Özet
Giriş ve Amaç: Ankara Şehir Hastanesi'nde akut lösemi (AL) tanısıyla hematopoetik kök hücre nakli yapılan hastaların; kök hücre nakli hazırlık rejimi sırasında aldıkları total vücut ışınlanmasının (TVI), tiroid ve pankreas fonksiyonları üzerine etkisinin değerlendirilmesi amaçlandı. Materyal ve Metod: Ankara Şehir Hastanesi Çocuk Kemik İliği Nakil Ünitesi'nde akut lösemi tanısıyla hematopoetik kök hücre nakli yapılan, tedavi bitiminden en az 1 yıl geçmiş, 5 yaşından büyük 50 hasta çalışmaya alınmıştır. Total vücut ışınlamasının tiroid bezi üzerine etkileri tiroid fonksiyon testleri ile, pankreas üzerine etkileri ise HbA1c düzeyleri ve oral glukoz tolerans testi (OGTT) ile değerlendirildi.

Aim: We aimed to evaluate the effect of total body irradiation (TBI) on thyroid and pancreatic functions of patients who underwent hematopoietic stem cell transplantation with the diagnosis of acute leukemia (AL) in Ankara City Hospital during the stem cell transplantation preparation regimen for stem cell transplantation. Materials and Methods: Fifty patients older than 5 years of age, who were followed up with the diagnosis of acute leukemia in the Pediatric Bone Marrow Transplantation Unit of Ankara City Hospital and underwent hematopoietic stem cell transplantation at least 1 year after the end of the treatment were included in the study. The effects of total body irradiation on the thyroid gland were evaluated with thyroid function tests, and the effects on the pancreas gland were evaluated with HbA1c levels and oral glucose tolerance test (OGTT). Results: In our study, conducted with 50 patients who underwent bone marrow transplantation (BMT) 40 patients were diagnosis with acute lymphoblastic leukemia (ALL) and 10 patients with acute myeloblastic leukemia (AML). TBI was used in 32 (64%) of the patients. Thyroid function tests (TFT) and HbA1c values of the patients with and without TBI were similar. The mean insulin values at the 120th minute were 66,50 ± 71,70 in the patients who were treated with TBI; and were 33,15 ± 40,94 in the patients who did not treat TBI and ıt was statistically significant. Insulin peak values, total insulin values and insulin resistance were significantly higher in patients who treated with TBI compared to who did not treated with TBI. Of the patients evaluated with OGTT and HbA1c values measured, 34,4% of the patients were diagnosed with prediabetes and 12,5% with diabetes mellitus. Of the patients who did not treated with TBI, 27,8% were diagnosed with prediabetes and 11,1% with DM. Conclusion: In our study, the exocrine functions of the thyroid and pancreatic gland of patients were compared with and without used TBI. Our study indicates an impaired glucose regulation in patients receiving. Although the number of our patients is limiting factor; our results support that patients should be followed regularly in terms of endocrine side effects.
