Objective: The aim of the study was to compare the effects of traditional and artificial intelligence (AI)-assisted yoga on quality of life and mental well-being in university students in a randomized controlled trial. Method: The study was a single-blind randomized controlled follow-up study with two interventions: traditional yoga and AI-assisted yoga practice, and a pretest-posttest control group (Clinical Trials Number: NCT06186635). Additionally, we created an experimental design by using machine learning and deep learning models to predict correct posture in the AI-assisted yoga group. The population of the study consisted of approximately 880 students studying at Çanakkale Onsekiz Mart University's Faculty of Health Sciences, Departments of Nursing (n = 460), Midwifery (n = 310), and Emergency Aid and Disaster Management (n = 110) in the 2023–2024 academic year. The study randomly assigned sixty-nine students who volunteered and met the inclusion criteria to three groups: traditional yoga (n = 23), AI-assisted yoga (n = 23), and a control group (n = 23). 4 students dropped out of the study. We completed the study with 65 students across three groups: traditional yoga (n = 21), AI-supported yoga (n = 21), and control (n = 23). According to the posthoc power analysis conducted for a sample size of 65 at the end of the study, it was found that the number of samples was sufficient with 99% power within a 95% confidence interval (effect size: 0.5).The study looked at traditional yoga, yoga with AI support, and a control group. It used the Introductory Information Form, the Multidimensional Quality of Life Scale (MQOLS), and the Warwick-Edinburgh Mental Well-Being Scale (WEMWBS) as pre-, follow-up-, and post-tests. Using key point coordinates from images, Mediapipe provided real-time correct posture feedback to the students during the AI-assisted yoga practice. Traditional and AI-assisted yoga practice was applied to two separate intervention groups for 12 weeks, twice a week, for 1 hour each session, averaging 2 hours per week. We performed data analysis using Python 3.0 and IBM SPSS 25.0 programs, utilizing descriptive statistics, the t-test for independent groups, Kruskal Wallis, the two-way ANOVA test for repeated measures, the Bonferroni test, and intention-to-treat analyses. Effect sizes were evaluated with Cohen's d and eta-square (η2) at 95% confidence intervals. Statistical significance was accepted as p<0.05. Results: The total MQOLS score of the traditional yoga group was significantly higher than the control group (p<0.05). In the AI-supported yoga group, there was a significant increase in the MQOLS scores compared to the control group (p<0.05). However, the traditional yoga group and the AI-supported yoga group showed no significant difference. Group-time interactions confirmed that the observed increases in quality of life were intervention-related. The WEMWBS total scores of the students in the traditional yoga and AI-supported yoga groups significantly increased compared to the control group (p<0.05). We observed the impact of time on mental well-being, yet the interaction between group and time was not statistically significant. Deep learning (CNN) and machine learning (SVM) algorithms showed the highest success in performing yoga postures correctly. CNN in particular stood out with high accuracy rates (over 90% accuracy). Conclusion: Traditional yoga offered significant improvements, especially in physical and mental health, while AI-assisted yoga provided long-term benefits through personalized guidance and accurate posture prediction. Both practices enhanced the quality of life and mental well-being, yet there was no significant difference in mental well-being between the groups. Algorithms (especially CNN) used in AI-assisted yoga practices achieved high success rates in correct posture prediction. The results demonstrate that yoga practices can enhance the physical and mental well-being of individuals, with technology serving as an effective tool in this process.
