Özet

Giriş ve Amaç: Platelet rich plasma (PRP) tam kandan elde edilen, trombosit konsantrasyonu ile zenginleştirilmiş bir otolog plazma preparasyonudur. İçerdiği yüksek trombosit konsantrasyonu ve fizyolojik seviyenin üstündeki büyüme faktörleri nedeniyle düşük iyileşme potansiyeline sahip dokularda onarımı destekleyen rejeneratif bir uyarı sağlamak için tıbbın çeşitli alanlarında kullanılmaktadır. Bu çalışmada, elde edilmesi ve uygulanması kolay ve maliyeti düşük olan PRP'nin timpanoplasti yapılan kronik otitis medialı hastalarda intraoperatif olarak kullanımının greft tutma başarısı ve odyolojik sonuçlar üzerine etkisi araştırıldı. Yöntem: Basit randomizasyon ile oluşturulan çalışma grubundaki hastalarda operasyon sırasında, otolog PRP emdirilmiş spongostanlar hem orta kulağa hem de greftin lateraline yerleştirildi. Kontrol grubundaki hastalarda ise spongostanlar kuru olarak, herhangi bir ürünle muamele edilmeden aynı lokalzasyonlara yerleştirildi. Tüm hastalar aralıklı olarak kontrol edildi. Postoperatif 2. ay kontrolünde çalışma ve kontrol gruplarındaki kulaklarda greftin güncel durumu kaydedilip tüm hastalar odyometrik olarak değerlendirildi. Sonuçlar istatistiksel olarak kıyaslandı. Bulgular: Çalışmaya toplam 48 hasta (27 kadın, 21 erkek) hasta dahil edildi. Cerrahi sahaya intraoperatif PRP'li spongostan yerleştirilen 24 hasta çalışma grubuna, PRP uygulanmayan 24 hasta kontrol grubuna alındı. Postoperatif 2. ay kontrolünde çalışma grubunda 22 hastada (%91,6) greftin intakt olduğu, kontrol grubunda ise 17 hastada (%70.8) timpanik membrandaki perforasyonun kapanmış olduğu gözlendi. Niceliksel olarak çalışma grubunda kontrol grubu ile kıyaslandığında daha yüksek greft başarısı sağlanmış olsa da aralarında istatistiksel olarak anlamlı fark saptanmadı (p=0,068). Bununla beraber, odyometrik değerlendirme sonuçları, hava-kemik yolu aralığının çalışma grubundaki hastalarda (10.67±7,90 dB) kontrol grubuna (12,34± 0,71dB) kıyasla istatistiksel olarak anlamlı miktarda azaldığını gösterdi (p=0,002). Sonuçlar: Bu çalışmada PRP uygulanan hastalarda işitme kazancı kontrol grubuna göre istatistiksel olarak anlamlı düzeyde yüksek iken, greftin tutma oranı daha yüksek olsa da iki grup arasında istatistiksel olarak anlamlı farklılık tespit edilmedi. Örneklem sayısının arttırıldığı cohortlar farklı sonuçlar gösterebileceğinden literatüre katkı sağlayacaktır.

Introduction and aim: Platelet rich plasma (PRP) is an autologous plasma preparation obtained from whole blood, enriched with platelet concentration. It is used in various fields of medicine to provide a regenerative stimulus that supports repair in tissues with low healing potential due to its high platelet concentration and growth factors above physiological level. In this study, the effect of intraoperative use of PRP, which is easy to obtain and apply, and low in cost, on the success of graft retention and audiological results in patients with chronic otitis media who underwent tympanoplasty was investigated. Materials and methods: In the study group formed by simple randomization, autologous PRP-impregnated spongostans were placed both in the middle ear and lateral to the graft during the operation. In the patients in the control group, spongostans were placed in the same localizations as dry, without being treated with any product. All patients were checked intermittently. At the postoperative 2nd month follow-up, the current status of the graft was recorded in the ears of the study and control groups, and all patients were evaluated audiometrically. The results were compared statistically. Results: A total of 48 patients (27 female, 21 male) were included in the study. Twenty-four patients who were placed in the surgical field from spongos with intraoperative PRP were included in the study group, and 24 patients without PRP were included in the control group. In 22 patients (22/24) in the study group at postoperative 2nd month control It was observed that the graft was intact and the perforation of the tympanic membrane was closed in 17 (17/24) patients in the control group. Although more graft success was achieved in the study group when compared to the control group, there was no statistically significant difference between them (p=0.068). However, the audiometric evaluation results showed that the air-bone gap was statistically significantly reduced in patients in the study group (10.67±7.90 dB) compared to the control group (12.34±0.71dB). Conclusion: In this study, although the hearing gain was statistically significantly higher in the PRP group compared to the control group, the success of graft retention was proportionally higher, but there was no statistically significant difference between the two groups. Cohorts with increased sample size will contribute to the literature as they may show different results.
