Özet

6. ÖZET RA'in en sık görülen göz belirtisi KKS' dır. Çalışmada RA'li 38 hastanın 76 gözünde KKS araştırıldı. Araştırmada kullanılan gözyaşı fonksiyon testleri Schirmer-1, GPZ ve RB boyama testlerinden oluşuyordu. Bu testler, laboratuvar koşullarına ihtiyaç duyulmadan kolayca uygulanabilen klinik testlerdir. Çalışmada RA'li hastaların %39.47' sinde KKS tespit edildi. KKS saptanan hastalarda testlerin tanı duyarlılığı araştırıldı. Ayrıca 23 kişilik kontrol grubunun 46 gözünde testlerin tanı duyarlılığı araştırıldı. RA'li hastalar ve kontrol grubu arasında, yaş ve cins dağılımı açısından istatistiksel olarak anlamlı bir fark yoktu (p>0.05). GPZ testi %90 ile tamsal duyarlılığı en yüksek test olarak bulunurken, spesiflte oranı %76.08 ile diğer iki testten anlamlı olarak daha düşüktü (p<0.05). Spesifitesi en yüksek olan test %95.65 ile RB boyama testi idi. Fakat bu testin de tamsal duyarlılığı %66.66 ile diğer iki testten anlamlı olarak daha düşüktü (p<0.05). Testlerin kombine olarak kullanılmasıyla yüksek duyarlılık ve spesiflte oluşturulmakta ve bu da doğru tanıya gidilmesinde önemli bir etken olmaktadır. 21

7. SUMMARY The most common ophthalmological sign of RA is KCS. In this study, KCS was investigated in 76 eyes of 38 patients with RA. The tear function tests used in this investigation were Schirmer-1, tear film break up time and RB staining tests. These tests were carried out easily in the outpatient service, without the aid of laboratory conditions. In this study, KCS was diagnosed in 39.47 % of the patients with RA and the diagnostic sensitivity of these tests were investigated in KCS. In addition, the spesificity ratio of the tests were determined in 46 eyes of 23 subjects compromising the control group. In terms of age and sex distribution, the difference between the patients with RA and control group was not statistically significant (p>0.05). Tear film break up time test was the most sensitive test with 90% sensitivity, while its specificity with 76.08% was significantly lower than that of the other two tests (p<0.05). The most spesific test with 95.65% specificity was the RB staining test. However, the 66.66% sensitivity of this test was significantly lower than that of the other two tests (p<0.05). With combined usage of these tests, higher specificity and sensitivity can be obtained and this is an important leading factor for accurate diagnosis. 22
