Özet

Amaç: Maksilla posterior bölgeye dört farklı implant protez kombinasyonu uygulanarak hangisinin en ideal tedavi olduğu konusunda sonlu elemanlar analizi kullanılarak kıyaslamalar yapılmıştır. Serbest sonlanan posterior maksillada kısa implant, açılı implant uygulamasının, kanat uzantısının ve greftlemenin implant, abutmentler, implant üstü protezler üzerinde oluşturduğu von Mises stres değerlerinin, kortikal ve trabeküler kemikler üzerinde oluşturduğu maximum ve minimum principal strain değerlerinin sonlu elemanlar analizi ile incelenmesi amaçlanmıştır. Materyal Metot: Bu çalışmada bilgisayar ortamında atrofik maxilla posterior bölgesini temsil eden dört farklı model tasarlanmıştır. Birinci model hariç diğer modellerde 4.1 mm çapında 10 mm uzunluğunda ikişer adet standart implant kullanılmıştır. Birinci modelde 4 no'lu diş bölgesine standart implant, 6 no'lu diş bölgesine ise artmış sinüs pnömatizasyonundan dolayı kısa implant yerleştirilmiştir. İkinci modelde 4 ve 5 no'lu diş bölgelerine standart implantlar yerleştirilmiştir ancak 5 no'lu diş bölgesinde implant sinüs bölgesini teğet geçecek şekilde distale 30° eğimli olarak yerleştirilmiştir. Üçüncü modelde artmış sinüs pnömatizasyonu bulunan bölgeye greft uygulaması tasarlanmıştır, 4 ve 6 no'lu diş bölgelerine standart implantlar yerleştirilmiştir. Dördüncü modelde ise iki adet standart implant 4 ve 5 no'lu diş bölgelerine yerleştirilmiş, distal kanat yapılmıştır. Tüm modellerde ti-base abutmentler kullanılmış ve 4-5-6 no'lu diş bölgelerini kapsayacak şekilde implant üstü hibrit protezler tasarlanmıştır. Bu protezlerin bukkal tüberküllerinin üzerine, implantların uzun aksına 30º açı olacak şekilde toplamda 200N oblik kuvvet uygulanmıştır. Bulgular: İmplant ve abutmentlerde oluşan en yüksek von Mises stres değerleri ve kortikal kemikte oluşan en yüksek strain değerleri distal kanadın bulunduğu dördüncü modelde, trabeküler kemikte oluşan en yüksek maximum principal strain değeri kısa implantın bulunduğu birinci modelde, implant üstü hibrit protezlerde oluşan en yüksek von Mises stres değerleri ise açılı implantın bulunduğu ikinci modelde gözlemlenmiştir. Sonuç: Tüm modeller arasında genel olarak en güvenilir değerler greft kullanılarak hazırlanan üçüncü modelde, en riskli değerler ise distal kanadın kullanıldığı dördüncü modelde elde edilmiştir. Açılı ve kısa implant bulunan modeller karşılaştırıldığında ise açılı implant sisteminin genel olarak daha az stres ve strain değerlerine sebep olduğu tespit edilmiştir.

Purpose: Comparisons were made using finite element analysis on which one was the most ideal treatment by applying four different implant prosthesis combinations to the maxilla posterior. It is aimed to investigate the von Mises stress values created by short implant, angled implant, cantilever and grafting on implants, abutments and prostheses in edentulous posterior maxilla and the maximum and minimum principal strain values created on cortical and trabecular bones by finite element analysis. Material and Methods: In our study, four different models representing the atrophic maxilla posterior region were designed in computer environment. Except for the first model, two standard implants with a diameter of 4.1 mm and a length of 10 mm were used in all models. In the first model, a standard implant was placed in the tooth no. 4 and a short implant was placed in the tooth no. 6 due to increased sinus pneumatization. In the second model, standard implants were placed in the 4th and 5th tooth regions, but the implant placed in the 5th tooth region was placed distally inclined 30°, tangent to the sinus region. In the third model, graft application was designed to the region with increased sinus pneumatization and standard implants were placed in tooth regions 4 and 6. In the fourth model, two standard implants were placed in tooth regions 4 and 5. Tibase abutments were used in all models and hybrid prostheses on implants were designed to cover tooth regions 4-5-6. A total of 200N oblique force was applied to the buccal tubercules of these prostheses at an angle of 30º to the long axis of the implants. Results: The highest von Mises stress values in the implant and abutments and the highest strain values in the cortical bone in the fourth model with the distal wing, the highest maximum principal strain in the trabecular bone in the first model with the short implant, and the highest von Mises stress values in the hybrid prosthesis on implants was observed in the second model with the angled implant. Conclusion: In general, the most reliable values among all models were obtained in the third model prepared using the graft, and the most risky values were obtained in the fourth model, in which the distal wing was used. When the angled and short implant models are compared, it has been determined that the angled implant system causes less stress and strain values in general.
