Açık kalp cerrahisinde erektor spina plan bloğunun postoperatif analjezi ve gelişmiş iyileşme süreci (ERAS) üzerine etkileri: prospektif, randomize, kontrollü çalışma

Özet

Giriş ve Amaç: Kalp cerrahisinde perioperatif iyi bir ağrı yönetimi morbidite ve mortalitede önemli rol oynar. Son yıllarda Erektör Spina Plan Bloğunun kardiyak cerrahide etkin postoperatif analjezi sağladığı gösterilmiştir. ERAS protokolleri, geleneksel perioperatif hasta yönetimine kıyasla genel komplikasyonlarda azalma ve hastanede kalış süresinde azalma ile ilişkilendirilmiştir. Kalp cerrahisine ait ERAS Protokollerinde postoperatif ağrı yönetiminde multimodal analjezi ve opioid kullanımının azaltılması vurgulanmıştır. Bu çalışmamızda amacımız, açık kalp cerrahisi geçiren hastalarda erektor spina plan bloğunun postoperatif analjezi ve gelişmiş iyileşme süreci (ERAS) üzerine etkilerini belirlemektir. Gereç ve Yöntem: Prospektif, randomize, kontrollü olarak planlanan çalışmamıza hastanemiz yerel etik kurulundan onay alındıktan sonra başlandı. Çalışmaya Median sternotomi ile kalp cerrahisi uygulanacak 18 yaş üzeri, ASA II-III 72 hasta dahil edildi. Erektör Spina Plan Bloğu (Grup ESP) ve Kontrol Grubu (Grup K) olarak 2 grup oluşturuldu. Tüm hastaların yaş, cinsiyet gibi demografik verileri, ek hastalıkları kaydedildi. Anestezi öncesi, anestezi indüksiyonu sonrası, sternotomi sırasında ve postoperatif Kalp Tepe Atımı, Tansiyon Arteryel ve SPO2 değerleri, Perioperatif toplam fentanil tüketimi, tahmini kanama miktarı, transfüzyon miktarı, inotrop ihtiyacı varlığı , Cerrahi ve Pompa Süreleri kaydedildi. Postoperatif dönemde ekstübasyon sonrası istirahat ve öksürük sırasında 0., 2., 4., 8., 12., 24. ve 48. Saatteki VAS değerleri, Postoperatif tramadol ihtiyacı kaydedildi. Ekxtübasyon süreleri, ilk mobilizasyon süreleri, dren, kateter, idrar sondası çıkarılma süreleri, Yoğun bakımda ve hastanede kalış süreleri kaydedildi. Bulgular: ESP blok olan grupta ek hastalık oranı ve hipertansiyon görülme oranı daha düşüktü (p<0.05). ESP blok olan grupta indüksiyon sonrası, sternotomi sonrası KTA değeri daha düşüktü (p<0.05). ESP blok olan grupta preop ve sternotomi sonrası ortalama arter basıncı daha yüksekti (p<0.05). ESP blok olan grupta toplam perioperatif fentanil ve postoperatif tramadol kullanım miktarı daha düşüktü (p<0,05). ESP blok olan grupta inotrop ihtiyacı ve kanama miktarı daha düşüktü (p<0.05). ESP blok olan grup ile kontrol grubu arasında mekanik ventilasyon, yoğun bakım yatış, santral venöz kateter ve idrar sondası çekilme süreleri arasında anlamlı farklılık yoktu. ESP blok olan grupta dren çekilme ve hastaneden taburculuk süreleri daha kısaydı (p<0.05). ESP blok olan grupta istirahat halinde ve öksürürken 0.-8.saat arası VAS skorları daha düşüktü (p<0.05). 12.-48.saat VAS skorlarında gruplar arasında farklılık yoktu. Sonuç: Açık kalp cerrahisi geçiren hastalarda uygulanan ESP Blok ekstübasyon sonrası 8. saate kadar istirahat halinde ve öksürme durumunda düşük ağrı skorları, intraoperatif ve postoperatif azalmış opioid tüketimi sağladı. Postoperatif dren çekilme ve hastaneden taburculuk sürelerini kısalttı. İntraoperatif kanama miktarı ve inotrop ihtiyacını azalttı. Ancak yoğun bakım yatış, mekanik ventilasyon süreleri ile santral venöz kateter ve idrar sondası çekilme sürelerini değiştirmedi.

Introduction and Objective: Perioperative pain management in cardiac surgery plays an important role in morbidity and mortality. In recent years, ERAS has been shown to provide effective postoperative analgesia in cardiac surgery. ERAS protocols have been associated with a reduction in overall complications and reduced length of hospital stay compared to traditional perioperative patient management. ERAS Protocols for cardiac surgery emphasize multimodal analgesia and reduction of opioid use in postoperative pain management. In this study, we aimed to determine the effects of erector spina plan block on postoperative analgesia and enhanced recovery process (ERAS) in patients undergoing open heart surgery. Materials and Methods: Our prospective, randomized, controlled study was initiated after obtaining approval from the local ethics committee of our hospital. The study included 72 ASA II-III patients over 18 years of age who were to undergo cardiac surgery via median sternotomy. Two groups were formed as Erector Spina Plan Block (Group ESP) and Control Group (Group K). Demographic data such as age, gender and comorbidities of all patients were recorded. Before anesthesia, after induction of anesthesia, during sternotomy and postoperative Heart Peak Beat, Blood Pressure Arterial and SPO2 values, Perioperative total fentanyl consumption, estimated amount of bleeding, amount of transfusion, presence of inotrope requirement, Surgical and Pump Times were recorded. Postoperative VAS values at 0, 2, 4, 8, 12, 24 and 48 hours after extubation during resting and coughing, postoperative tramadol requirement were recorded. Extubation times, initial mobilization times, drain, catheter and urinary catheter removal times, intensive care unit and hospital stays were recorded. Results: The rate of comorbidity and hypertension were lower in the ESP block group (p<0.05). Post-induction and post-sternotomy heart rate values were lower in the group with ESP block (p<0.05). Mean arterial pressure was higher in the ESP block group preop and after sternotomy (p<0.05). Total perioperative fentanyl and postoperative tramadol use was lower in the ESP block group (p<0.05). Inotrope requirement and amount of bleeding were lower in the group with ESP block (p<0.05). There was no significant difference in the duration of mechanical ventilation, intensive care unit hospitalization, central venous catheter and urinary catheter withdrawal between the ESP block group and the control group. Drain removal and hospital discharge times were shorter in the ESP block group (p<0.05). VAS scores at rest and while coughing were lower in the ESP block group between 0.-8.h (p<0.05). There was no difference in 12th-48th hour VAS scores between the groups. Conclusion: ESP block in patients undergoing open heart surgery resulted in lower pain scores at rest and when coughing up to 8 hours after extubation, and decreased intraoperative and postoperative opioid consumption. It shortened postoperative drain removal and hospital discharge times. Reduced intraoperative bleeding and inotrope requirement. However, it did not change the duration of intensive care unit stay, mechanical ventilation, central venous catheter and urinary catheter withdrawal times.
