Özet
Sağlık hizmeti sunumunda tıbbi hatalardan kaynaklanan ölümler hasta güvenliği kavramını beraberinde getirmiştir. Hasta güvenliğini sağlık hizmetinin hastaya zarar vermeden sağlanması olarak tanımlamak mümkündür. Hasta güvenliğinin sağlanabilmesi için alınabilecek en temel önlemlere yönelik hasta güvenliği hedefleri Joint Commission International (JCI) tarafından geliştirilmiştir. Hasta güvenliği hedeflerinin etkin olarak sağlanması tıbbi hataların meydana gelmesinin önlenmesinde büyük bir öneme sahiptir. Bir sağlık kuruluşunda hasta güvenliği hedeflerinin etkin bir şekilde sağlanması kurumun güvenlik kültürünün sürekli geliştirilmesi ve iyileştirilmesi ile mümkündür. Bu ancak kurum çalışanlarının hasta güvenliği hedeflerine uyumunun sadece teorik bilgi ile sınırlı kalmadan bir davranış haline getirebilmesi ve bir kültür olarak benimseyebilmeleri ile sağlanabilir. Hasta güvenlik kültürünün kurum çalışanları tarafından davranış / kültür haline getirilebilmesi ve bu doğrultuda sürekli iyileştirmeye yönelik uygulamaların hayata geçirilebilmesi hasta güvenlik kültürünün değerlendirilmesi ihtiyacını beraberinde getirmiştir. Bu çalışma kapsamında, özel bir üniversite hastanesinde çalışanların hasta güvenliği konusundaki algılarının objektif bir şekilde değerlendirilebilmesine yönelik çok kriterli karar verme yönteminden yararlanılmıştır. Hasta güvenliği kültürünün değerlendirilmesine yönelik yapılan çalışmaların sadece frekans analizine yönelik olması değerlendirme sürecinde“Her boyut aynı öneme mi sahiptir?”sorusunu beraberinde getirmiştir. Birçok alanda kullanılan ve sağlık hizmetlerinde de daha çok sağlık teknolojisi değerlendirme çalışmalarında kullanılan çok kriterli karar verme yönteminin hasta güvenliği kültürünün değerlendirmesinde de uygulanabileceği fikri ortaya çıkmıştır. Böylece hasta güvenliği kültürünün değerlendirmesinde kullanılan boyutların önem dereceleri belirlenmiş ve kurumun hasta güvenliği kültürü bu önem derecelerine göre daha objektif bir şekilde gözden geçirilebilmiştir. Bu doğrultuda 5 sağlık profesyoneline hasta güvenliği kültürü anketinde yer alan soruların ve boyutların birbirlerine göre önem derecelerinin belirlenebilmesine yönelik uzman görüşlerine başvurulmuştur. Uzman görüşlerinden elde edilen veriler SWARA (Stepwise Weight Assessment Ratio Analysis) yöntemi kullanılarak hasta güvenliği kültürünün değerlendirmesinde kullanılan soruların ve boyutların ağırlıkları belirlenmiştir. Uzman görüşleri doğrultusunda elde edilen ağırlıklar ve hasta güvenliği anketinden elde edilen sonuçlar hasta güvenlik kültürü indeksinin hesaplanmasında kullanılmıştır. Çalışma sonucunda hasta güvenliği kültürünün değerlendirilmesine farklı bir bakış açısı getirilerek iyileştirmeye açık süreçlerin daha objektif bir şekilde belirlenebilmesi ve bu doğrultuda hasta güvenlik kültürünün sürekli geliştirilmesine katkı sağlayacak uygulamaların hayata geçirilebilmesi amaçlanmıştır.

Deaths caused by medical errors in the provision of health care services have brought the concept of patient safety. It is possible to define patient safety as providing health care services without harming the patient. The patient safety targets for the most basic measures that can be taken to ensure patient safety have been developed by the Joint Commission International (JCI). Effective achievement of patient safety goals is of great importance in preventing the occurrence of medical errors. Effective achievement of patient safety goals in a healthcare institution is possible with the continuous development and improvement of the institution's safety culture. This can only be achieved by ensuring that the compliance of the employees of the institution with the patient safety goals can become a behavior and adopt it as a culture, without being limited only to theoretical knowledge. The ability of the patient safety culture to be made into a behavior / culture by the employees of the institution and the implementation of continuous improvement practices in this direction has brought the need to evaluate the patient safety culture. Within the scope of this study, a multi-criteria decision-making method was used to objectively evaluate the perceptions of employees about patient safety in a private university hospital. During the evaluation process, the fact that the studies conducted to evaluate the patient safety culture are aimed only at frequency analysis has brought the question of“Does every dimension have the same importance?”. The idea that the multi-criteria decision-making method, which is used in many fields and mostly used in health technology evaluation studies in health services, can also be applied in the evaluation of patient safety culture, has emerged. Thus, the importance degrees of the dimensions used in the evaluation of patient safety culture were determined and the patient safety culture of the institution could be reviewed in a more objective way according to these importance degrees. In this direction, expert opinions of 5 health professionals were consulted in order to determine the importance of the questions and dimensions in the patient safety culture questionnaire relative to each other. The weights of the questions and dimensions used in the evaluation of patient safety culture were determined by applying the SWARA (Stepwise Weight Assessment Ratio Analysis) method to the data obtained from expert opinions. The weights obtained in accordance with the expert opinions and the results obtained from the patient safety questionnaire were used to calculate the patient safety culture index. As a result of the study, it is aimed to bring a different perspective to the evaluation of patient safety culture by determining the processes that are open to improvement in a more objective way and implementing practices that will contribute to the continuous development of patient safety culture in this direction.
