Özet

Covid-19 salgını, birçok hastanenin çalışma stratejilerini ve hastanenin mevcut güvenlik ortamını gözden geçirmesine neden olmuştur. Süreçte çalışan güvenliği kadar hasta güvenliği (HG) de önem kazanmıştır. Hasta güvenliğinin oluşturulmasında hasta güvenliği iklimi (HGİ) değerlendirilmelidir. HGİ, sağlık hizmetinde kalitenin kilit unsuru olarak, dünyadaki tüm ülkelerin sağlık sistemlerinde bir öncelik olmalıdır. HG programlarının amacı, sağlık hizmetlerinin sunumunda hataları önlemek ve hastalara olası zarar azaltmaktır. Sağlık çalışanları için de güvenli bir çalışma ortamı oluşturmak için bir güvenlik iklimi gereklidir. Sağlık çalışanlarının farklı ortamlarda verdikleri sağlık hizmetlerinin sunumunda HG'ne hasta güvenliğine yönelik tutumları, servisin organizasyonu ve işleyişi, kuruluşun güvenlik kültürünün oluşturulmasında önemlidir. Bu çalışmanın amacı, Covid-19 pandemisi sürecinde sağlık profesyonellerinin HGİ ile ilgili görüşlerini belirlemektir. Çalışma Ocak 2021 ve Mart 2022 tarihleri arasında Muğla Eğitim ve Araştırma Hastanesi'nde çalışan 311 sağlık profesyoneli ile gerçekleştirilmiştir. Çalışmada, katılımcıların meslek grupları ile yöneticilerin güvenlik liderliği anlaşması, hasta güvenlik komitesi liderliği, kurallar/ekip uyumu, hasta güvenliği iklimi kategorileri ve kurumsal faktörler alt boyutları arasında istatistiksel olarak anlamlı fark olduğu saptanmıştır (p<0.05). Katılımcıların çalıştıkları birimler ile hasta güvenlik komitesi liderliği, kurallar/ekip uyumu ve kurumsal faktörler alt boyutları arasında istatistiksel olarak anlamlı fark belirlenmiştir (p<0.05). Çalışmada katılımcıların hasta bakım hizmeti verirken kişisel koruyucu ekipmandan etkilenenleri, HGİÖ ve ana ölçek toplam puan ortalamalarının etkilenmeyenlerden daha yüksek, aralarındaki farkın da istatistiksel olarak anlamlı olduğu belirlenmiştir (p<0.05). Katılımcıların“Çalışanların Tutumları”ile“Kurumsal Faktörler”ölçeği toplam puan ortalamaları arasında anlamlı bir korelasyon olduğu saptanmıştır. Sonuç olarak, katılımcıların pandemi sürecinde HGİ ölçeği, çalışanların tutumları ve kurumsal faktörleri ölçekleri toplam puan ortalamalarının yüksek olduğu, önlemlerin etkili olduğu söylenebilir.

The Covid-19 pandemic has caused many hospitals to review their operating strategies and the hospital's current safety environment. In this process, patient safety (HG) has gained importance as much as employee safety. Patient safety climate (HGI) should be evaluated in establishing patient safety. HGI, as a key element of quality in health care, should be a priority in the health systems of all countries in the world. The aim of HG programs is to prevent errors in the delivery of health services and to reduce possible harm to patients. A climate of safety is necessary to create a safe working environment for healthcare workers as well. Attitudes of healthcare professionals towards patient safety in the presentation of healthcare services in different environments, organization and functioning of the service are important factors in establishing the safety culture of the institution. The aim of this study is to determine the opinions of health professionals about HGI during the Covid-19 pandemic process. The study was carried out with 311 healthcare professionals working at Muğla Training and Research Hospital between January 2021 and March 2022. In the study, a statistically significant difference was found between the occupational groups of the participants and the managers' safety leadership agreement, patient safety committee leadership, rules/team compliance, patient safety climate categories and institutional factors sub-dimensions (p<0.05). A statistically significant difference was determined between the units where the participants worked and the patient safety committee leadership, rules/team compliance and institutional factors sub-dimensions (p<0.05). It was determined that the number of monthly shifts held by health professionals during the pandemic process increased, the mean scores of the Patient Safety Climate Scale and the main scale were higher than the other groups, and the difference between them was statistically significant (p<0.05). In the study, it was determined that the participants were affected by personal protective equipment while providing patient care, the mean scores of the Patient Safety Climate Scale and the main scale were higher than those who were not, and the difference between them was statistically significant (p<0.05). It was determined that there was a significant correlation between the participants'“Attitudes of Employees”and the total mean scores of the“Institutional Factors”scale. In addition, it was determined that the participants exhibited similar behavior in terms of HGI in the hospital, and the group of doctors and those working in the service were close to the general average. As a result, it can be said that the total score averages of the participants in the patient safety climate scale, the attitudes of the employees and the institutional factors scales were high and the measures taken were effective during the pandemic process.
