Özet
Stomatognatik sistemde bulunan oklüzyon ve dişler, çiğneme kasları, tükürük bezleri, alveolar kemik ve temporomandibular eklem (TME) nöromuskuler sistemle bir bütün halinde çalışmaktadır. Periodontal desteğin kaybedilmesi sebebiyle azalan mekanoreseptörler oklüzal yükün karşılanmasında yetersiz kalmakta ve bu da kısıtlı bir şekilde gerçekleşen geri bildirim mekanizması sebebiyle çiğneme kaslarının işlevleri ve yapılarını etkilemektedir. Periodontitis tablosundaki hastalarda ayrıca dişleri çevreleyen kemiğin kaybı arttıkça özellikle anterior dişlerde meydana geldiği bilinen patolojik migrasyon ile oluşan protrüzyon ile anterior kesici yolu eğimi etkilenir ve ayrıca oklüzal eğimde de değişiklikler meydana gelir. Çene kompozisyonundaki bu farklılıklar, TME fonksiyonu açısından belirleyicidir. Tüm bu bilgilerin ışığında periodontitis tablosunun ortaya çıkardığı sonuçların ve eklem içerisine iletilen orantısız yüklerin direk veya dolaylı olarak nöronal sirkülasyonun bozulması ve eklemin fonksiyonel mekanizmasının hücresel seviyede etkilenmesi ile TME içi dokularda hasar ile sonuçlandığı hipotezi kurulmuştur. Çalışmamızda bu hipotez periodontitis olan hastalar ile herhangi bir krestal kemik rezorbsiyonu gözlenmeyen kontrol grubundaki bireylere ait mandibular kondillerin panoramik radyografi üzerinden fraktal analiz yöntemi ile değerlendirilmesi ve bu mandibular kondillerin yine panoramik radyografi üzerinde saptananabilecek osteofit, düzleşme, konkavite, erozyon, skleroz ve subkortikal kist başlığındaki osteoartritik değişikliklerinin Muir&Goss skalası temel alınarak derecelendirilmesi ile irdelenmiştir. Occlusion and teeth in the stomatognathic system, chewing muscles, salivary glands, alveolar bone and temporomandibular joint (TMJ) work together with the neuromuscular system. The decreased mechanoreceptors due to the loss of periodontal support are insufficient to meet the occlusal load and this affects the functions and structures of the masticatory muscles due to the limited feedback mechanism. In patients with periodontitis, as the loss of the bone surrounding the teeth increases, the anterior inclination of the incisor is affected by protrusion, which is known to occur especially in the anterior teeth, and also changes in the occlusal slope. These differences in jaw composition are decisive for TMJ function. In the light of all this information, it has been hypothesized that the results of the periodontitis and disproportionate loads transmitted into the joint result in damage to the tissues within the TMJ by directly or indirectly impairing neuronal circulation and affecting the functional mechanism of the joint at the cellular level. In our study, this hypothesis was tested on the mandibular condyles of patients with periodontitis and control group without any crestal bone resorption were evaluated by fractal analysis method on panoramic radiography and osteophyte, flattening, concavity, erosion, sclerosis and subcortical cyst in the head of these mandibular condyles that could be detected on panoramic radiography.
