Özet
Araştırma, subkutan heparin enjeksiyonunda ShotBlocker ve lokal soğuk uygulamanın ağrı ve hasta memnuniyetine etkisini belirlemek amacıyla tek kör, randomize kontrollü, deneysel olarak gerçekleştirilmiştir. Araştırma, Nisan 2022- Nisan 2024 tarihleri arasında İzmir Atatürk Eğitim ve Araştırma Hastanesinde Kardiyoloji ve İç Hastalıkları kliniklerinde yatmakta olan 208 hasta ile gerçekleştirilmiştir. Hastalar rastgele sıralama randomizasyon algoritması kullanılarak ShotBlocker (n=52), ShotBlocker plasebo (n=52), lokal soğuk uygulama (n=52) ve kontrol (n=52) grupları olmak üzere dört gruba atanmıştır. ShotBlocker grubundaki hastaların koluna enjeksiyon sırasında ShotBlocker yerleştirilmiş ve enjeksiyon süresince parmak uçları ile hafifçe bastırılarak açıklığın olduğu kısımdan enjeksiyon gerçekleştirilmiştir. ShotBlocker plasebo grubundaki hastaların koluna enjeksiyon sırasında ShotBlocker'ın arka yüzü yerleştirilmiş ve enjeksiyon süresince parmak uçları ile hafifçe bastırılarak enjeksiyon gerçekleştirilmiştir. Lokal soğuk uygulama grubundaki hastaların koluna soğuk paket kullanılarak beş dakika süre ile lokal soğuk uygulama yapılmış ve ardından enjeksiyon gerçekleştirilmiştir. Kontrol grubundaki hastalara ise rutin subkutan enjeksiyon prosedürü ile enjeksiyon gerçekleştirilmiştir. Tüm gruplarda enjeksiyon araştırmacı tarafından gerçekleştirilmiştir. Enjeksiyon sonrası birinci dakikada, hastalara enjeksiyon esnasındaki hissettikleri ağrı ve enjeksiyon memnuniyet durumu farklı bir araştırmacı tarafından değerlendirilmiştir. Veriler, bilgilendirilmiş onam formu, birey tanıtım formu, visüel analog skala ve görsel hasta memnuniyeti ölçeği kullanılarak yüz yüze görüşme tekniği ile toplanmıştır. Araştırmanın uygulanmasında etik kurul izni, kurum izni ve hastalardan yazılı izin alınmıştır. Verilerin analizinde sayı, yüzde, medyan, ortalama ve standart sapma, Mann Whitney U testi, Kolmogorov-Smirnov testi, Kruskal Wallis H testi, Bonferroni ve ki kare testi kullanılmıştır. Araştırma sonucunda kontrol grubu hastalarının ağrı puanlarının ShotBlocker, plasebo, lokal soğuk uygulama grubu hastalarına göre daha yüksek olduğu ve yine kontrol grubu hastalarının memnuniyet puanlarının diğer üç gruba göre daha düşük olduğu sonucuna varılmıştır. Bu sonuçlara göre subkutan heparin enjeksiyonunda ağrıyı azaltmak ve memnuniyeti artırmak için lokal soğuk uygulama ve ShotBlocker'ın klinikte hemşireler tarafından kullanılması önerilmektedir.

The research was conducted as a single-blind, randomized controlled, experimental study to determine the effect of ShotBlocker and local cold application on pain and patient satisfaction in subcutaneous heparin injection. The research was conducted with 208 patients hospitalized in the Cardiology and Internal Medicine clinics at Izmir Atatürk Training and Research Hospital between April 2022 and April 2024. Patients were assigned to four groups: ShotBlocker (n=52), ShotBlocker placebo (n=52), local cold application (n=52) and control (n=52) groups using the randomization algorithm. A ShotBlocker was placed on the arm of the patients in the ShotBlocker group during the injection, and the injection was performed from the part where the opening was located by lightly pressing with the fingertips throughout the injection. During the injection, the back side of the ShotBlocker was placed on the arm of the patients in the ShotBlocker placebo group and the injection was performed by gently pressing with the fingertips throughout the injection. Local cold application was applied to the arm of the patients in the local cold application group for five minutes using a cold pack, and then the injection was performed. Patients in the control group were injected with a routine subcutaneous injection procedure. In all groups, the injection took 30 seconds and was performed by researcher. At the first minute after the injection, the patients were evaluated about the pain they felt during the injection and their satisfaction with the injection by a different reseacher. Data were collected by face-to-face interview technique using an informed consent form, individual identification form, visual analog scale and visual patient satisfaction scale. Ethics committee permission, institutional permission and written permission from the patients were obtained for the implementation of the research. Number, percentage, median, mean, and standard deviation, Mann Whitney U test, Kolmogorov-Smirnov test, Kruskal Wallis H test, Bonferroni and chi-square test were used in the analysis of the data. As a result of the research, it was concluded that the pain scores of the control group patients were higher than the ShotBlocker, placebo and local cold application group patients, and the satisfaction scores of the control group patients were lower than the other three groups. According to these results, it is recommended that local cold application and ShotBlocker be used by nurses in the clinic to reduce pain and increase satisfaction during subcutaneous heparin injection.
