Özet
Orta Düzey Bilişsel Bozukluk (OBB), beyinde bozulmuş glikoz metabolizmasıyla karakterize edilen bir durumdur. Son yıllarda yapılan çalışmalarda ketojenik diyetin bağırsak mikrobiyota modülasyonu aracılığıyla bilişsel fonksiyon üzerine olumlu etkiler sağlayabileceği öne sürülmektedir. Bu çalışmanın amacı, OBB oluşturulan ratlarda farklı ketojenik diyet modellerinin bağırsak mikrobiyotası ve bilişsel fonksiyon ile ilişkisini incelemektir. Çalışmaya 8 haftalık erkek Wistar albino rat (n=28) dahil edilmiş ve dört gruba ayrılmıştır. Bilişsel olarak normal Kontrol grubu (KN) ve skopolamin ile bilişsel bozukluk modeli oluşturulan Skopolamin grubu (SKOP), standart kemirgen yemi ile; diğer iki grupta ise skopolamin ile bilişsel bozukluk oluşturulmuş olup, biri Akdeniz Uyumlu Ketojenik Diyet (AUKD) ve diğeri klasik ketojenik diyet (KD) ile beslenmiştir. Her grup için diyet müdahalesi 9 hafta sürmüştür. AUKD modeli %60 yağ (%76,29'u zeytinyağı), %30 protein, %10 karbonhidrat ve resveratrol (75,8 mg/100 mg yem); KD ise %84 yağ (%47,63'si tereyağ ve %47,63'ü domuz yağı) ve %16 protein içeriğine sahiptir. Diyet müdahalesi öncesi ve sonrasında mekânsal hafıza ve öğrenme davranışını değerlendirmek için Morris Su Labirenti (MWM) ve görsel hafızayı değerlendirmek için Yeni Nesne Tanıma Testi (NORT) uygulanmıştır. Müdahale sonunda toplanan dışkı örnekleri ile bağırsak mikrobiyota kompozisyonu 16S rRNA dizileme yöntemi ile analiz edilmiştir. Günlük vücut ağırlığı başına ortalama enerji alımının KD grubunda, KN ve SKOP gruplarına göre daha düşük olduğu saptanmıştır (p<0,05). Ayrıca, AUKD grubunda protein, omega-9 ve omega-3 yağ asidi alımlarının diğer gruplara kıyasla daha yüksek olduğu belirlenmiştir (p<0,05). MWM testinde, AUKD grubunda diğer gruplara göre platforma ulaşma süresinin daha kısa olduğu saptanmıştır (p<0,05). AUKD grubunun, Chao1 ve Gözlemlenen türler indekslerine göre α çeşitliliğinin KD grubuna kıyasla daha yüksek olduğu belirlenmiştir (p<0,05). Shannon indeksine göre ise tüm gruplar arasında α çeşitliliğin farklı olduğu saptanmıştır (p<0,05). Bray-Curtis, Jaccard ve ağırlıksız unifrac mesafe matrisine dayalı PCoA'e göre, AUKD ve KD grupları arasında bağırsak mikrobiyata kompozisyonunda anlamlı bir fark olduğu gösterilmiştir (p<0,05). Lactobacillus AUKD grubunun, Roseburia KD grubunun, [Ruminococcus]_torques_group, [Eubacterium]_coprostanoligenes_group ve Dorea ise SKOP grubunun karakteristik bakterileri olarak tanımlanmıştır. AUKD grubunda, diğer gruplara göre Bacteroides bolluğu daha düşük, Prevotellaceae_UCG-001, Lachnospiraceae_NK4A136-group ve Akkermansia bolluğu ise daha yüksek bulunmuştur (p<0,05). Sonuç olarak OBB'de, AUKD modelinin KD modeline kıyasla bağırsak mikrobiyotasını olumlu yönde destekleyerek bilişsel fonksiyon üzerinde yarar sağlayacağı ve bu nedenle özellikle Alzheimer hastalığının gelişimini önleme veya geciktirme potansiyeline sahip olabileceği düşünülmektedir.

Mild Cognitive Impairment (MCI) is a condition characterized by impaired glucose metabolism in the brain. Recent studies suggest that the ketogenic diet may provide positive effects on cognitive function through gut microbiota modulation. The aim of this study is to investigate the relationship between different ketogenic diet models, gut microbiota, and cognitive function in rats with MCI. Eight-week-old male Wistar albino rats (n=28) were included in the study and divided into four groups. Cognitively normal Control group (CN) and Scopolamine group (SCOP) with cognitive impairment model induced by scopolamine were fed with standard rodent feed; the other two groups had cognitive impairment induced by scopolamine, with one fed Mediterranean-Compatible Ketogenic Diet (MMKD) and the other classic ketogenic diet (KD). Dietary intervention lasted 9 weeks for each group. The MMKD model contains 60% fat (76.29% olive oil), 30% protein, 10 % carbohydrate, and resveratrol (75.8 mg/100 mg feed); KD contains 84% fat (47.63% butter and 47.63% lard) and 16% protein. Morris Water Maze (MWM) was used to evaluate spatial memory and learning behavior, and Novel Object Recognition Test (NORT) was used to evaluate visual memory before and after dietary intervention. Gut microbiota composition was analyzed using 16S rRNA sequencing method with fecal samples collected at the end of the intervention. The average daily energy intake per body weight in the KD group was found to be lower compared to the KN and SKOP groups (p<0,05). Additionally, protein, omega-9, and omega-3 fatty acid intakes were determined to be higher in the MMKD group compared to other groups (p<0,05). In the MWM test, the MMKD group showed shorter platform reaching time compared to other groups (p<0,05). According to Chao1 and Observed species indices, α diversity of the MMKD group was determined to be higher compared to the KD group (p<0,05). According to the Shannon index, α diversity was found to be different among all groups (p<0,05). Based on Bray-Curtis, Jaccard, and unweighted unifrac distance matrix PCoA, a significant difference in gut microbial composition was shown between MMKD and KD groups (p<0,05). Lactobacillus was identified as characteristic bacteria of the MMKD group, Roseburia of the KD group, and [Ruminococcus]_torques_group, [Eubacterium]_coprostanoligenes_group, and Dorea of the SCOP group. In the MMKD group, Bacteroides abundance was lower, while Prevotellaceae_UCG-001, Lachnospiraceae_NK4A136-group, and Akkermansia abundance was higher compared to other groups (p<0,05). In conclusion, in MCI, the MMKD model is thought to provide benefits on cognitive function by positively supporting gut microbiota compared to the KD model, and therefore may have the potential to prevent or delay the development of Alzheimer's disease.
