Özet
Amaç: Bu çalışmada, hastanemizin genel ameliyathanesinde kulak burun boğaz (KBB) operasyonu geçiren yetişkin hastalarda, perioperatif istenmeyen hipotermi ve postoperatif derlenme ajitasyonu ilişkisinin incelenmesi amaçlanmıştır. Gereç ve Yöntem: Çalışma, 01.03.2024-01.09.2024 tarihlerinde Kocaeli Şehir Hastanesi genel ameliyathanesinde kulak burun boğaz operasyon odalarında prospektif olarak yapıldı. 18-65 yaş arası, ASA 1-2 risk grubunda olan ve KBB operasyonu için genel anestezi alacak hastalar çalışma için bilgilendirilerek onamları alındı. Hastaların demografik verileri, sigara kullanma durumu, eşlik eden hastalıklar, operasyon tipi, preoperatif vücut sıcaklık değeri (T0), perioperatif vücut iç sıcaklık değerleri (T1, T2), operasyon süresi, anestezi süresi, foley sonda işlemi, ASA risk skoru, preoperatif ve perioperatif hemodinamik ve solunumsal parametreler, postoperatif derlenme ajitasyon Richmond Ajitasyon Skalası Skorları ve Riker Sedasyon Ajitasyon Skalaları standart hasta takip formuna kaydedildi.Bulgular: Çalışmaya alınan toplam 123 hastanın yaş ortalaması 35,3±12,3 olarak saptandı. Hastaların %48,8'i (n=60) kadın iken %51,2'si (n=63) erkekti. Ameliyat öncesi değerlendirmede; 71 kişinin (%57,7) ASA skoru 2 olarak belirlendi. Toplam alınan 123 hastadan %59,3' ü (n=73) ameliyat bitiminde vücut iç sıcaklık değeri (T2) 36 °C altında olup hipotermik grup olarak sayıldı. Hipotermik ve normotermik gruplar arasında demografik veriler, ASA skoru, geçirilen operasyon tipi açısından anlamlı farklılık saptanmadı (p>0,05). Hastaların T2 sıcaklık ortalaması, T1 ve T0'dan anlamlı olarak düşük saptandı (p<0,05). Bu bulguya göre ameliyat bitimindeki vücut sıcaklığı, ameliyat girişindeki vücut sıcaklığından anlamlı olarak düşük saptandı. Ekstübasyonun 0. dakikasında 123 hastanın RASS'a göre %26,82'si (n=33), RSAS' a göre %21,95'i (n=27) ajite olarak uyandı. T2 vücut sıcaklığının 5.dk RASS (p=0,18) ve 5.dk Riker (p=0,44) skorları ile arasında pozitif yönlü korelasyon saptandı. Operasyon sonu sıcaklığı yüksek olan hastalar, ekstübasyondan 5 dk sonra daha ajite olarak uyandı. Hastalar hipotermi eşiği olan 36 °C'ye göre hipotermik ve normotermik olarak belirlendiği vii durumda Riker 0. dk skorları anlamlı farklılık göstermedi (p=0,381). T2-T1 sıcaklık farkının 0.dk RASS (p=0,01) , 5.dk RASS (p<0,001), 0.dk Riker (p=0,02) ve 5.dk Riker (p<0,001) skorları ile arasında pozitif yönlü korelasyon saptandı. Çalışmamızda perioperatif sıcaklık kaybı fazla olan hastalar, postoperatif daha ajite uyandı. Sonuç: Çalışmamızın sonucu, perioperatif dönemde hastalarda meydana gelen sıcaklık kaybının postoperatif derlenme ajitasyonu ile ilişkili olduğunu ortaya koymaktadır. Çalışmamızda elde edilen diğer sonuçlar, ameliyat sonundaki tek bir vücut sıcaklığından ziyade, ameliyat boyunca vücut sıcaklığındaki değişimin derlenme ajitasyonu üzerinde daha belirleyici bir faktör olduğunu ortaya koymuştur. Perioperatif süre boyunca 36 °C olan hipotermi eşiğine göre hastaları takip etmenin yetersiz olabileceği sonucuna vardık.

Aim: This study aimed to investigate the relationship between perioperative unintended hypothermia and postoperative recovery agitation in adult patients undergoing ear, nose and throat (ENT) surgery in the general operating room of our hospital. Material and methods: The study was conducted prospectively in the ear, nose and throat operation rooms of Kocaeli City Hospital general operating room between 01.03.2024 and 01.09.2024. Patients aged between 18-65, in the ASA 1-2 risk group and who will receive general anesthesia for ENT surgery were informed about the study and their consent was obtained, and a total of 123 patients were included in the study. Demographic data of the patients, smoking status, concomitant diseases, operation type, preoperative body temperature value (T0), perioperative core body temperature values (T1, T2), operation time, anesthesia time, foley catheter procedure, ASA risk score, preoperative and perioperative hemodynamic and respiratory parameters, postoperative recovery agitation Richmond Agitation Scale Scores and Riker Sedation Agitation Scales were recorded on the standard patient follow-up form. Results: The mean age of the patients was determined as 35.3±12.3. While 48.8% (n=60) of the patients included in the study were female, 51.2% (n=63) were male. 16.3% (n=20) of the patients had a comorbidity. In the pre-procedure evaluation; ASA score of 71 people (57.7%) was determined as 2. Of the 123 patients included in the total, 73 people had an internal body temperature (T2) below 36 °C at the end of the surgery and were considered as the hypothermic group. Hypothermia incidence was calculated as 59.3%. No significant difference was found between the hypothermic and normothermic groups in terms of age, height, body weight, BMI, gender, presence of comorbidity, medication use, ASA score, and previous surgery (p>0.05). The mean T2 of the patients included in the study was found to be significantly lower than T1 and T0 (p<0.05). The patients body temperature at the end of the surgery was found to be significantly lower than the body temperature at the beginning of the surgery. According to the results of our study, 26.82% (n=33) of 123 patients woke up agitated according to RASS and 21.95% (n=27) according to RSAS at the 0th minute of extubation. A positive correlation was found between the T2-T1 temperature difference and the 0th min RASS (p=0.01), 5th min RASS (p<0.001), 0th min Riker (p=0.02) and 5th min Riker (p<0.001) scores. In our study, patients with greater perioperative temperature loss woke up more agitated postoperatively. Conclusion: The results of our study show that the temperature loss in patients during the perioperative period is associated with postoperative recovery agitation. Other results obtained in our study revealed that the change in body temperature during surgery is a more decisive factor on recovery agitation rather than a single body temperature at the end of surgery. According to our study, it was concluded that monitoring patients according to the hypothermia threshold of 36 °C during the perioperative period may be insufficient.
