Özet
Sağlık bakımı ile ilişkili enfeksiyonlar (SBİE) hem dünyada hem ülkemizde morbidite ve mortalitenin önemli bir sebebidir. SBİE'ler antibiyotik kullanımını ve mikroorganizmaların çoklu antibiyotik direnci gelişimi riskini arttırmakta, hastaların hastanede yatış süresini uzatmakta, bununla birlikte bakım maliyetlerini de arttırmaktadır. Günümüzde, SBİE'nin önlenmesi ve kontrolünün temelini el hijyeni, temizlik, dezenfeksiyon, sterilizasyon, epidemiyolojik yöntemler, izolasyon önlemleri ve sürveyans çalışmaları oluşturmaktadır. COVID-19 salgını sırasında sağlık çalışanları ve hastalar arasında kişisel koruyucu ekipman kullanımı artmış, el yıkama ve ekipman dezenfeksiyonu gibi kişisel hijyen önlemlerinin arttırılmasının önemi vurgulanmıştır. Bu tez çalışmasında COVID-19 pandemisinin hastanemizde uygulanan tedbirler çerçevesinde SBİE'lerin değişimine olan etkisini değerlendirmeyi amaçladık. Ocak 2018- Mart 2020 tarihleri arasında COVID-19 pandemisi öncesi 2 yıllık dönem ve Mart 2020-Ocak 2022 tarihleri arasında COVID-19 pandemisi sırasındaki 2 yıllık dönemde Selçuk Üniversitesi Tıp Fakültesi Hastanesi YBÜ'lerinde yatan, sürveyans çalışmaları ve Enfeksiyon Kontrol Komitesi (EKK) hemşireleri tarafından yapılan takiplerde CDC tanı kriterlerine göre SBİE olarak kaydedilen hastaların dosyaları ve EKK kayıtları retrospektif olarak incelendi. Çalışmaya 18 yaş üzeri olan ve YBÜ'lerdeki takiplerinde CDC tanı kriterlerine göre SBİE gelişen tüm hastalar ile COVID YBÜ'lerinde yatarken SBİE gelişen hastalar dahil edildi. SBİE gelişen 827 hastada görülen 1089 SBİE atağı retrospektif olarak değerlendirildi. Çalışmaya alınan hastaların; yaşı, cinsiyeti, altta yatan kronik hastalıkları, hastaneye yatış tarihleri, takip edildiği YBÜ, geçirdiği enfeksiyon sayısı, belirlenen SBİE türü, belirlenen SBİE'nin gelişme günü, SBİE gelişimi için risk faktörleri, kültürde üreyen mikroorganizma ve üreyen etkenin antibiyotik direnç profili ve mortalite gibi özellikleri değerlendirildi. COVID-19 öncesi ve COVID-19 sonrası dönemde hastanemiz YBÜ'lerine yatan hastalarda gelişen SBİE oranı değerlendirildiğinde; COVID-19 sonrası dönemde YBÜ'lerde gelişen SBİE'lerde azalma gözlendi. Bu azalma istatistiksel olarak anlamlı saptandı (p=0,001). COVID-19 öncesi ve COVID-19 sonrası dönemde hastanemize yatışlar değerlendirildiğinde COVID-19 sonrası dönemde görülen hasta sayılarındaki azalma istatistiksel olarak anlamlı saptandı (p=0,001). Hastanemiz YBÜ'lerinde COVID-19 öncesi ve sonrası 4 yıllık dönemde tanımlanan SBİE'lerin genel dağılımı incelendiğinde; %55 pnömoni, %16,6 SVKKDE, %11,80 Kİ-İYE (Semptomatik İYE 1a), %6,20 LTD-KDE-1, %4,3 VİO ve %1,7 CYDE mevcuttu. SBİE gelişen hastaların 52 (%4,8)'sinde SEK-KDE saptandı. COVID-19 öncesi ve COVID-19 sonrası dönemde gelişen SBİE'ler incelendiğinde ise COVID-19 sonrası dönemde pnömoni gelişimindeki artış anlamlı tespit edildi (p=0,001). Ayrıca SEK-KDE gelişiminde de COVID-19 sonrası dönemde anlamlı artış mevcuttu (p=0,001). Kültürlerde üreyen mikroorganizmalar değerlendirildiğinde COVID-19 öncesi ve sonrası 4 yıllık dönemde genel olarak; Gram negatif üreme %73,1 oranında olup Acinetobacter spp. üremesi %46,4 olarak en yüksek oranda saptandı. Kültürde üreyen mikroorganizmalar COVID-19 öncesi ile COVID-19 sonrası dönem olarak değerlendirildiğinde; COVID-19 sonrası dönemde Gram pozitif üremede azalma ve Gram negatif üremede artış saptandı ve bu değişim anlamlı bulundu (p=0,003). COVID-19 sonrası dönemde COVID-19 öncesine göre Klebsiella spp. ve S.maltophilia üremelerinde anlamlı artış tespit edildi (p=0,001, p=0,005). E.coli, Candida spp. ve E.faecium üremelerindeki azalma ise anlamlı olarak değerlendirildi (p=0,038, p=0,022, p=0,008). COVID-19 YBÜ'lerde yatan hastalarda gelişen SBİE dağılımları diğer YBÜ'lerle karşılaştırıldığında COVID-19 YBÜ'de pnömoni ve SEK-KDE anlamlı yüksek saptandı (p=0,001). SVK-KDE gelişimi ise COVID-19 dışı hasta takibi yapılan diğer YBÜ'lerde anlamlı yüksek saptandı (p=0,018) SBİE'ye bağlı genel mortalite oranı ise %48,6 olarak bulundu. COVID-19 öncesi ve sonrası dönemde SBİE'ye bağlı mortalite değerlendirildiğinde COVID-19 sonrası mortalitede anlamlı artış saptandı (p=0,003) Tüm YBÜ'lerde gelişen SBİE'lerin tür dağılımına göre mortalite oranları değerlendirildiğinde; SVK-KDE ve SEK-KDE gelişen hastalarda mortalite anlamlı yüksek tespit edildi (p=0,001, p=0,013). Sonuç olarak; SBİE'ler yüksek mortalite ve morbidite oranlarına sahip ciddi enfeksiyonlardır. SBİE türleri ve dağılımı ülkeler, bölgeler, hastaneler ve yıllara göre değişiklikler göstermekle birlikte hep birlikte tanık olduğumuz pandemi gibi tüm dünyayı etkileyen ciddi durumlarda hem sayı hem de türler açısından değişiklikler gösterebilir. SBİE'leri daha iyi yönetebilmek, mortalite ve morbidite oranlarını azaltabilmek için sürveyans sisteminin iyi işlemesi, SBİE'lerde gelişen değişimin ve dağılımın farklarının iyi gözlenmesi ve klinikler bazında önlemlerin alınması en doğru yaklaşım olacaktır.

Healthcare-associated infections (HAIs) are an important cause of morbidity and mortality both in the world and in our country. HAIs increase the risk of antibiotic use and the development of multi-antibiotic resistance of microorganisms, prolong the hospitalization period of the patients, and increase the cost of care. Today, hand hygiene, cleaning, disinfection, sterilization, epidemiological methods, isolation precautions, and surveillance studies constitute the basis for the prevention and control of HAI. During the COVID-19 pandemic, the use of personal protective equipment has increased among healthcare workers and patients, and the importance of increasing personal hygiene measures such as hand washing and equipment disinfection has been emphasized. In this thesis, we aimed to evaluate the impact of the COVID-19 pandemic on the change in HAIs within the framework of the measures implemented in our hospital. Surveillance studies and Infection Control Committee inpatients in Selcuk University Medical Faculty Hospital ICUs during the 2 years before the COVID-19 pandemic between January 2018 and March 2020 and during the 2 years between March 2020-January 2022 during the COVID-19 pandemic The files and ECC records of the patients who were registered as HAIs according to the CDC diagnostic criteria during the follow-ups by their nurses were retrospectively reviewed. All patients over the age of 18 who developed HAI according to CDC diagnostic criteria during their follow-up in ICUs and patients who developed HAI while staying in COVID ICUs were included in the study. 1089 episodes of HAI in 827 patients who developed HAI were evaluated retrospectively. The patients included in the study; such as age, gender, underlying chronic diseases, hospitalization dates, ICU followed, the number of infections, the type of infection determined, the date of development of the determined HAI, the risk factors for the development of HAI, the microorganism grown in the culture and the antibiotic resistance profile of the growing agent, and mortality features was evaluated. When the rate of HAI developed in patients hospitalized in the ICUs of our hospital in the pre-COVID-19 and post-COVID-19 period is evaluated; In the post- COVID-19 period, a decrease was observed in HAIs developing in ICUs. This decrease was found to be statistically significant (p=0.001). When hospitalizations were evaluated during the pre-COVID-19 and post- COVID-19 period, the decrease in the number of patients in the post-COVID-19 period was found to be statistically significant (p=0.001). When the general distribution of HAIs defined in the ICUs of our hospital during the 4 years before and after COVID-19 is examined; 55% had pneumonia, 16.6% CVC-LSI, 11.80% CI-UTI (Symptomatic UTI 1a), 6.20% LTD-BSI-1, 4.3% VIO, and 1.7% SSI. SEK-BSI was detected in 52 (4.8%) of the patients who developed HAI. When SBIs developing in the pre-COVID-19 and post-COVID-19 periods were examined, the increase in the development of pneumonia in the post- COVID-19 period was found to be significant (p=0.001). In addition, there was a significant increase in the development of SEK-BSI in the post-COVID-19 period (p=0.001). When the microorganisms grown in cultures are evaluated, in general in the 4 years before and after COVID-19; the Gram-negative growth rate is 73.1%, and Acinetobacter spp. reproduction was found at the highest rate at 46.4%. When the microorganisms grown in the culture are evaluated in the pre-COVID-19 and post- COVID-19 period; A decrease in Gram-positive growth and an increase in Gramnegative growth were detected in the post-COVID-19 period, and this change was found to be significant (p=0.003). In the post-COVID-19 period compared to the pre- COVID-19 period, Klebsiella spp. and S.maltophilia growths were significantly increased (p=0.001, p=0.005). E.coli, Candida spp., and a decrease in E. faecium growths were evaluated as significant (p=0.038, p=0.022, p=0.008). When the distribution of HAIs developed in patients hospitalized in COVID- 19 ICUs was compared with other ICUs, pneumonia and SEK-BDI were found to be significantly higher in COVID-19 ICUs (p=0.001). The development of CVC-BSI was found to be significantly higher in other ICUs that followed non-COVID-19 patients (p=0.018) The overall mortality rate due to HAI was found to be 48.6%. When mortality due to HAI was evaluated in the pre-and post-COVID-19 period, a significant increase was found in post-COVID-19 mortality (p=0.003) When the mortality rates of HAIs developing in all ICUs are evaluated according to the species distribution; Mortality was found to be significantly higher in patients who developed CVC-LSI and SEC-BSI (p=0.001, p=0.013). As a result; HAIs are serious infections with high mortality and morbidity rates. Although the types and distribution of HAIs vary according to countries, regions, hospitals, and years, they may vary in terms of both numbers and types in serious situations affecting the whole world, such as the pandemic we have witnessed together. To manage HAIs better and reduce mortality and morbidity rates, the best approach would be to ensure that the surveillance system works well, the differences in the change and distribution of HAIs are well observed and precautions are taken based on clinics.
