Özet
Amaç: Bu araştırma total/parsiyel diz protezi ameliyatı sonrası iyileştirici çevre uygulamalarının hastalara cerrahi korkuyu, preoperatif anksiyete, spinal anestezi ajistasyonu, yaşam bulguları, ağrı, hasta memnuniyetini ve şefkatli bakım algısının etkisini belirlemek amacıyla yapıldı. Materyal ve Metot: Araştırma iyileştirici çevresel tasarımlı modelde olup araştırma 93 (31 kontrol, 31bilgilendirme ve 31iyileştirici çevre grubu) diz protezi yapılan hasta ile yürütüldü. Kontrol grubuna girişim yapılmazken bilgilendirme grubuna ameliyathane ve ameliyat süreci hakkında bilgilendirme yapıldı, iyileştirici çevre grubuna iseameliyathane içerisinde bulunan premedikasyonda odasında çevre düzenlemesi, relaksasyon müzik dinletisi, derin nefes alam egzersizi yaptırıldı. Veriler Hasta Tanıtıcı Özellikler Formu, Cerrahi Korku Ölçeği, Perioperaif Anksiyete Ölçeği, Spinal Anestezi Ajitasyon Ölçeği, Yaşam Bulguları Takip Formu, Numerik Orantılama Ölçeği, Watson İyileştirme Süreçlerine Göre Hasta Memnuniyet Formu, Şefkatli Bakım Algısı Formu kullanılarak toplandı. Verilerin değerlendirilmesinde; sayı, yüzde, ortalama, standart sapma, Ki-kare, ANOVA testi, ShapiroWilk testi, Kolmogorov Simirnov testi, Q-Q plot, skewness ve kurtosis, Kruskal Wallis testi kullanılarak analiz yapıldı. Bulgular: Gruplar arasındayaşam bulguları (vücut sıcaklığı, nabız, kan basıncı, solunum sayısı, satürasyon) değerlendrilmesinde iyileştirici çevre grubunda olumlu etkilerinin olduğu bulunmuştur (p<0.05). Cerrahi korkunun kontrol grubunda ortalama 55.06±14.03 bilgilendirme grubunda 48.52±8.11 ve iyileştirici çevre grubunda 22.55±10.04 olduğu görülmektedir. İyileştirici çevre grubunda cerrahi korkunun daha düşük olduğu bulunmuştur (p<0.05). Preoperatif anksiyete ve bilgi ölçeği (APAIS)nin kontrol grubunda ortalama 15.58±3.79, bilgilendirme grubunda ortalama 11.74±2.84, iyileştirici çevre grubunda ortalama 6.97±2.65 olduğu görülmektedir. İyileştirici çevre grubunda preoperatif anksiyetenin daha düşük olduğu bulunmuştur (p<0.05). Spinal Anestezi Ajitasyon Ölçeği (SAAÖ) puan ortalamasının kontrol grubunda ortalama 27 ± 0, bilgilendirme grubunda ortalama 26.32 ± 0.87ve iyileştirici çevre grubunda 26.19 ± 5.35olduğu görülmektedir. Toplam SAAÖ de fark olmamasına karşın Sözel Agresif Olmayan Davranışlar alt boyurundaiyileştirici çevre grubunda ajitasyonun daha düşük olduğu bulunmuştur (p<0.05). Hastaların memnuniyetlerinin ve şefkatli bakım algısıkarşılatırılmasında iyileştirici çevre grubunun memnuniyet düzeyi ve şefkatli bakım algısı daha yüksek bulunmuştur (p<0.05). Sonuç: Diz protezi ameliyat sonrası iyileştirici çevre uygulamalarının cerrahi korkuyu, perioperaif anksiyeteyi, spinal anestezi ajitasyonunu ve ağrıyı azalttığı, yaşam bulgularının üstünde olumlu etki gösterdiği, hasta memnuniyetini ve şefkatli bakım algısını artırdığı sonucuna ulaşıldı.

Aim: This study aims to determine the effects of healing environment practices on surgical fear, preoperative anxiety, spinal anesthesia agitation, vital signs, pain, patient satisfaction, and perception of compassionate care in patients who have undergone total/partial knee prosthesis surgery. Materials and Methods: Conducted with an experimental research design, this study was carried out with 93 patients who had undergone knee prosthesis surgery (31 in the control group, 31 in the informed group, and 31 in the healing environment group). While the control group received no intervention, the informed group received information about the operating room environment and the surgical procedure. The healing environment group, on the other hand, received information about the operating room and surgical process following environmental adjustments in the premedication room, listened to relaxation music, and performed deep breathing exercises. The data were collected using the Patient Introductory Characteristics Form, Surgical Fear Questionnaire, Perioperative Anxiety Scale, Spinal Anesthesia Agitation Scale, Vital Signs Monitoring Form, Numeric Rating Scale, Patient Satisfaction Form Based on Watson's Healing Processes, and Perceptions of Compassionate Care Questionnaire. Numbers, percentages, mean, standard deviation, Chi-square, ANOVA test, Shapiro-Wilk test, Kolmogorov-Smirnov test, Q-Q plot, skewness and kurtosis, and Kruskal-Wallis test were used in the evaluation of the data. Results: When assessing the vital signs at different times among the groups, it was found that the vital signs of the healing environment group were positively affected (p<0.05). The mean surgical fear score was 55.06±14.03 in the control group, 48.52±8.11 in the informed group, and 22.55±10.04 in the healing environment group, showing a significant reduction in surgical fear in the healing environment group (p<0.05). The mean preoperative anxiety and information scale score was 15.58±3.79 in the control group, 11.74±2.84 in the informed group, and 6.97±2.65 in the healing environment group, with significantly lower preoperative anxiety in the healing environment group (p<0.05). The mean Spinal Anesthesia Agitation score was 27 ± 0 in the control group, 26.32 ± 0.87 in the informed group, and 26.19 ± 5.35 in the healing environment group. Although there was no statistical difference in the total spinal anesthesia agitation scores, the healing environment group had lower scores in the Nonverbal Aggressive Behavior sub-scale (p<0.05). Comparing satisfaction and perception of compassionate care scores between groups, the healing environment group had higher satisfaction and perception of compassionate care (p<0.05). Conclusion: Healing environment practices during the perioperative period were found to reduce surgical fear, perioperative anxiety, spinal anesthesia agitation, and pain, regulate vital signs, and increase patient satisfaction and perception of compassionate care.
