Özet

Bu çalışmanın amacı dini içerikli OKB'ye sahip olup Bilişsel Davranışçı Terapi temelli grup terapisine katılmış fakat OKB rahatsızlığı devam eden bir kadın hastada Dini İçerikli Psiko-Eğitim -4T Modeli- içeren terapi müdahalesinin etkinliğini derinlemesine incelemektir. Bu amaç doğrultusunda nicel ve nitel verilerin entegre edildiği vaka çalışması yöntemi kullanılmıştır. Bu uygulamada hastanın mevcut durumunun değerlendirilmesi adına DSM-5 Bozuklukları için yapılandırılmış Klinik Görüşme-Klinisyen Versiyonu, Klinik Değerlendirme Görüşmesi yapılmış, Yale-Brown Obsesyon Kompulsiyon Ölçeği (Y-BOCS), Penn Dinsel Obsesyonlar Envanteri (PDOE), Brown İnançların Değerlendirilmesi Ölçeği (BİDÖ), Beck Depresyon Envanteri (BDE) ve Beck Anksiyete Envanteri (BAE) nicel ölçümleri üzerinden Ön Test-Son Test ve İzlem Testi ölçümleriyle tedavinin seyri ölçülüp tedavi sonunda genel bir değerlendirmeyi içeren“Terapi Değerlendirme Formu”yanıtlanmıştır. Tedavi sürecinde, BDT'nin temel OKB müdahalesine ek olarak özellikle dini içerikli OKB açısından önemli bir etkisi olacağı düşünülen; 'tasdik', 'iman' ve 'kalp' gibi dini bilgi ve kavramlar öneren 4T psiko-eğitim modeli uygulanmış ve süreç sonunda Y-BOCS sonuçlarına göre hastanın OKB şiddetinin“orta”düzeyden“hafif”düzeye indiği; 1 ve 3 aylık izlemlerde de bu sonuçların korunduğu gözlemlenmiştir. Benzer şekilde Penn Dinsel Obsesyonlar Envanteri (PDOE), Beck Depresyon Envanteri (BDE) ve Beck Anksiyete Envanteri (BAE) puanları da tedavi boyunca azalma eğilimi göstermiş olup, bu ölçeklerdeki puan düşüşleri de olumlu değerlendirilmiştir. Aynı zamanda içgörü düzeyinde de tedaviyle birlikte artış görülmüştür. Bu anlamlı değişimlerin yanı sıra hastadan alınan yazılı ve sözlü geribildirimlerle de 4T müdahalesinin söz konusu dini içerikli OKB vakasında etkili olduğu görülmüştür. Gelecek çalışmalarda, 4T psiko-eğitim modelinin etki gücünün daha belirgin şekilde ortaya konulabilmesi için BDT'nin bilişsel müdahalesi yerine ERP'ye eklendiği örnek çalışmalar yapılması önemli olacaktır.

The aim of this study is to examine in depth the effectiveness of a therapy intervention containing religious psycho-education section -4T Model- on a female patient with an ongoing religious OCD, although that she has been previously participated in a Cognitive Behavioral Therapy (CBT) based group therapy. The case study method, which integrates quantitative and qualitative data, was employed. To assess the current level of impairment caused by OCD, the Clinical Evaluation Interview, based on the DSM-5 criteria, was conducted. The Yale-Brown Obsessive Compulsive Scale (Y-BOCS) Self-Report Form, the Penn Inventory of Scrupulosity (PIOS), the The Brown Assessment of Beliefs Scale (BABS), Beck Depression Inventory (BDI) and Beck Anxiety Inventory (BAI) were used as Pre-Test, Post-Test and Follow-up Test measures to measure the progress and a Therapy Evaluation Form, which includes a general evaluation at the end of the treatment was administered. The treatment course considered the potential impact of CBT, particularly for religious-based OCD. The 4T psycho-education model, which incorporates religious knowledge and concepts such as 'confirmation,' 'faith,' and 'heart,' was applied. At the end of the process, it was observed that the severity of OCD, as measured by the Y-BOCS, decreased from“moderate”to“mild,”and these improvements were maintained during follow-up assessments. Similarly, PIOS, BDI and BAI also decreased throughout the treatment, indicating positive changes. The patient's written and verbal feedback further supported the effectiveness of the 4T intervention in this case of religious OCD. At the same time, the level of insight increased with treatment. In addition to these significant changes, written and verbal feedback from the patient showed that the 4T intervention was effective in the aforementioned scrupulosity case. In future studies, it will be important to conduct case studies in which CBT is added to ERP (Exposure and Response Prevention) instead of cognitive intervention in order to demonstrate the effectiveness of the 4T psycho-education model more clearly.
