Özet

Bu çalışma Multiple Sklerozlu(MS)hastalarda mekanik hippoterapi cihazıyla yapılan egzersizlerin ve Cawthorne Cooksey egzersizlerinin denge, baş dönmesi, yorgunluk ve yaşam kalitesi üzerine etkilerini incelemek amacıyla yapılmıştır. Çalışmaya, MS tanılı 34'ü kadın 16'sı erkek 50 hasta dahil edildi. Katılımcılar rastgele 25 kişilik 2 farklı egzersiz grubuna ayrıldı. İki gruba da 35 dakikalık geleneksel fizik tedavi programı uygulanırken; ek olarak ilk gruba mekanik hippoterapi cihazıyla yapılan egzersizler ikinci gruba ise Cawthorne Cooksey egzersizleri 15'er dakika verildi. Bu egzersizler gruplara 8 hafta boyunca, haftada 3 gün, günde tek seans(15 dakika)olacak şekilde uygulandı. Katılımcıların sosyodemografik bilgileri kaydedildi. Katılımcıların denge düzeyleri Tinetti Denge Değerlendirme Ölçeği; baş dönmesi etkilenimleri Baş Dönmesi Engellilik Envanteri; yorgunluk şiddetleri Yorgunluk Şiddet Ölçeği; yorgunluk etki düzeyleri Yorgunluk Etki Ölçeği; yaşam kaliteleri Ferrans&Powers Yaşam Kalitesi İndeksi MS Uyarlaması ile çalışma öncesi ve sonrasında değerlendirildi. Tedavi sonrasında Cawthorne Cooksey egzersiz grubu hastalarında yorgunluk şiddeti ve yorgunluk etki ölçümlerinde azalma daha belirgin gözlenmiştir(Zaman test p0,05). Cawthorne Cooksey egzersiz grubu hastalarında Yaşam Kalitesi Aile alt parametresi ölçümünde artış daha belirgin gözlenmiştir. Mekanik Hippoterapi egzersiz grubu hastalarında Sağlık ve Fonksiyonellik, Sosyal ve Ekonomik, Psikolojik ve İnançsal alt parametreleri ve Yaşam Kalitesi ölçümlerinde artış daha belirgin gözlenmiştir (Zaman ii test p0,05). Çalışmamız Multiple Sklerozlu bireyleri çok yönlü değerlendiren bir çalışmadır. Fizyoterapistlerin uygulayacakları nörorehabilitasyon programlarının egzersiz içeriği hakkında literatüre önemli katkılar sağlayabilir. Sonraki çalışmaların daha uzun takipli olması, daha büyük bir örneklem grubunu içermesi ve benzer başka tedavi protokollerinin karşılaştırmalı incelenmesi önerilmektedir.

This study was conducted to examine the effects of exercises performed with a mechanical hippotherapy device and Cawthorne Cooksey exercises on balance, dizziness, fatigue and quality of life in patients with Multiple Sclerosis.50 definitively diagnosed patients with Multiple Sclerosis (MS) participated in the study.Of these patients, 34 were female and 16 were male. The participants were randomly divided into 2 different exercise groups of 25 people. While a 35-minute traditional physical therapy program was applied to both groups; In addition, the first group was given exercises with a mechanical hippotherapy device and the second group was given Cawthorne Cooksey exercises for 15 minutes. These exercises were applied to both groups for 8 weeks, 3 days a week, once a day, with each session lasting 15 minutes. The sociodemographic information of the participants was recorded. The participants balance levels were assessed using the Tinetti Balance Rating Scale, the effects of dizziness were measured using the Dizziness Disability Inventory, fatigue severity was evaluated using the Fatigue Severity Scale, fatigue impact levels were measured using the Fatigue Impact Scale, and quality of life was assessed before and after the study using the Ferrans&Powers Quality of Life Index MS Adaptation. After the treatment, a significantly greater decrease in fatigue severity and fatigue impact measurements was observed in patients in the Cawthorne Cooksey exercise group (Time test p0.05). In the Cawthorne Cooksey exercise group, a more prominent increase in the Quality of Life Family subparameter was observed in patients.The increase in the sub-parameters of Health and Functionality, Social and iv Economic, Psychological and Belief, as well as the overall Quality of Life measures, was significantly greater in patients in the Mechanical Hippotherapy exercise group (Time test p0.05). The study is a multidimensional evaluation of individuals with Multiple Sclerosis. It can make important contributions to the literature about the exercise content of neurorehabilitation programs that physiotherapists will implement. It is recommended that future studies should have longer follow-up, include a larger sample group, and compare other similar treatment protocols.
