The current models of eating disorders predominantly focus on cognitive and behavioural aspects however tend to leave causal factors associated with mentalization, affect regulation and coherence of mind out of the frame. In order to address these shortcomings in the conceptualization of eating disorders, attachment and mentalization have recently become two interesting domains. However, the role of attachment and mentalization functioning on the development, maintenance, and treatment of the eating disorders in adults is still unclear. The purpose of this review is to review the literature systematically in this promising field in order to elucidate the areas still needed to provide an evidential base for developing new approaches to treatment of the eating disorders. Two databases (PsychINFO and MedLine/Pubmed) were searched from available years of publication in August 2018. Twenty-five studies were identified as meeting criteria for the review. In comparison to controls, those with eating disorders have higher levels of attachment insecurity. The insecure attachment patterns of patients with eating disorders have been associated with the maladaptive mechanisms of coping with distress, body dissatisfaction, and embodiment problems. Moreover, psychotherapeutic process is likely to be influenced by the attachment patterns of patients. Higher attachment avoidance may lead to dropout of the treatment. The available findings also suggest that patients are likely to have poor mentalizing abilities that is especially associated with anorexia nervosa. Improvement in mentalization enhances the therapeutic alliance and promotes better outcomes in the treatment. The positive influence of mentalization on the treatment might be related to a better emotion recognition and affect regulation abilities. This review discusses the potential importance of the attachment and mentalization concepts in the development of new approaches that hypothesize about possible causal models of illness and innovative interventions for the treatment of eating disorders
