Background The diagnosis of advanced cancer does affect not only the patient but also family members of the patient. A number of studies demonstrated that the diagnosis of advanced cancer adversely affects family functioning. Expressiveness, cohesiveness and conflict resolution are mostly affected as the dimensions of family functioning. Although families facing cancer need an intervention aiming to improve family functioning during palliative care and bereavement, family functioning has been neglected by therapeutic approaches used during cancer care. Thus, family-focused grief therapy emerged as a preventive intervention for families whose functioning is at-risk due to the demanding nature of cancer care. Aim This systematic review aims to systematically identify and evaluate studies examining the effectiveness of family-focused grief therapy on family functioning and its dimensions which are expressiveness, cohesiveness, conflict resolution for families with an advanced cancer member. Methods The following electronic databases were searched by using pre-determined terms: MEDLINE, PsycINFO and Scopus. The search was conducted in May 2020. Out of the 2519 papers obtained from the search, nine studies met selection criteria. A grey literature search was conducted using the BASE, British Library, Open Grey to find more studies. However, there was no additional study found. Main results 4 Nine studies involving 440 families with an advanced cancer member were included in this review. It is evident that family-focused grief therapy is beneficial in increasing expressiveness. However, whether or not family-focused grief therapy is effective on family functioning remained unclear because contradictory results emerged from the selected studies. Conclusion Families deteriorated in expressiveness, cohesiveness, and conflict together did not benefit from family-focused grief therapy while therapy was beneficial for the families deteriorated in only one of the dimensions of family functioning.
