Özet

Bu çalışmanın amacı, dominant ve nondominant taraf alt ekstremite protezi kullanan amputelerde fonksiyonelliği, yaşam kalitesini ve enerji tüketimini karşılaştırmaktı. Çalışmaya Bilim Ortez Protez Merkezi'ne ve Dinamik Ortez Protez Merkezi'ne gelen 18-65 yaş aralığında erkek ve unilateral diz altı amputasyonuna sahip olan, 10 dominant taraf ampute ve 9 nondominant taraf ampute gönüllü dahil edildi. Enerji tüketimi ve fonksiyonel kapasite için 6 Dakika Yürüme Testi (6DYT) ve Zamanlı Kalk ve Yürü Testi (ZKYT) kullanıldı. Denge değerlendirilmesi; statik denge için Tek Ayak Üzerinde Durma Testi, dinamik denge için Y Denge Testi (YDT) kullanıldı. Yaşam kalitesi ve protezin çok yönlü değerlendirilmesi amacıyla, Trinity Amputasyon ve Protez Deneyim Ölçeği (TAPES) kullanıldı. Elde edilen verilere göre; 6 Dakika Yürüme Testinde nondominant taraf ampute grubunda yürüme mesafesi daha uzun bulundu (p<0,05). Zamanlı Kalk ve Yürü Testi açısından gruplar arasında fark bulunmadı (p>0,05). Statik denge yönünden fark bulunamasada, dominant taraf alt ekstremite amputelerinin, nondominant alt ekstremite amputelerine kıyasla protezli ayak üzerinde daha kısa sürede durdukları gözlendi (p>0,05). Dinamik denge değerlendirmesinde gruplar arasında fark bulunmadı (p>0,05). TAPES değerlendirme sonuçlarında da benzer şekilde, fark bulunmadı (p>0,05). Bu çalışma nondominant taraf alt ekstremite protezi kullanan amputelerin fonksiyonel kapasitesinin dominant taraf ampute grubuna kıyasla daha yüksek olduğunu göstermiştir. Bu çalışma, ampute rehabilitasyonuna ve protez tasarımında dominant ve nondominant taraf amputelerin fonksiyonellik açısından farklılık gösterebileceğini göstermiştir. Farklı amputasyon seviyelerinin de içinde yer alacağı çalışmaların yol gösterici olacağı görüşünü taşımaktayız.

The aim of this study was to compare of functionality, quality of life and energy consumption in amputees using dominant and nondominant side lower limb prosthesis. 10 dominant side amputees and 9 nondominant side amputees who is a man and who has unilateral below knee amputation between the ages of 18-65, who came to the Bilim Orthotics and Prosthetics Center and Dinamik Orthotics and Prosthetics Center were voluntarily included in Six-Minute Walk Test (6MWT) and Timed Up & Go Test (TUG) that were conducted for energy consumption and functional capacity. Balance assessment; Single Leg Stance Test for the static balance, Y Balance Test was conducted for the dynamic balance. Trinity Amputation and Prosthesis Experiences Scales was conducted for the purpose of the quality of life and the multifaceted assessment of the prosthesis. According to the data obtained; the walking distance was found to be longer for the nondominant side group in the Six-Minute Walking Test (p<0.05). There was no difference between the groups in terms of Timed Up & Go Test (TUG) (p>0.05). It was observed that the dominant side lower extremity amputees stood on prosthetic legs in a shorter time compared to the nondominant lower extremity amputees, even if there was no difference in terms of static balance (p>0.05). There was no difference between the groups in the dynamic balance assessment (p>0.05). Similarly, no difference was found in the TAPES evaluation results (p>0.05). This study demonstrated that functional capacity of amputees using nondominant side lower extremity prosthesis was higher compared to the dominant side amputee group. This study demonstrated that dominant and nondominant side amputees may differ in terms of functionality in amputee rehabilitation and prosthesis design. We have the opinion that studies that will include different levels of amputation will be guiding.
