Özet
Amaç; Çalışmamızda acil serviste akut böbrek yetmezliği tanısı alan hastaların periferikperfüzyon indekslerinin değerlendirilerek AKIN ve RİFLE skorlama sistemleriyle karşılaştırılması amaçlanmıştır. Materyal ve Metot; Çalışmaya Tekirdağ Namık Kemal Üniversitesi Tıp Fakültesi 26.07.2022 tarih ve 2022.143.07.10 sayılı etik kurul onayı alındıktan sonra başlanmıştır. Çalışma prospektif tanımlayıcı bir çalışmadır. Bulgular; Bu çalışmaya toplamda 96 hasta dahil edilmiştir. Çalışma metodolojik olarak 66 olgu (%69) ve 30 kontrol (%31) olmak üzere iki gruba ayrılmıştır. Olgu kontrol grubu arasında hipertansiyon (p=0,001), KAH (p=0,009), pH (p=0,015), HCO3 (p=0,001), sodyum (p=0,001), PerfuzyonIndeksı (p=0,001), Ure (p=0,001), ve Kreatının (p=0,001) değişkenleri yönünden istatistiksel olarak anlamlı farklar saptanmıştır. AKIN Skora göre çalışmaya katılan hastaların %63,6'sının Evre 1'de olduğu saptanmıştır. Evre 2 ve 3 sırası ile %28,8 ve %7,6 olarak saptanmıştır. AKIN skor evreleri arasında, pH,CO2,HCO3, Perfuzyonındeksı, Ure, Kreatının, Potasyum yönünden istatistiksel olarak anlamlı farklar saptanmıştır (p<0,05). RıfleIndeksine göre çalışmaya katılan hastaların en fazla Rısk%66,7 (n=44), ikinci sırada ınjury %21,2 (n=14), sonrasında ise %6,1 ile failure ve loss saptanmıştır. Rıfleındeksi evreleri arasında O2, CO2,HCO3, Perfuzyonındeksı, Ure, Kreatınınyönünden istatistiksel olarak anlamlı farklar saptanmıştır (p<0,05). Sonuç;Bu çalışmada, perfüzyon indeksi (PI) ve kan gazı değerlerinin, ABH'nin erken tanısındaki potansiyel rolü AKIN ve RIFLE skorlama sistemleriyle karşılaştırılmıştır. Hem AKIN hem de RIFLE indekslerinin akut böbrek hasarının evrelerinin değerlendirilmesinde etkili araçlar olduğu sonucuna varılmıştır.

Aim:Our study aimed to evaluatethe peripheral perfusion index values of patients diagnosed with acute kidney failure in the emergency department and compare them with the AKIN and RIFLE scoring systems. Materials and Methods: The study commence dafter obtaining ethical approval from the Namik Kemal University Medical Faculty in Tekirdag on 26.07.2022 with the reference number [2022.143.07.10]. This research is a prospective descriptive study. Results: A total of 96 patients were included in this study. Methodologically, the study was divided into two groups: 66 cases (69%) and 30 controls (31%). Statistically significant differences were observed between the case-control groups in terms of hypertension (p=0.001), CAD (p=0.009), pH (p=0.015), HCO3 (p=0.001), sodium (p=0.001), Perfusion Index (p=0.001), Urea (p=0.001), and Creatinine (p=0.001). Accordingtothe AKIN Score, 63.6% of thepatients were determined to be in Stage 1. Stages 2 and 3 were found to be 28.8% and 7.6%, respectively. Statistically significant differences were found between the AKIN score stages in terms of pH, CO2, HCO3, Perfusion Index, Urea, Creatinine, and Potassium (p<0.05). According to the RIFLE Index, the majority of the patients were at Risk (66.7%, n=44), followed by Injury (21.2%, n=14), and lastly, 6.1% were identified with failure and loss. Statistically significant differences were detected between the RIFLE index stages in terms of O2, CO2, HCO3, Perfusion Index, Urea, and Creatinine (p<0.05). Conclusion:In this study, the potential role of the perfusion index (PI) and blood gas values in the early diagnosis of AKI was compared with the AKIN and RIFLE scoring systems. It was concluded that both the AKIN and RIFLE indices are effective tools in assessing the stages of acute kidney injury. KeyWords; AKIN Index, RIFLE Score, Perfusion Index, AKI.
