Özet

IV. ÖZET Alkol büyük ölçüde karaciğerde metabolize olur ve alkolizm bu organda şiddetli patolojilere sebep olur. Diğer yandan, tüm vücut sisteminde değişik patolojilerin sebebi olarak diabet ; bir başka önemli hastalıktır. Alkolizm ve diabet karaciğerde ciddi hasarlara neden olurlar. Etanolun hepatik oksidasyonu ; NAD+ nin NADH a redüklenmesine ve beraberinde birçok metabolik bozuklukla karakterize şiddetli redoks değişimlerine, bunun sonucunda da serbest oksijen radikallerinin artmasına, karaciğer a-tokoferol içeriğinin azalmasına ve lipid peroksidasyonu yolu ile karaciğer hasarının oluşmasına neden olur. Alkol tüketiminin dokuda etkilerini ölçmenin en basit metodu, alkol kullanımı ile yükselen hepatik enzimlerin serum düzeylerini ölçerek karaciğer hasarını değerlendirmektir. Alkol kullanımın en iyi göstergesi olarak bilinen enzimler ; GGT, ALP ve transaminazlardır. Bu tez çalışmasının esas amacı diabetik sıçan modelinde alkolik karaciğer oluşturmaktı ve daha sonra bir grup sıçana yeterli miktarda E vitamini ve diğer bir gruba yeterli miktarda kateşin vererek antioksidan terapiyle karaciğer hasarının ne derece geri dönüşebileceğini görmekti. Amaç olarak odaklandığımız diğer bir nokta da ; değişimleri hem karaciğer dokusu hem de kanda marker enzim aktiviteleri, lipid peroksidasyonu düzeyleri ve histokimyasal çalışmalar yoluyla incelemekti. Bu tez çalışmasının sonucunda, diabetik-alkolik grupta hafif karaciğer hasar oluşumu gözlenmiştir. Hem karaciğer dokusunda hem de kanda marker enzimler ve lipid peroksidasyon seviyeleri anlamlı olarak yükselmiştir. Histokimyasal çalışmalar da bu bulguları desteklemiştir. E vitamini ve kateşin ile yapılan antioksidan terapinin, bu seviyedeki karaciğer hasarında anlamlı ve yeterli seviyede iyileşme meydana getirdiğini söyleyebiliriz. 59

VII. SUMMARY Alcohol is predomiantly metabolised in the liver and alcoholism causes severe pathologies in this organ. On the other hand, diabetes being the reason of different pathogeneses of the whole body system is also another important disease. Alcoholism and diabetes lead to serious damages to the liver. Hepatic oxidation of ethanol causes the reduction of NAD+ to NADH, the latter produces a striking redox change with various associated metabolic disorders ; hence an increase in the generation of oxygen free radical species, reduction of the hepatic a-tokoferol content and existance of liver damage via lipid peroxidation. The simplest method of gauging the tissue effects of alcohol consumption is the assessment of liver damage by measuring the serum levels of the hepatic enzymes which are raised as a result of alcohol abuse. The enzymes most regarded as being the best indicators of alcohol abuse are ; GGT, ALP and transaminases. The primary aim of this study was to form alcoholic liver in diabetic rat model, and then administering one group of rats the adequate amount of catechin and the other group the adequate amount of vitamin E to see the extent of recovery of the liver damage by the antioxidant therapy. The next point we aimed to focus on was to observe the changes both in liver tissue and blood via marker enzymes' activities 5lipid peroxidation and histochemical studies. As a result of this study, a slight liver damage occurrence was observed in the diabetic-alcoholic group.As well, both in the liver tissue and in blood, marker enzymes and levels of lipid peroxidation were significantly elevated. Histochemical studies also supported these findings. We would like to conclude by stating that the antioxidant therapy of vitamin E and catechin produced a significant and adequate extent of recovery at this level of liver damage. 60
