Özet

Amaç: Bu çalışmada amacımız, ileri derecede varus deformiteli hastalarda yapılan total diz artroplastisi(TDA) uygulamalarımızın sonuçlarının incelenmesidir. Bununla birlikte total diz artroplasti türleri, tarihçesi, endikasyonları, kontraendikasyonları, komplikasyonları, diz eklem anatomisi, kinematiği hakkında genel bilgilerinde sunulması amaçlanmıştır. Gereç ve Yöntem: Bolu Abant İzzet Baysal Üniversitesi Eğitim ve Araştırma Hastanesi Ortopedi ve Travmatoloji Anabilim Dalı'nda Ocak 2015- Mart 2022 yılları arasında ileri derecede varus deformitesi nedeniyle gonartroz tanısı alan ve TDA ile cerrahi olarak tedavi edilen 36 hasta çalışmaya dahil edildi. Hastaların 24 tanesi kadın, 12 tanesi erkek idi.Olguların ortalama takip süresi 16,5 ay, yaş ortalaması ise 69,6'dır. Hastaların operasyon öncesi ve sonrası Knee Society Skoru (KSS), Hospital for Special Surgery (HSS) Skoru, The Western Ontario and McMaster Universities Arthritis Index (WOMAC) ve Oxford Diz Skorlama ve varus-valgus açılanmaları değerlendirildi. Bunlarla birlikte Demografik veri olarak ad, soyad, cinsiyet, yaş ve vücut kitle indeksi (VKİ) kullanıldı. Bulgular: Diz skoru preoperatif dönemde 23,64±8,21, postoperatif dönemde 92,17±3,21 olarak hesaplandı. Fonksiyon skoru preoperatif dönemde 45,28±8,7, postoperatif dönemde 90,67±5,63 olarak hesaplandı. Womac skoru preoperatif dönemde 44,25±9,35, postoperatif dönemde 92,7±3,25 olarak hesaplandı. Oxford skoru preoperatif dönemde 25,08±5,47, postoperaif dönemde 40,78±3,02 olarak hesaplandı. HSS skoru preoperatif dönemde 46,31±6,01, postoperatif dönemde 89,83±2,88 olarak hesaplandı. Preoperatif dönemde bacak-uzunluk filmlerinde ölçülen varus deformitesi ortalaması 18,61±3,64, postoperatif dönemde bacak-uzunluk filmlerinde ölçülen valgus deformitesi ortalaması 4,06±1,49 olarak hesaplandı. İdeal dizilimin sağlandığı grupta diz skoru 93,04±2,96, ideal dizilimin dışında kalan grupta diz skoru 90,18±2,96 olarak hesaplandı.VKİ 30 altı olan grupta postoperatif dönemde diz skorlama sistemlerinde VKİ 30 ve üzerinde olan gruba göre anlamlı fark bulunmuştur. 65 yaş altı ile 65 yaş ve üstü grup arasında preoperatif ve postoperatif skor ölçümlerinde anlamlı bir fark bulunmamıştır. Erkek ve kadın cinsiyetler arasında preoperatif ve postoperatif skor ölçümlerinde anlamlı bir fark bulunmamıştır. Sonuç: İleri derecede varus deformiteli dizlerde preoperatif ölçülen skorlar ile total diz artroplastisi sonrası ölçülen skorlar arasında istatiksel olarak anlamlı bir fark bulunmasına rağmen skorlama sistemlerinin kombine kullanımının hasta memnuniyetinin daha net anlaşılabilmesi için yararlı olacağını düşünmekteyiz. VKİ 30 altında olan grupta hasta memnuniyeti ve diz skorları açısından daha başarılı sonuçlar alınmaktadır. Hastalara cerrrahi planlama yapılmadan önce özellikle morbid obez olan grubun öncelikli olarak kilo vermek açısından değerlendirilmesi gerektiğini düşünmekteyiz. Başarılı cerrahi sonuç ve hasta memnuniyeti için ideal dizilim ve yumuşak doku dengesinin sağlanması gerekmektedir.

Objective: Our aim in this study is to examine the results of our total knee arthroplasty applications in patients with severe varus deformity. In addition, it is aimed to present general information about total knee arthroplasty types, history, indications, contraindications, complications, knee joint anatomy and kinematics. Material and Method: Thirty-six patients diagnosed with gonarthrosis due to advanced varus deformity and treated surgically with TKA between January 2015 and March 2022 in Bolu Abant İzzet Baysal University Training and Research Hospital Department of Orthopedics and Traumatology were included in the study. Twenty-four of the patients were female and 12 were male. The mean follow-up period of the cases was 16.5 months, and the mean age was 69.6 years. Knee Society Score (KSS), Hospital for Special Surgery (HSS) Score, The Western Ontario and McMaster Universities Arthritis Index (WOMAC) and Oxford Knee Score and varus-valgus angulation were evaluated before and after the operation. In addition to these, name, surname, gender, age and body mass index (BMI) were used as demographic data. Findings: The knee score was calculated as 23.64±8.21 in the preoperative period and 92.17±3.21 in the postoperative period. Function score was calculated as 45.28±8.7 in the preoperative period and 90.67±5.63 in the postoperative period. Womac score was calculated as 44.25±9.35 in the preoperative period and 92.7±3.25 in the postoperative period. Oxford score was calculated as 25.08±5.47 in the preoperative period and 40.78±3.02 in the postoperative period. The HSS score was calculated as 46.31±6.01 in the preoperative period and as 89.83±2.88 in the postoperative period. The mean varus deformity measured in the leg-length films in the preoperative period was 18.61±3.64, and the mean valgus deformity measured in the leg-length films in the postoperative period was 4.06±1.49. In the group with ideal alignment, the knee score was calculated as 93.04±2.96, and in the group outside the ideal alignment, the knee score was calculated as 90.18±2.96. In the postoperative period, there was a significant difference in the knee scoring systems in the group with a BMI below 30 compared to the group with a BMI of 30 and above. found. There was no significant difference in preoperative and postoperative score measurements between the group under 65 years of age and those aged 65 and over. There was no significant difference in preoperative and postoperative score measurements between male and female genders. Results: Although there is a statistically significant difference between preoperative and postoperative scores in knees with severe varus deformity, we think that the combined use of scoring systems will be useful for a clearer understanding of patient satisfaction. In the group with a Body Mass Index below 30, more successful results are obtained in terms of patient satisfaction and knee scores. We think that especially the morbidly obese group should be evaluated primarily in terms of weight loss before making a surgical planning for the patients. Ideal alignment and soft tissue balance must be achieved for successful surgical results and patient satisfaction.
