Özet
Amaç: Bu çalışmada obezite ile vitamin D, SOCS3 (Sitokin Sinyal Baskılayıcı 3 Molekülü), ALDH2 (Aldehit Dehidrojenaz 2) ve IL-6 (İnterlökin-6) gen polimorfizmleri arasındaki ilişkinin araştırılması amaçlandırılmıştır. Gereç ve Yöntem: Araştırmaya 100 sağlıklı 99 obez hasta grubu olmak üzere toplamda 199 kişi katıldı. Çalışma Ağustos 2020 - Aralık 2020 tarihleri arasında Karabük Üniversitesi Eğitim ve Araştırma Hastanesi İç Hastalıkları kliniğinde yapılmıştır. Çalışmaya 99 obez ve 100 sağlıklı olmak üzere toplamda 199 kişi dahil edildi. Katılımcılardan alınan tam kan örneklerinden izole edilen genomik DNA kullanılarak belirtilen genlere ait 7 polimorfik bölge PCR yöntemi kullanılarak analiz edilmiştir. Obez hasta grubu ile sağlıklı kotrol grubu polimorfizmleri birbirleri ile karşılaştırılmıştır. İstatistiksel anlamlılık düzeyi p<0,05 olarak ele alınmıştır. Obez ve sağlıklı gönüllülerden izole edilen genomik DNA kullanılarak belirtilen genlere ait 7 polimorfik bölge PCR yöntemi kullanılarak analiz edilmiştir. Genotipleme“Melting Curve Genotyping”analizi ile yapıldı. İstatistiksel anlamlılık düzeyi p<0,05 olarak ele alınmıştır. Bulgular: Çalışmamızda kontrol grubunda olanların % 49' u erkek, % 51' i kadın; obezite grubunda olanların % 27,3'ü erkek, % 72,7'si kadındır. Kontrol grubunda olanların yaş ortalaması 36,59 ± 10,36 yıl, boy ortalaması 168,7 ± 7,44 cm, kilo ortalaması 65,51± 8,15 kgdır. Obezite grubunda olanların yaş ortalaması 44,81 ± 12,01 yıl,boy ortalaması 161,94 cm ± 8,56, kilo ortalaması 99,89 ± 11,47 kg dır. Araştırmada elde edilen sonuçlara bakıldığında obezite grubunda kadınların oranının erkeklere göre daha yüksek olduğu gözlemlenmiştir. Ayrıca obezite grubunda hiperlipidemi, diyabetes mellitus, hipertansiyon ve koroner arter hastalığı tanılarına daha sık rastlanmaktadır. Obezite grubunda olanların yaş ortalamasına bakıldığında ise kontrol grubuna göre anlamlı şekilde daha yüksek olduğu görülmüştür. Benzer şekilde, obezite grubunda olanların kilo ortalamaları da kontrol grubuna göre anlamlı şekilde daha yüksektir. Yine araştırmamızda obezite grubunda olanların boy ortalaması, kontrol grubuna göre daha düşüktür. Laboratuvar değerleri incelendiğinde, obezite grubunda glukoz, açlık insülin ve HbA1c, ALT, Total Kolesterol, trigliserid, LDL-Kolesterol, lökosit, trombosit, RDW, değerlerinin kontrol grubuna göre anlamlı şekilde yüksek olduğu tespit edilmiştir. Genetik analiz sonuçlarına göre, obezite ve kontrol grupları ile rs1800795, rs671, rs4969169, rs1061489 ve rs1544410 polimorfizmleri arasında anlamlı bir ilişki bulunmamaktadır. Ancak rs8064821 polimorfizmi obezite grubunda anlamlı bir şekilde daha yüksek oranda bulunmuştur. Kontrol grubundakilerin %35'inin rs1800795 polimorfizmi heterozigot ve %61'inin homozigot normal iken obezite grubundakilerin %33'ünün rs1800795 polimorfizmi heterozigot ve % 59'unun homozigot normaldir. Kontrol grubundakilerin % 100'ünün ve obezite grubundakilerin %100'ünün rs671 polimorfizmi homozigot mutanttır. Kontrol grubundakilerin % 12'sinin rs4969169 polimorfizmi heterozigot ve %88'inin homozigot normal iken obezite grubundakilerin %19'unun rs4969169 polimorfizmi heterozigot ve % 81'ininn homozigot normaldir. Kontrol grubundakilerin %100'ünün ve obezite grubundakilerin %100'ünün rs1061489 polimorfizmi homozigot mutanttır. Kontrol grubundakilerin %38'inin rs1544410 polimorfizmi homozigot mutant, % 43'ünün heterozigot ve % 19'unun homozigot normal iken obezite grubundakilerin % 45'inin rs1544410 polimorfizmi homozigot mutant, % 37'sinin heterozigot ve % 18'inin homozigot normaldir. Kontrol grubundakilerin % 50'sinin rs8064821 polimorfizmi homozigot mutant, % 42'sinin heterozigot ve %8'inin homozigot normal iken obezite grubundakilerin % 64'ünün rs8064821 polimorfizmi homozigot mutant, % 34'ünün heterozigot ve % 2'sinin homozigot normaldir. Bununla birlikte obezite grubunda olanların daha yüksek bir seyirle homozigot mutant olduğu, heterozigot ve homozigot normalin ise kontrol grubunda olma oranının daha yüksek olduğu görülmüştür. Sonuç: Çalışmamızda genetik analiz sonuçlarına göre, obezite ve kontrol grupları arasında rs1800795, rs671, rs4969169, rs1061489 ve rs1544410 polimorfizmleri arasında anlamlı bir ilişki bulunmamaktadır. Ancak elde edilen bulgulara göre obezite grubu ile rs8064821 gen polimorfizmi arasında anlamlı bir ilişki saptanmıştır. Sonuç olarak, bu çalışma obezite ile kontrol grubu arasında cinsiyet dağılımı, hastalık tanıları, antropometrik ölçümler ve laboratuvar değerleri gibi farklılıkları ortaya koymuştur. Bu bulgular, obezitenin çeşitli sağlık riskleriyle ilişkili olduğunu ve genetik faktörlerin obezite riskinde rol oynayabileceğini düşündürmektedir.

Aim: In this study, it was aimed to investigate the relationship between obesity and vitamin D, SOCS3 (Cytokine Signal Repressor 3 Molecule), ALDH2 (Aldehyde Dehydrogenase 2) and IL-6 (Interleukin-6) gene polymorphisms. Materials and Methods: A total of 199 people, including 100 healthy and 99 obese patients, participated in the study. The study was carried out between August 2020 - December 2020 in Karabuk University Training and Research Hospital Internal Diseases Clinic. A total of 199 people, 99 of whom were obese and 100 were healthy, were included in the study. Using the genomic DNA isolated from the participants, 7 polymorphic regions of the indicated genes were analyzed using the PCR method. The polymorphisms of the obese patient group and the healthy control group were compared with each other. Statistical significance level was taken as p<0.05. Using genomic DNA isolated from obese and healthy volunteers, 7 polymorphic regions of the indicated genes were analyzed using PCR method. Genotyping was done by“Melting Curve Genotyping”analysis. Statistical significance level was taken as p<0.05. Results: In our study, 49% of those in the control group were male, 51 % were female; 27.3 % of those in the obesity group are men and 72.7 % are women. The mean age of the control group was 36.59 ± 10.36 years, the mean height was 168.7 ± 7.44 cm, and the mean weight was 65.51 ± 8.15 kg. The mean age of those in the obesity group was 44.81 ± 12.01 years, the mean height was 161.94 cm ± 8.56, and the mean weight was 99.89 ± 11.47 kg. Considering the results obtained in the study, it was observed that the rate of women in the obesity group was higher than that of men. In addition, the diagnoses of hyperlipidemia, diabetes mellitus, hypertension and coronary artery disease are more common in the obesity group. When the average age of those in the obesity group was examined, it was observed that it was significantly higher than the control group. Similarly, the weight averages of those in the obesity group were also significantly higher than in the control group. Again, in our study, the average height of those in the obesity group was lower than the control group. When laboratory values were examined, glucose, fasting insulin and HbA1c, ALT, Total Cholesterol, triglyceride, LDL-Cholesterol, leukocytes, thrombocyte, RDW values were found to be significantly higher in the obesity group compared to the control group. According to genetic analysis results, there is no significant relationship between obesity and control groups and rs1800795, rs671, rs4969169, rs1061489 and rs1544410 polymorphisms. However, rs8064821 polymorphism was found at a significantly higher rate in the obesity group. While 35% of those in the control group had rs1800795 polymorphism heterozygous and 61 % were homozygous normal, 33 % of those in the obesity group had rs1800795 polymorphism heterozygous and 59 % were homozygous normal. 100% of those in the control group and 100 % of those in the obesity group were homozygous mutants for the rs671 polymorphism. While 12% of those in the control group had rs4969169 polymorphism heterozygous and 88% were homozygous normal, 19 % of those in the obesity group had rs4969169 polymorphism heterozygous and 81 % were homozygous normal. 100% of those in the control group and 100 % of those in the obesity group were homozygous mutants of the rs1061489 polymorphism. While 38% of those in the control group had rs1544410 polymorphism homozygous mutant, 43 % heterozygous and 19% homozygous normal, 45 % of those in the obesity group had rs1544410 polymorphism homozygous mutant, 37% heterozygous and 18 % homozygous normal. While 50 % of those in the control group had rs8064821 polymorphism homozygous mutant, 42% heterozygous and 8% homozygous normal, 64 % of those in the obesity group had rs8064821 polymorphism homozygous mutant, 34 % heterozygous and 2 % homozygous normal. However, it was observed that those in the obesity group were homozygous mutants with a higher course, while the rate of heterozygous and homozygous normal was higher in the control group. Conclusion: According to the results of genetic analysis in our study, there was no significant relationship between rs1800795, rs671, rs4969169, rs1061489 and rs1544410 polymorphisms between obesity and control groups. However, according to the findings, a significant relationship was found between the obesity group and the rs8064821 gene polymorphism. In conclusion, this study revealed differences between obesity and control group such as gender distribution, disease diagnoses, anthropometric measurements and laboratory values. These findings suggest that obesity is associated with various health risks and that genetic factors may play a role in obesity risk.
