NLO'nun, sistemik komplikasyon gelişimi için ROC eğrisi kullanılarak elde edilen cut-off değeri > 7,13 idi ( Sensitivite:87,50 spesifite:69,05 AUC:0,81 CI% 0,72-0,88 p<0,0001). 48. saatte bakılan NLO'nun, sistemik komplikasyon gelişimi için ROC eğrisi kullanılarak elde edilen cut-off değeri > 6,2 idi ( Sensitivite:93,75 spesifite:88,10 AUC:0,93 CI% 0,86-0,97, p<0,0001).
Objective : The aim of this prospective study was; To investigate the relationship between neutrophil / lymphocyte ratio (NLR), serum procalcitonin and lactate levels and disease severity and prognosis in patients with acute pancreatitis (AP) diagnosed in the emergency department. Method: In this prospective study, 100 patients with AP who met the inclusion criteria were included in the study regardless of their etiology. The NLO, procalcitonin and lactate levels of the patients included in the study were compared with the Revised Atlanta Classification, admission and 48th hour Ranson score and CTSI scores. In addition, the relationship between the duration of hospitalization, need for intensive care, surgical treatment requirements, complications and NLR, serum procalcitonin and lactate levels were investigated. Results : In our study, the mean age of the patients was 58.55 ± 18.42 years. Sixty (60%) of the cases were female and 40 (40%) were male. There was a statistically significant difference between the procalcitonin levels, lactate levels, NLR at admission and at hour 48 between the mild and severe AP groups formed according to the revised Atlanta classification (p<0,01). There was a statistically significant difference between the procalcitonin levels, lactate levels, NLR at admission and at hour 48 between the mild and severe AP groups formed according to the Ranson score (p<0,01). We found a positive correlation between serum procalcitonin, lactate and NLR at the time of admission and 48th hour and duration of hospitalization (p <0.01). The cut off value of NLR calculated using the ROC curve for systemic complications is > 7.13 (sensitivity: 87.50, specificity: 69.05, AUC: 0.81 CI % 0.72-0.88 p<0.0001). The cut-off value of the NLR measured at 48 h calculated using the ROC curve for the development of systemic complications was> 6.2 (Sensitivity: 93.75, specificity: 88.10, AUC 0.93, CI % 0.86-0.97, p<0.0001). Conclusion: In our study; We found that blood procalcitonin level, arterial blood gas lactate level and NLR both correlated positively with scoring systems and showed the severity of the disease. Furthermore, we consider that NLR and 48th hour NLR may be an important predictor of systemic complications. Blood procalcitonin and lactate levels and NLR can be used to determine the severity and prognosis of AP.
