Introduction and Objective: Serratus anterior plane block (SAPB) and modified pectoral nerve block (PECS-2) are interfascial plane blocks that can be used for analgesic purposes in breast surgery. In our study, we aimed to find the difference in postoperative analgesic requirements in patients with breast reduction surgery in the SAPB and PECS-2 group compared to the control group. We also aimed to investigate whether these two block techniques are superior to each other in postoperative analgesia. Material and Method: This study was a single-center, prospective, observational study. Our study was conducted at Cumhuriyet University Medical Faculty Hospital. The study was started after the decision dated 03.03.2021 and numbered 2021-03/05 was obtained from the Clinical Research Ethics Committee of Cumhuriyet University Faculty of Medicine. A total of 60 patients with American Society of Anesthesiologist scores (ASA) I-II and age range of 18-70 who underwent breast reduction surgery were included in the study. The patients were divided into three groups and no block technique was applied to the control group consisting of 20 patients, SAPB was applied to 20 patients and PECS-2 block was applied to 20 patients. All patients were awakened after 1 gram of paracetamol iv was administered. Patient-controlled analgesia device (PCA) containing 4 mg tramadol in 1 ml was used in all patients and the postoperative tramadol consumption of the patients at the 1st, 6th, 12th, and 24th hours was recorded. Static numerical rating scale scores (S-NRS) and dynamic numerical rating scale scores (D-NRS) at 1st, 6th, 12th, and 24th hours of all patients were recorded. Side effects secondary to the block were noted in the patients. Opioid-dependent side effects seen in patients were noted. Results: As a result of this study, tramadol consumption of patients in the SAPB and PECS-2 groups was found to be significantly lower than the control group patients. (p<0.001) There was no significant difference in tramadol consumption between patients in the PECS-2 and SAPB groups. D-NRS and S-NRS scores at 1st, 6th, 12th, and 24th hours were significantly higher in the control group compared to the patients in the SAPB and PECS-2 groups. (p<0.001) There was no significant difference in S-NRS and D-NRS scores between patients in the PECS-2 and SAPB groups. None of the patients had complications secondary to the block. No opioid-dependent side effects were seen among the patients. Conclusion: In our study, we found that SAPB and PECS-2 blocks are two effective methods in postoperative analgesia in breast reduction surgeries. We did not find any significant difference between the methods. We think that both methods can be preferred in breast reduction surgeries based on the experience of the anesthesiologist.
