Özet

Bu araştırma mastektomili kadınların beden algısı ve cinsel uyumlarını etkileyen faktörlerin belirlenmesi amacıyla tanımlayıcı ve kesitsel türde yapıldı. Araştırmanın evrenini İstanbul Eğitim ve Araştırma Hastanesinde mastektomi ameliyatı olmuş hastalar, örneklemini ise İstanbul Eğitim ve Araştırma Hastanesi'nin Tıbbi Onkoloji Kliniğine Bağlı Gündüz Kemoterapi Ünitesine 1 Mayıs- 15 Temmuz 2019 tarihleri arasında başvuran mastektomili 167 kadın hasta oluşturdu. Araştımanın verileri Sosyo-demografik ve Klinik Özellikler Formu, Cinsel Uyum ve Beden İmajı Ölçeği kullanılarak toplandı. Araştırma verilerinin değerlendirilmesinde, sayı-yüzdelik hesaplamaları, Shapiro-Wilk testi, t testi, Mann-Whitney U testi, Kruskal Walls testi, Pearson korelasyon analizi ve Dunn-Bonferroni post-hoc testi kullanıldı. Araştırmaya katılan kadınların yaş ortalamasının 47,15±10,15 yıl olduğu, %88'inin evli, %61,1'inin ilköğretim mezunu, %60,5'inin gelirinin giderine denk olduğu, %62,3'ü kanserin II evresinde olduğu, %99,4'ünün kemoterapi aldığı, %65,9'unun hormonterapi aldığı, %56,3'ünün radyoterapi aldığı, %23,4'ünün total mastektomi ameliyatı olduğu bulundu. Kadınların Beden İmajı Ölçeği Önceki Beden İmajı alt boyut puan ortalamasının 9.50±2.05, Sonraki Beden İmajı alt boyut puan ortalamasının 5.45±2.50 olduğu saptandı. Araştırma kapsamına alınan kadınların Cinsel Uyum Ölçeği Önceki Cinsel Uyum alt boyut puan ortalamasının 6,77±1,41, Cinsel Fonksiyon Üzerine Etkisi alt boyut ortalamasının -4,10±2,15 ve Memelerin Cinsellikteki Rolü alt boyut ortalamasının 8,67±1,48 olduğu bulundu. Bu araştırmanın sonucunda kadınların önceki beden imajlarınn olumlu olduğu, önceki cinsel uyumu ve memelerin cinsellikte rolünün iyi olduğu bulundu. Kadınların sonraki beden imajlarının olumsuz olduğu ve mastektominin cinsel fonksiyon üzerine etkisinin kötü olduğu bulundu. Araştırmanın farklı değişkenler ile daha büyük örneklem grubunda tekrarlanması önerilmektedir.

The study was carried out in complemantary and cross-sectional type in order to determine the factors affecting body image and sexual adjustment of women with mastectomy. The research universe consisted of patients who underwent mastectomy surgery at the İstanbul Training and Research Hospital and the sample group consisted of 167 women who underwent a mastectomy operation and then attend to the day chemotherapy unit of the Istanbul Training and Research Hospital oncology clinic between May 1st and July 15th 2019. Research data was collected using ' socio-demographic and clinical features form' and 'sexual adjustment and body image scale'. Normal percentile calculations, Shapiro-Wilk test, t test, Mann-Whitney U test, Kruskal Walls test, Pearson correlation analysis and Dunn-Bonferroni post-hoc test were used to evaluate the research data. The average age of the women participating in the study was 47.15 ± 10.14 years. 88% were married, 61.1% were primary school graduates and 60.5% had income that was equivalent to their expenses. 62.3% of the women participating in the study were in the second stage of cancer. 99.4% received chemotherapy, 65.9% received hormonetherapy, 56.3% received radiotherapy and 23.4% of them underwent a total mastectomy. The sub-dimension average score of the women for the Body Image prior to mastectomy based on the Body Image Scale was found to be 9.50 ± 2.50, while their sub-dimension average score for the Body Image in post-mastectomy was found to be 5.45 ± 2.50. The sub-dimension average score of the women for the previous sexual adjustment based on the Sexual Adjustment Scale was found to be 6.77 ± 1.41, the sub-dimension average score for the Influence on Sexual Function was found to be -4.10 ± 2.15. sub-dimension average score for the Sexual Importance of Breasts was 8.67 ± 1.48. As a result of the research, it was found that women's previous body images were positive, their previous sexual adjustment and the role of breasts in sexuality were good. After mastectomy, body image of women was negative and the effect of mastectomy on sexual fuction was bady. The study recommended to repeat in different sample groups with different variables.
