This study aims to evaluate the effectiveness of telerehabilitation-based exercises in myofascial temporomandibular dysfunction (TMD) accompanied by bruxism. The research was conducted at the Faculty of Dentistry at Nevşehir Hacı Bektaş Veli University. The study included 30 individuals aged 18-60 who were diagnosed with bruxism and temporomandibular dysfunction. Participants were randomly divided into two groups: the telerehabilitation group (n=15) and the home exercise group (n=15). The telerehabilitation group received exercise guidance through WhatsApp video calls three times a week, while the home exercise group was provided with brochures explaining how to perform the exercises. Both groups followed Rocabado's 6x6 exercise program for four weeks. Rocabado's 6x6 exercise program includes a series of movements designed to treat temporomandibular joint dysfunction. These exercises address the postural relationships between the head and neck, the neck and shoulders, and the lower and upper jaws, with patients instructed to complete each activity six times a day. The evaluation methods included a demographic information form, Temporomandibular Joint (TMJ) range of motion measurements, pain assessment using the Visual Analog Scale (VAS), Perceived Stress Scale (PSS), Jenkins Sleep Scale (JSS), Oral Habits Questionnaire, and Fonseca Anamnestic Index (TMD severity). Assessments were conducted before and after the treatment. Results showed that the telerehabilitation group demonstrated greater improvements in pain, sleep quality, stress, and oral habits compared to the home exercise group, although these improvements were not statistically significant. Pain assessment using VAS showed that the telerehabilitation group's scores decreased significantly post-treatment (p<0.001). In the home exercise group, these scores also showed significant improvement (p<0.001). Maximum mouth opening (MMO) measurements in the telerehabilitation group showed significant improvement post-treatment (p<0.001), as did the home exercise group (p<0.001). The Perceived Stress Scale results showed significant improvement post-treatment in the telerehabilitation group (p<0.001) and the home exercise group (p=0.037). According to the Jenkins Sleep Scale, the telerehabilitation group showed significant improvement post-treatment (p<0.001), and the home exercise group also showed significant improvement (p=0.001). Telerehabilitation group showed significant improvements in oral habits (p=0.005) and Fonseca Anamnestic Index (TMD severity) (p=0.084) scores. These findings suggest that telerehabilitation could be an effective method for treating TMD and bruxism. Telerehabilitation facilitates easier access to treatment and results in better outcomes during the treatment process. It was also observed that telerehabilitation increases patient compliance and helps patients perform exercises regularly. Therefore, it is recommended that telerehabilitation be more widely used in clinical practice and that further research be conducted in this field.
