Varus gonartrozlu ileri yaş hastalarda arka çapraz bağ koruyan total diz artroplastisinin erken dönem klinik, radyolojik ve fonksiyonel sonuçları

Özet

Amaç: Bu çalışmada ileri yaş varus gonartrozlu dizlerde arka çapraz bağ korunarak uygulanan total diz protezi ameliyatlarının prospektif olarak incelenerek erken dönem klinik, fonksiyonel ve radyolojik sonuçlarının ortaya konulması ayrıca bunların literatürde diğer protez tasarımlarının uygulandığı total diz protezi sonuçları ile karşılaştırılması ve böylece bağ korumanın öneminin ortaya konması amaçlanmıştır. Materyal ve Metod: Harran Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Kliniği'nde Kasım 2020 ve Mayıs 2022 tarihleri arasında primer osteoartritli 36 hastanın 40 dizi çalışmaya dâhil edildi. 65 yaş ve üzeri, primer osteoartritli, varus gonartrozlu hastalar çalışmaya dâhil edilirken, 65 yaşından genç hastalar, romatoit artrit, posttravmatik artrit veya septik artrite sekonder osteoartrit gibi sekonder osteoartritli ve valgus gonartrozlu hastalar çalışmaya dâhil edilmedi. Hastalara arka çapraz bağ koruyan çimentolu total diz protezi uygulandı. Hastalar ameliyat öncesinde diz eklem hareket açıklığı ile beraber tüm dünyada kabul gören Amerikan Diz Cemiyeti skorlama sistemi ve WOMAC skorlama sistemine göre değerlendirildi. Hastaların taburculuk sonrası ikinci haftada yara yeri durumu kontrol edilerek dikişleri aındı. Birinci ve altıncı aylarda grafi kontrolleri yapıldı. En az 12 aylık son kontrollerinde diz eklem hareket açıklığı ile Amerikan Diz Cemiyeti skoru ve WOMAC skoru tekrar değerlendirilerek ameliyat öncsi ile karşılaştırıldı. Hastaların radyolojik değerlendirilmesi ise Total Diz Protezi Radyolojik Değerlendirme Kriterlerine göre yapıldı. Elde edilen sonuçlar literatürde bağ koruyan ve diğer protez tasarımları ile yapılan total diz protezi sonuçları ile karşılaştırıldı. Bulgular: Çalışmaya dahil edilen 36 hastanın 10'u (%27,8) erkek ve 26'sı (%72,2) kadındı. Hastaların yaş ortalaması 69,4 (65-84), takip süresi ise 14,3 ay (12-18) olarak saptandı. Diz skoru ameliyat öncesi 17 (-14-57) iken ameliyat sonrası 80,2 (60-95) olarak hesaplandı. Diz skoru bakımından dizlerin 12'sinde (%30) mükemmel, 25'inde (%60) iyi, 3'ünde (%7,5) orta ve 1'inde (%2,5) kötü sonuç elde edilmiştir. Hastaların fonksiyonel skoru ameliyat öncesi ortalama 38,6 iken ameliyat sonrası 82,5'e (50-100) yükseldiği görüldü. Fonksiyonel skor baz alınarak yapılan XI değerlendirmede, ameliyat sonrası dizlerin 23'ünde (%57,5) mükemmel, 11'inde (%27,5) iyi, 4'ünde (%10) orta ve 2'sinde (%5) kötü sonuç alınmıştır. WOMAC skoru toplam değeri ameliyat öncesi 79,5 (75,7-83,4) iken, ameilyat sonrası 10,4(6,8-14)'e gerilediği görüldü. Hastalarda ameliyat öncesi ortalama 12,4º (4º-25º) varus varken ameliyat sonrası bu değer ortalama 4,7º (0-8,2º) valgus olarak tespit edildi. Hastaların ameliyat öncesi diz eklem hareket açıklığı ortalama 95,6º (55º-128º) iken ameliyat sonrası 115º (85º-141º)'ye yükselmiştir. Hastaların radyolojik incelemesinde hiçbir hastada osteoliz bulgusuna rastlanmadı. Takip süresi içerisinde hiçbir hastada derin protez enfeksiyonu görülmedi ve hiçbir hastaya revizyon uygulanmadı. Sonuç: 65 yaş üstü varus gonartrozlu hastalara uyguladığımız arka çapraz bağ koruyan total diz artroplastisi kısa dönem sonuçları, literatürde diğer protez tasarımlarının uygulandığı total diz protezi başarı oranlarına benzer, hatta bazı gruplarda diz fonksiyon skoru açısından daha başarılı bulunmuştur. İleri yaş hastalarda dejenere ve afonksiyonel olduğu düşünülerek uygulanması önerilmeyen arka çapraz bağ korunan total diz protezi, uygun cerrahi teknik ile uygulandığında varus gonartrozlu ileri yaş hastalarda sonuçları başarılı ve yüz güldürücüdür. Literatür taramasında çapraz bağ korunan ve kesen protez tasarımlarının fonksiyonel sonuçları benzer olmasıyla beraber, çapraz bağın korunması diz kinematiği ve hasta kliniği üzerinde ekstra fayda sağlamaktadır. Objective: This study, it is aimed to prospectively examine the short-term clinical, functional, and radiological results of total knee replacement surgeries performed by preserving the posterior cruciate ligament in knees with advanced age and varus gonarthrosis; moreover, to compare them with the results of a total knee prosthesis in which other prosthesis designs are applied in the literature and thus reveal the importance of ligament preservation. Materials and Method: Between November 2020 and May 2022, 40 knees of 36 patients with primary osteoarthritis were included in the Orthopedics and Traumatology Clinic of Harran University Faculty of Medicine. Patients 65 years of age and older with primary osteoarthritis and varus gonarthrosis were included in the study. In comparison, patients younger than 65, patients with secondary osteoarthritis such as rheumatoid arthritis, posttraumatic arthritis, or osteoarthritis secondary to septic arthritis, and patients with valgus gonarthrosis were not included in the study. Cemented total knee prosthesis protecting the posterior cruciate ligament was applied to the patients. Patients were evaluated according to the American Knee Society scoring system and WOMAC scoring system, which is accepted all over the world, along with knee joint range of motion before surgery. In the second week after discharge, the wound site condition of the patients was checked, and their stitches were removed. Graphy controls were performed in the first and sixth months. At the last controls of at least 12 months, the knee joint range of motion, the American Knee Society score, and the WOMAC score were re-evaluated and compared with the preoperative results. Radiological evaluation of the patients was performed according to Total Knee Prosthesis Radiological Evaluation Criteria. The results obtained were compared with the results of total knee prosthesis performed with ligament preservation and other prosthesis designs in the literature. This study, it is aimed to prospectively examine the short-term clinical, functional, and radiological results of total knee replacement surgeries performed by preserving the posterior cruciate ligament in knees with advanced age and varus gonarthrosis; moreover, to compare them with the results of a total knee prosthesis in which other prosthesis designs are applied in the literature and thus reveal the importance of ligament preservation. XIII Results: Of the 36 patients included in the study, 10 (27.8%) were male, and 26 (72.2%) were female. The mean age of the patients was 69.4 (65-84), and the follow-up period was 14.3 months (12-18). The mean knee score was calculated as 80.2 (60-95) after surgery, while it was 17 (-14-57) before surgery. In terms of knee score, 12 (30%) of the knees were excellent, 25 (60%) were good, 3 (7.5%) were moderate, and 1 (2.5%) was poor. The mean functional score of the patients increased from 38.6 before surgery to 82.5 (50-100) after surgery. In the evaluation based on the functional score, 23 (57.5%) of the postoperative knees were excellent, 11 (27.5%) were good, 4 (10%) were moderate, and 2 (5%) were bad. While the total value of the WOMAC score was 79.5 (75.7-83.4) before surgery, it decreased to 10.4 (6.8-14) after surgery. In the radiological examination, patients had 12.4º (4º-25º) varus preoperatively, and 4.7º (0-8.2º) valgus angle was detected after surgery. The mean range of motion of the knee joint before surgery was 95.6º (55º-128º) and increased to 115º (85º-141º) postoperatively. In addition, in the radiological examination of the patients, no findings of osteolysis were found in any patient. During the follow-up period, no deep prosthesis infection was seen in any patient, and no revision was applied to any patient. Conclusion: The short-term results of total knee arthroplasty preserving the posterior cruciate ligament that we applied to patients with varus gonarthrosis over the age of 65 were similar to the success rates of total knee prosthesis applied to other prosthesis designs in the literature and even more successful in terms of knee function score in some groups. Despite the fact that the posterior cruciate ligament is thought to be degenerate and afunctional in elderly patients, the total knee prosthesis design, which is applied by preserving the posterior cruciate ligament with appropriate surgical technique, is a surgical procedure with successful and satisfactory results like other designs even in elderly patients with varus gonarthrosis.
