INTRODUCTION: The National Institutes of Health Stroke Scale (NIHSS) is a scale used to determine the severity of the disease and to make treatment decisions in ischemic stroke patients. However, the need for a neurologist to look for NIHSS before thrombolytic therapy may prolong the treatment process. The primary aim of our study was to demonstrate the agreement between emergency physicians and neurologists in the NIHSS assessment in patients with acute ischemic stroke; The secondary aim is to show the compatibility of the thrombolytic decision made between the two physician groups and to reveal the effect of the thrombolytic treatment decision made by the neurologists on the treatment delay. MATERIALS AND METHODS: Our prospective, single-center, observational planned study included patients who presented to the emergency department of SBU Trabzon Faculty of Medicine Kanuni Training and Research Hospital in the first 24 hours after the onset of symptoms and were diagnosed with ischemic stroke between 15.09.2022 and 15.06.2023. NIHSS was evaluated by the emergency medicine physician and neurologists who evaluated the patient in the emergency department, and the decisions on whether to give thrombolytic or not and the time taken for this decision were recorded and compared. RESULTS: A very high agreement was found when the total NIHSS scores of emergency medicine physicians and neurologists were compared (ICC = 0.973 [95%CI 0.96 - 0.98]). When NIHSS parameters were evaluated individually, the state of consciousness (K=0.842), consciousness level questions (K=0.888), consciousness level orders (K=0.806), visual field (K=0.796), left arm motor examination (K=0.858), right arm motor examination (K=0.883), left leg motor examination (K=0.879), right leg motor examination (K=0.865), speech (K=0.792) and dysarthria (K=0.717) parameters were found to be in high agreement. A moderate level of agreement was observed in view (K=0.647), facial paralysis (K=0.676), ataxia (K=0.574), and sensory examination (K=0.548). Only very low agreement was found in the neglect parameter (K=0.234). Thrombolytic therapy was applied to 52 (46.8%) of the 111 patients included in the study. A high level of agreement was found between the decisions of emergency medicine physicians and neurologists whether to give thrombolytic therapy or not (K=0.873). In patients given thrombolytic therapy, emergency medicine physicians made a decision for thrombolytic treatment earlier than neurologists, and there was a significant difference of 14±12 minutes between the decisions of emergency physicians and neurologists (p<0.001). CONCLUSION: There is a high agreement between emergency medicine physicians and neurologists in the NIHSS evaluation and thrombolytic decision in patients with acute ischemic stroke. According to our results, emergency medicine physicians can give thrombolytic treatment in accordance with neurologists and this shortens the time for thrombolytic treatment.
