Özet

Amaç: Erken evre meme kanserinde COVID-19 pandemisi öncesi uygulanan Konvansiyonel Fraksiyone Tüm Meme Radyoterapi rejimi (KF-TMRT) ile pandemi sürecinde hastalarımızı ve radyoterapi profesyonellerini potansiyel COVID-19 maruziyetinden korumak ve iş yükünü azaltmak amaçlı olarak kullandığımız Ilımlı Hipofraksiyone Tüm Meme Radyoterapi (IH-TMRT) rejimlerinin erken-geç radyasyon toksisitesi ve kozmetik açıdan karşılaştırmalı olarak değerlendirilmesi ve IH-TMRT rejiminin etkinlik ve güvenliğini test etmektir. Gereç ve Yöntem: Çalışmamıza Gülhane Tıp Fakültesi Radyasyon Onkolojisi Departmanında Ocak 2019 –Eylül 2022 tarihleri arasında T1-2 N0 M0 Erken Evre meme kanseri nedeniyle iki ayrı post-operatif Radyoterapi rejimi uygulanan 96 hasta dahil edilmiştir. COVID-19 öncesinde kullanılan Konvansiyonel Fraksiyone Tüm Meme Radyoterapi rejimi uygulanan 43 hastaya 50Gy/2Gy/fraksiyon (fr)'dan toplamda 25 fraksiyon TMRT + 10Gy/2Gy/fr'dan toplamda 5 fr tümör yatağına Boost, günde 1 fraksiyon, haftada 5 fraksiyon, toplam süre 6 hafta - 30 fraksiyon; COVID-19 sonrası uygulamaya konulan Ilımlı Hipofraksiyone Tüm Meme Radyoterapi rejimi 53 hastaya; 40Gy/2,66Gy/fr'dan toplamda 15 fraksiyon TMRT + 2Gy/fr'dan toplamda 5 fr Boost, günde 1 fr, haftada 5 fr, toplam süre 4 hafta – 20 fraksiyon olarak uygulanmıştır. Erken-geç radyasyon toksisiteleri Common Terminology Criteria For Adverse Events (CTCAE) versiyon 5.0 kullanılarak, kozmezis hasta ve doktor bildirimine göre Harvard/NSABP/RTOG skalası kullanılarak ölçülmüştür. Bulgular: COVID-19 sürecinde tedavi edilen 53 Meme Koruyucu Cerrahi (MKC) uygulanmış hasta çalışma ve öncesinde tedavi edilen 43 MKC'li hasta kontrol grubu olarak toplam 96 hasta çalışmamıza dahil edilmiştir. Her iki gruptaki hastalar demografik ve tümör karakteristikleri açısından benzerdir. COVID-19 öncesinde uygulanan Konvansiyonel Radyoterapi ile Ilımlı Hipofraksiyone rejimlerin kronik-geç yan etkileri açısından istatistiksel anlamlı fark bulunmazken (p>0,05), akut-erken yan etkiler açısından Hipofraksiyone kolda grad 2 ve üzeri yan etkiler, dermatit, kaşıntı ve yorgunluk istatistiksel olarak anlamlı farklılıkla daha az gözlendi (p<0,05). Her iki Konvansiyonel ve Hipofraksiyone radyoterapi gruplarının tamamında doktor bildirimli mükemmel/iyi kozmezis %93,8 oranında sağlanmış ve her iki kol arasında kozmezis açısından istatistiksel olarak anlamlı fark bulunmamıştır. Kozmetik sonuçlar için doktor ve hasta bildirim uyumu Kappa testi ile anlamlı derecede uyumlu tespit edilmiştir (Kappa:0,582; p:0,000). Sonuç: Meme Koruyucu Cerrahi sonrası post-operatif radyoterapi programlanan meme kanseri tedavisinde IH-TMRT ile tedavi, KF-TMRT'ye göre toplam tedavi süresini kısaltan, daha az akut-erken radyotoksisiteye neden olan, merkezin iş yükünü azaltan ve hasta tahammülünü artıran etkin ve güvenli bir radyoterapi tedavi opsiyonudur.

Objective: Conventional Fractionated Whole Breast Radiotherapy regimen (CF-WBRT) applied before the COVID-19 pandemic in early stage breast cancer and Moderately Hypofractionated Whole Breast Radiotherapy regimen (MH-WBRT), which we used to protect our patients and radiotherapy professionals from potential COVID-19 exposure to reduce the workload during the pandemic process, are compared for early and late radiation toxicity and cosmetic effects of both WBRT regimens to test the efficacy and safety of the Moderately Hypofractionated Whole Breast Radiotherapy regimen. Materials and Methods: CF-WBRT regimen of 5-week 50 Gy in 25 fractions of 2,0 Gy versus MH-WBRT regimen of 3-week 40 Gy in 15 fractions of 2,66 Gy WBRT with additional 1-week 10 Gy tumor-bed Boost in 5 fractions of 2 Gy in both groups is compared. Moderately Hypofractionated and Conventional Fractionated WBRT applied post-operatively to T1-2 N0 M0 Early Stage Breast Cancer patients in the Radiation Oncology Department of Gülhane Faculty of Medicine between January, 2019 and September, 2022 is compared for acute/chronic radiation toxicity and cosmesis evaluation using Common Terminology Criteria For Adverse Events (CTCAE) version 5.0 for toxicity and Harvard/NSABP/RTOG scales for cosmesis evaluation according to Doctor and Patient-Doctor reporting respectively. Results: A total of 96 patients were included in our study as 53 patients who underwent Breast Conserving Surgery (BCS) treated with MH-WBRT during the COVID-19 process and 43 control group patients with BCS plus CF-WBRT treated before the start of COVID-19 Pandemic. Patients in both groups were similar in terms of demographics and tumor characteristics. There was no statistically significant difference between CF-WBRT applied before COVID-19 and MH-WBRT regimens in terms of chronic-late side effects (p>0.05). Whilst in terms of acute-early side effects; any grade 2 or higher grade toxicity, dermatitis, pruritus and fatigue were observed less frequently with statistically significant differences in the hypofractionated arm (p<0.05). Physician-reported excellent/good cosmesis was achieved by 93,8% in the total 96 patients of both groups given either Conventional or Hypofractionated WBRT with no statistically significant difference (p>0.05). Physician and patient reporting scoring compatibility for cosmesis in both groups was found to be significant with Kappa test for cosmetic results (Kappa: 0.582; p=0.000). Conclusion: Treatment with MH-WBRT is an effective and safe radiotherapy treatment option that shortens the total treatment time, causes less acute-early radiotoxicity, reduces the workload of the center and increases patient compliance compared to CF-WBRT in the radiotherapy of breast cancer scheduled to be given after Breast Conserving Surgery
