The impact of child psychopathology on the quality of child-carer relationships in foster children: A retrospective longitudinal study

A history of maltreatment is very prevalent in children in foster care (Oswald, Heil, & Goldbeck, 2009). Maltreatment is often linked to different kinds of psychopathologies which usually linger into care placement (Oswald et al., 2009). Evidence also suggests an association between early deficient care and attachment disorders (Lehmann et al., 2020). However, the link between child psychopathology and child-carer relationships in foster children has received less attention. This study explored the impact of child psychopathology on the quality of child-carer relationships. Methods The current study conducts secondary analysis of The Best Services Trial (BeST?) data. The 31 children (0-60 months) constituting the current study's sample had been placed in care for reasons associated with maltreatment. This study uses Strengths and Difficulties Questionnaire (SDQ) data from the children's Time 1 (T1) visits, and Parent-Infant Relationship Global Assessment Scale (PIR-GAS) and Emotional Signalling Scale (ESS) from their Time 3 (T3) visits, 2.5 years after the T1 visits. The T1 data was readily available for use, but the T3 data was generated from 57 videos of child-carer interactions. The link between psychopathology and the quality of the child-carer relationship was examined from two perspectives; the impact of psychopathology on the quality of the child-carer relationship when measured within the same time periods (T1 and T3), and the impact of psychopathology seen in T1 on the quality of the child-carer relationship seen in T3. Results SDQ's prosocial behaviour item mean, and the quality of the child-carer relationship (PIR-GAS and ESS) increased significantly from T1 to T3. Psychopathology (SDQ) seen in neither T1 nor T3 impacted the quality of the child-carer relationship seen in T3. Similarly, psychopathology seen in T1 did not impact the quality of the child-carer relationship in terms of the child's approach to the carer (ESS) seen in T1. Psychopathology seen in T1, however, negatively impacted the quality of the childcarer relationship in terms of the functioning of the child and carer as a pair (PIR-GAS) seen in T1. Ethnicity contributed significantly to the quality of the child-carer relationship seen in T1. Conclusion Child psychopathology negatively impacts the quality of the child-carer relationship in the context of the pair's functioning at the time of care entry, but no impact is observed 2.5 years later. This change over time was in the pair's dyadic functioning rather than in the child's approach to the relationship. Future early intervention studies should start before the child is placed with a carer to improve the developmental functioning of the pair.
