade inflammation in women who are obese was reported. The qualitative study suggests most women in the study believed that fatigue had negative effects on their perceived ability to make lifestyle changes. Conclusion: Weight management has a positive impact on reducing inflammation, improving iron status, fatigue level, quality of life and motivation/ability to make lifestyle changes in women of reproductive age who are obese.
