Özet

Hipoglisemi, plazma glikoz seviyesinin <70 mg/dl olması durumudur. Strese cevapta Hipotalamus-Hipofiz-Adrenal (HPA) aksı çok önemli rol oynar ve verilen doğru kortizol cevabı organizmanın homeostasisi için çok önemlidir. Hipoglisemi HPA aksı için kuvvetli bir stres faktörüdür ve her stres epizodu HPA aksının yeniden programlanmasına neden olur fakat bu yeniden programlamanın mekanizması henüz tam olarak aydınlatılamamıştır. Bu durum özellikle sıklıkla hipoglisemik strese maruz kalan hastaların yönetiminde önemli bir faktördür. Bu tez çalışmasında HPA aksının yeniden programlanmasına neden olabilecek olan tekrarlı hipogliseminin hipotalamus, hipofiz ve adrenalde DNA metilasyon patern değişimi üzerine etkisi olup olmadığının ortaya konması amaçlanmıştır ve oluşturulan akut ve tekrarlı insülin ile indüklenmiş hipoglisemik fare modellerinde akut ve kronik fazda HPA aksı ile ilgili hipotalamus, hipofiz ve adrenal dokularında DNA metilasyonunun regülasyonunda görevli olan Dnmt1, Dnmt2, Dnmt3A, Dnmt3B, Dnmt3L, Tet1, Tet2 ve Tet3 genlerinin transkript seviyeleri Real-Time PCR'da belirlenmiştir. Elde ettiğimiz veriler özellikle tekrarlayan hipoglisemi ve enjeksiyon stresi ile birlikte HPA aksına ait dokularda en fazla Dnmt1 ve Dnmt2 transkript seviyelerinin etkilendiği gösterilmiştir. Ayrıca Tet1, Tet2 ve Tet3 transkript seviyelerinin de HPA aksına ait bütün dokularda r-AH-K grubunda artması da fizyolojik ve psikolojik stresin her ikisinin de DNA metilasyon paternini değiştirebileceğini bu yolla da HPA aksının yeniden programlanmasına neden olabileceğini göstermektedir.

Hypoglycemia is the condition where the plasma glucose level is <70 mg/dl. The Hypothalamus-Pituitary-Adrenal (HPA) axis plays a very important role in the response to stress and the correct cortisol response is very important for the homeostasis of the organism. Hypoglycemia is a strong stress factor for the HPA axis and each stress episode causes reprogramming of the HPA axis, but the mechanism of this reprogramming has not yet been completely elucidated. This is an important factor in the planning of treatment, especially for patients who are frequently exposed to hypoglycemic stress. In this thesis, it is aimed to reveal whether recurrent hypoglycemia, which may cause reprogramming of the HPA axis, has an effect on DNA methylation pattern change in the hypothalamus, pituitary and adrenal and in the acute and recurrent insulin-induced hypoglycemic mouse models created, in the acute and chronic phases of the hypothalamus, pituitary and adrenal tissues related to the HPA axis, transcript levels of Dnmt1, Dnmt2, Dnmt3A, Dnmt3B, Dnmt3L, Tet1, Tet2 and Tet3 genes which are responsible for the regulation of DNA methylation were determined with Real-Time PCR. With the data we obtained, it was shown that Dnmt1 and Dnmt2 transcript levels were most affected in the tissues of the HPA axis, especially with recurrent hypoglycemia and injection stress. In addition, the increase in Tet1, Tet2 and Tet3 transcript levels in the r-AH-K group in all tissues of the HPA axis indicate that both physiological and psychological stress can change the DNA methylation pattern and thus cause reprogramming of the HPA axis.
