SUMMARY PRIMARY HIP HEMIART HROPLASTY FOR UNSTABLE INTERTROCHANTERIC FRACTURES IN ELDERLY PATIENTS Purpose: The purpose of this study is to evaluate, the clinical and radiological results of patients with unstable intertrochanteric fractures treated with primary hemiarthroplasty. Patients and Methods; 39 elderly patients with unstable intertrochanteric fracture of femur were treated by primary hip hemiarthroplasty. Mean age at surgery was 80,1 and average follow-up was 18 months. 23 of them were female and 16 of them were male 39 patients were treated with cemented bipolar hemiarthroplasty, and 21 of them were followed. All of the patients were mobilized by walker or crutches within the six days (average 2,9 days) after the operation. One of them died during the surgery, after the surgery, two patients died during the stay in hospital. 20 patients returned to their prefracture status. All of them except one pleased with surgery, after the surgery, 5 patients had decubitis ulcer, 1 patient had prostetic dislocation, 3 patients had early superficial and early deep infection and 7 patients had myositis ossificans. Discussion: Treatment of intertrochanteric fractures of the femur in elderly patients is controversial and complications due to immobilization during convalescence are very frequent, including pneumonia, pressure sures, metabolic disturbances, circulatuary and respiratuary system disorders. Primary hemiarthroplasty seems tobe effective in preventing these cumplications and maintaining prefracture status by early weight bearing.
