Özet

İnme, dünya genelinde ikinci en yüksek ölüm nedeni ve önde gelen bir sakatlık sebebidir. Yapılan araştırmalarda iskemik inmeden sonraki ilk 3 ay içerisinde hastaların %19'unun en az bir kardiyak rahatsızlığa yakalandığı belirtilmektedir. Bu çalışmada beyin ve kalp arasındaki etkileşimlerde koruyucu mitokondri spesifik antioksidan (MitoTEMPO) uygulamasının önemi değerlendirilmiştir. Ayrıca inme geçirme insidansı diğer bireylere göre daha yüksek görülen kişilerde koruyucu tedbirlerin önemini vurgulayıcı özgün verilere ulaşılması hedeflenmiştir. Projemiz amacına ulaşabilmek için Wistar Albino türü erkek sıçanlar koruyucu MitoTEMPO enjeksiyonu (0,7 mg / kg / gün) sonrası monofilament ilerletme tekniği ile orta serebral arter (OSA)'de 1 saat iskemi gerçekleştirilmiştir. Ölçüm ve doku toplama işlemleri ise reperfüzyonun üçüncü günü yapılmıştır. Deney hayvanları rastgele seçimle SHAM, iskemi+reperfüzyon (IR) ve MitoTEMPO+IR (MT+IR) şeklinde üç gruba ayrılmıştır. Deneyler sonucunda torasik elektriksel biyoempedans (TEB) ölçümlerinde IR ile istatistiksel anlamlı olarak azalan VEPT ve LVET değerlerine karşı MitoTEMPO koruyucu etki göstermiştir. Ayrıca elektrokardiyografi (EKG) sonuçlarımız P-R interval, QTc, T dalga süresi ve R-R interval değerlerinin de IR ile anlamlı şekilde değişmiş ve MitoTEMPO bu değişimlere karşı koruyucu etki göstermiştir. Biyokimyasal sonuçlarımız ise kan serumu örneklerinde TAS, TOS ve OSİ, kalp sol ventrikül dokusunda cAMP ve IP3, beyin sağ serebral hemisfer dokusunda ise cAMP, IP3 ve katalaz değerlerinin IR ile istatistiksel olarak anlamlı şekilde etkilendiği ve MitoTEMPO'nun koruyucu etki gösterdiğini işaret etmektedir. Kalp ve beyne ait dokuların IR sonrası mikroskobik düzeydeki histopatolojik değişimlerine de MitoTEMPO büyük ölçüde koruyucu etki göstermiştir. Sonuç olarak inme geçirme insidansının diğer bireylere göre daha yüksek olduğu bilinen bazı hastalık gruplarında MitoTEMPO kullanımının IR kaynaklı kardiyopatolojik bulguları azaltabileceği tespit edilmiştir.

Stroke is the second highest cause of death and a leading cause of disability worldwide. Studies have shown that 19% of patients suffer from at least one cardiac disease in the first 3 months after ischemic stroke. In this study, the importance of protective mitochondria-specific antioxidant (MitoTEMPO) application in the interactions between brain and heart was evaluated. In addition, it was aimed to reach original data emphasizing the importance of protective measures in people whose stroke incidence is higher than other individuals. In order to achieve the aim of our project, male Wistar Albino rats were subjected to 1 hour ischemia in the right middle cerebral artery with the monofilament advancement technique after protective MitoTEMPO injection (0.7 mg / kg / day). Measurement and tissue collection procedures were performed in the third day of reperfusion. Experimental animals were randomly divided into three groups as SHAM, ischemia+reperfusion (IR) and MitoTEMPO+IR (MT+IR). As a result of the experiments, MitoTEMPO showed a protective effect against VEPT and LVET values that decreased statistically with IR in thoracic electrical bioimpedance (TEB) measurements. In addition, our electrocardiography (ECG) results showed that P-R interval, QTc, T wave duration and R-R interval values changed statistically significant with IR and MitoTEMPO showed a protective effect against these changes. Our biochemical results indicate that TAS, TOS and OSI in blood serum samples, cAMP and IP3 in heart left ventricular tissue, cAMP, IP3 and catalase values in brain right cerebral hemisphere tissue were significantly affected by IR and MitoTEMPO showed a protective effect. MitoTEMPO also showed a great protective effect on histopathological changes in the tissues of the heart and brain at the microscopic level after IR. As a result, it has been determined that the use of MitoTEMPO can reduce post-stroke cardiopathological findings in some disease groups known to have a higher incidence of stroke than other individuals.
