Özet
Çalışmalar perinatal ruhsal bozuklukların düşük ve orta gelirli ülkelerde yüksek gelirli ülkelere göre daha yaygın olduğunu göstermiştir; fakat düşük ve orta gelirli ülkelerde bu alanda yapılan çalışmalar çok sınırlıdır. Ülkemizde de perinatal ruh sağlığı alanında katılımcıların gebelikten doğum sonrası döneme kadar takip edildiği çalışmalar yok denecek kadar azdır. Bu nedenle perinatal dönemdeki yaygın ruh sağlığı bozukluklarını inceleyen prospektif boylamsal bir çalışma planlanmıştır. Bu çalışmada İstanbul'da Zeynep Kamil Kadın ve Çocuk Hastalıkları Eğitim ve Araştırma Hastanesi, Ankara'da Etlik Zübeyde Hanım Kadın Hastalıkları Eğitim ve Araştırma Hastanesi ve İzmir'de Buca Kadın Doğum ve Çocuk Hastanesi çalışma merkezleri olarak seçilmiş olup, bu hastanelere başvuran ve çalışmaya katılmak isteyen gebeler çalışmanın katılımcılarını oluşturmaktadır. Katılımcılar gebelikte çalışmaya dahil olup, doğumlarını takiben 4-6 haftada ve 6.ayda takip edilmişlerdir. Bu çalışmaya 950 gebe katılmıştır. Her katılımcı araştırmanın üç aşamasında da anksiyete, depresyon, travma sonrası stres bozukluğu, doğum korkusu ve alınan sosyal desteği değerlendiren ölçekleri doldurmuşlardır. Araştırmanın sonucu ülkemizde gebelikte ve doğum sonrası dönemde ruhsal bozuklukların yaygın olduğunu göstermiştir. Özellikle doğum sonrası 4-6. haftada bu bozuklukların arttığı gözlemlenmiştir. 4-6. haftada doğumunu travmatik olarak algılayan kadınların oranı (37.2%) ve travmatik doğuma bağlı olarak travma sonrası stres bozukluğu yaşayan katılımcıların oranı dikkat çekicidir (11.9%). Doğuma bağlı travma sonrası stres bozukluğu doğum sonrası depresyon ve anksiyete, doğum korkusu ve katılımcının doğumu gerçekleştiren doğum ekibinden ne ölçüde memnun kaldığı ile yakından ilişkili bulunmuştur. Aynı zamanda sosyal desteği düşük olan katılımcılarda da doğum sonrası stress bozukluğu daha fazla gözlemlenmiştir. Bu çalışma ülkemizdeki, doğuma bağlı olarak gelişen travma sonrası stres bozukluğunu boylamsal olarak ele alan ve katılımcılar gebelikten doğum sonrası 6.aya kadar takip eden ilk çalışmadır. Bu araştırma depresyon ve anksiyete gibi daha çok bilinen ve yaygın olan ruhsal bozuklukların yanısıra, travmatik doğumların ve buna bağlı olarak gelişen doğum sonrası travma stres bozukluğunun da ülkemizde önemli birer problem olduğunu göstermiştir. Bu nedenle ülkemizde perinatal dönemde ruh sağlığı bozukluklarının önlenebilmesi ve yaygınlıkların azaltılabilmesi adına çok daha fazla çalışma yapılmasına ihtiyaç duyulmaktadır.

Research has shown that perinatal mental health directly affects women and their babies. In high-income countries, approximately 13% of women suffer from postpartum psychological disorders, which predominantly comprise depression, anxiety and post-traumatic stress disorder (PTSD), and this may be greater in low- and middle-income countries. Turkey is a rapidly developing country, but to date very few studies have examined mental health of women in Turkey in pregnancy or after birth. This thesis examines perinatal mental health in Turkey, with a particular focus on PTSD. This thesis has been carried out by prospective publication and consists of five articles. Article 1 aimed to establish the prevalence of PTSD in pregnancy and after birth, and the course of PTSD over this time. A systematic review and meta-analysis of the prevalence of PTSD during pregnancy and after birth was conducted and found that the mean prevalence of PTSD in pregnancy was 3.3% and 4% after birth in community samples. Women in high-risk groups were at more risk of PTSD with a mean prevalence of 18.95% in pregnancy and 18.5% after birth. Articles 2 to 5 report results from the main empirical work of the thesis: a population-based longitudinal study of Pregnancy and Childbirth in Turkey (PACT) that followed 829 women from pregnancy to 6 months postpartum. Article 2 focused on measurement and examined the psychometric properties of the Turkish version of the Posttraumatic Diagnostic Scale (PDS) to screen for PTSD after birth among postpartum women and to identify the factorial structure of PTSD after birth. The findings support the use of the PDS as an effective screening measure for PTSD after birth among postpartum women. PTSD after birth was best identified by a three-factor model: re-experiencing and avoidance, numbing and dysphoric-arousal, and dysphoric-arousal and anxious-arousal symptoms. Article 3 assessed the prevalence of depression, anxiety and PTSD, and their comorbidity among Turkish women during the perinatal period. Results suggested a relatively high prevalence of psychological disorders in pregnancy and after birth in Turkey. The prevalence of depression was 14.6% in pregnancy, 32.6% at 4-6 weeks and 18.5% at 6 months postpartum, respectively. The prevalence of PTSD was 5.8% in pregnancy, 11.9% at 4-6 weeks postpartum and 9.2% at 6 months postpartum. Anxiety was highest in pregnancy (29.6%) and decreased to 24.6% 4-6 weeks after birth and to 16.2% 6 months after birth. The course of depression and PTSD showed an inverse-U pattern, whereas anxiety followed a declining linear pattern from pregnancy to 6 months postpartum. Article 4 determined the pregnancy and postpartum factors associated with post-traumatic stress symptoms (PTSS) after birth. Results demonstrated that PTSS 6 months after birth were associated with the following factors: anxiety and PTSS in pregnancy; complications during birth; satisfaction with health professionals; fear of childbirth 4-6 weeks after birth; PTSS and depression 4-6 weeks after birth; poor social support 4-6 weeks and 6 months after birth; further traumatic events after birth; and the need for psychological help after birth. PTSS were highly comorbid with depression and anxiety at all-time points. Article 5 identified trajectories of birth-related PTSD and risk factors associated with each trajectory. Results revealed four trajectories of PTSD after birth: resilience, recovery, chronic-PTSD and delayed-PTSD. Less affective symptoms at 4-6 weeks postpartum consistently distinguished resilience from other PTSD trajectories. When affective symptoms at 4-6 weeks postpartum were excluded from models, less social support and higher fear of childbirth 4-6 weeks after birth predicted chronic-PTSD and recovery trajectories, compared to resilience; whereas the experience of further trauma and low levels of satisfaction with health professionals were predictive of chronic-PTSD and delayed-PTSD trajectories, compared to resilience. The PACT study is the first epidemiological study to examine perinatal psychological disorders among Turkish women. The study has found high rates of affective disorders in pregnancy and after birth in Turkey, including PTSD and identified several factors associated with birth-related PTSD symptomatology and its trajectories. Future studies would benefit from examining the impact of PTSD on Turkish women, their infants, and other family members. Further prospective studies on the intervention, prevention and treatment of PTSD after birth in Turkish women are also warranted.
