Ek doz analjezi gereksinimleri kaydedildi. 2, 4, 6, 12 ve 24. saatlerde bulantı kusma varlığı not edildi. Erken dönem komplikasyonlar arasında 7 ve 30. günlerde hastalardan anamnez alınarak herhangi bir şikayet varlığı(karın ağrısı, şişkinlik, ishal vb dispeptik yakınmalar), yara yeri enfeksiyonu, kanama, karın içi apse/koleksiyon açısından sorgulandı ve Clavien- Dindo sınıflamasına göre derecelendirildi. Ameliyat süreleri, hastanede kalış süreleri, ameliyat sonrası gaz-gaita deşarjı olunan zaman ve bulantı kusma varlığı not edildi.
Background: Low-impact laparoscopy is an innovative surgical technique developed to reduce the physiological stress caused by conventional laparoscopy. With lower insufflation pressure and the use of specially designed surgical instruments, this method aims to improve the postoperative recovery process and preserve surgical efficacy. Our study aimed to compare the effects of low-impact laparoscopic cholecystectomy (LILC) and conventional laparoscopic cholecystectomy (CLC) on postoperative pain, early complications, mobilization, hospital stay, surgery duration and constipation. Methods: The study included 104 patients, 52 of whom were randomized into LILC and CLC groups. In the LILC group, cholecystectomy was performed under low pressure (8 mmHg) and minimal incisions (3-5 mm), and in the CLC group, cholecystectomy was performed under standard pressure (12 mmHg) and with standard-sized incisions (5-10 mm) using traditional methods. Pain was recorded using a visual analog scale (VAS) at 2, 4, 6, 12 and 24 hours after surgery. Additional analgesia requirements were recorded. Nausea and vomiting were noted at 2, 4, 6, 12 and 24 hours. Among early complications, anamnesis was taken from the patients on the 7th and 30th days, and they were questioned for any complaints (dyspeptic complaints such as abdominal pain, bloating, diarrhea), wound infection, bleeding, and intra-abdominal abscess/collection, and they were graded according to the Clavien-Dindo classification. Duration of surgery, length of hospital stay, time of postoperative fecal or gas defecation, and presence of nausea and vomiting were noted. Results: VAS values were found to be significantly lower in the LILC group at 2, 4, 6, 12 and 24 hours (p<0.01). Additional analgesia requirement was found to be significantly lower in the LILC group at 2, 4, 6, 12 hours (p<0.01). Nausea and vomiting were not present at the 12th hour in the LILC group (p<0.05). The duration of surgery was significantly lower in the LILC group (p<0.01). The presence of complaints was significantly less in the LILC group at the 30th day controls (p<0.01). The presence of nausea and vomiting could not be evaluated since the rate was the same in both groups at the 2nd hour, and no significant difference was observed at the 4th, 6th, 24th hours (p>0.05). No significant difference was observed between the duration of hospital stay, mobilization time, time of postoperative fecal or gas defecation, and 7th day follow-up and the complication grade according to the Clavien-Dindo classification (p>0.05). Conclusions: Low-impact laparoscopic cholecystectomy was evaluated as a safe and effective method in reducing postoperative pain, reducing the need for additional analgesia, shortening the duration of surgery, and reducing dyspeptic complaints. It is a method that can be used safely by an experienced surgeon in appropriate patient selection.
