Özet
Amaç: Çalışmamızda, kliniğimizde 1985-2023 yılları arasında pemfigus vulgaris tanısıyla takip ettiğimiz hastalarda kortikosteroidle birlikte adjuvan azatioprin veya mikofenolat mofetil kullanımının klinik ve serolojik sonuçları, yan etki profiline etkileri, karşılaştırmalı olarak ortaya konarak literatüre katkıda bulunmak amaçlanmıştır. Bununla birlikte nadir bir hastalık olan pemfigus vulgarisin klinik, serolojik özellikleri ve hastalık seyri geniş bir hasta serisinde değerlendirilmiştir. Gereç ve Yöntem: İstanbul Tıp Fakültesi Deri ve Zührevi Hastalıklar Polikliniğinde 1985-2023 yılları arasında klinik bulgulara ek olarak histopatolojik ve immunolojik yöntemlerden en az ikisiyle pemfigus vulgaris tanısı almış hastaların dosyaları geriye dönük olarak incelendi. Hastaların tanı anındaki demografik özellikleri, ek hastalıkları, laboratuvar bulguları kaydedildi. Adjuvan immunsupresif kullanımının hastalık seyrine etkisinin değerlendirilmesi için hastalar, başlangıç tedavisine göre yalnız kortikosteroid (KS), kortikosteroid ve azatioprin (AZA), kortikosteroid ve mikofenolat mofetil (MMF) alan hastalar olmak üzere üç gruba ayrıldı. Bu gruplarda minimal tedavili ve tedavisiz tam remisyona ulaşma oranları, ulaşan hastaların ulaşma süreleri ve kümülatif KS dozları hesaplanarak kaydedildi. Ayrıca gruplarda klinik takip boyunca tedavi değişikliğine gidilip gidilmediği, hem adjuvan tedavilere hem de kortikosteroide bağlı görülen yan etkiler, aktivasyon sayıları, serolojik takipleri ayrıntılı şekilde kaydedildi ve karşılaştırıldı. Değerlendirmelerde IBM SPSS version 20 programı kullanıldı. Çalışmamızda istatistiksel anlamlılık sınırı p<0.05 olarak kabul edildi. Bulgular: Çalışmamıza dahil edilen 329 hastada kadın/erkek oranı 1.43 olarak belirlendi. Olgular ortalama 108.51 ay takip edildi. Takipleri boyunca hastalarımızın %78.41'i en az bir kez adjuvan tedavi alırken, %21.58'i tedavisini yalnız sistemik kortikosteroid tedavisiyle tamamladı. Çalışma grubumuza dahil edilen 329 hastanın 220'si takipleri sürecince AZA tedavisi, 97'si MMF tedavisi aldı. AZA tedavisi alan hastalarımızda %12.27 oranında görülen 2 kemik iliği supresyonu, MMF alan hastalarımızın %12.37'sinde görülen hiperlipidemi en sık karşılaştığımız yan etkiler olarak belirlendi. Adjuvan immunsupresif kullanımının hastalık seyrine etkisi; başlangıç tedavisi KS, KS + AZA ve KS + MMF olan ve sırasıyla 96, 129 ve 39 hastadan oluşan toplam 264 kişilik alt grupta değerlendirildi. Yalnız KS alan grubun ortalama takip süresi ortalama 104.2, KS+AZA alan grubun 105.45, KS+MMF alan grubun ise 79.73 aydı ve takip süreleri arasında istatistiksel anlamlı fark saptanmadı (p>0.05). Bu tedavi grupları arasında KS'e bağlı en az bir yan etki görülme oranı, basit yan etki ve ciddi yan etki oranları benzerdi (p>0.05). KS+AZA kullanan grup ile, KS+MMF kullanan gruplarda adjuvan tedaviyle ilişkili yan etkilerin görülme oranları, tedavi değişikliği, hastanede yatış oranları benzerdi (p>0.05). Buna karşın AZA+KS alan hastalarda hastanede yatış sayısı, yalnız steroid alan hastalardan (p=0.01), hastanede yatış süresi ise yalnız KS (p<0.001) ve KS+MMF (p=0.017) alan hastalardan daha uzundu. Tedavi kollarında minimal tedavili tam remisyon oranları sırasıyla; %79.2, %76 ve %79.5, süresi 9.31, 11.43, 12.71 aydı ve fark istatistiksel olarak anlamlı bulunmadı (p>0.05). Kümülatif kortikosteroid dozu, yalnız KS alan grupta hem MMF (p=0.019), hem de AZA (p=0.006) alan gruptan daha düşüktü. Başlangıç tedavisinin devam ettiği dönem içerisindeki aktivasyon sayıları yalnız KS alanlarda ortalama 1.27±2.06, KS+AZA alan grupta 1.17±1.81 ve KS+MMF alan grupta 0.93±1.36 olarak saptandı. En az aktivasyon, adjuvan tedavi olarak MMF alan grupta görülse de gruplar arasında istatistiksel anlamlı fark bulunmadı (p>0.05). Gruplar arasında remisyondaki anti Dsg 1, Dsg 3 pozitiflik oranları ve anti Dsg 1 ve anti Dsg 3 antikor düzeyleri arasında istatistiksel olarak anlamlılık saptanmadı (p>0.05). Tartışma: Çalışmamıza dahil edilen PV hastalarına ilişkin demografik veriler ve klinik ve serolojik özellikler geçmiş çalışmalarla uyumludur. Çalışmamızda adjuvan tedavi alan hasta grupları arasında remisyon süresi, remisyona kadar kullanılan kümülatif kortikosteroid dozu, adjuvan ilaca bağlı yan etki görülme oranları, ilacın alındığı sürede gerçekleşen aktivasyon sayıları, farklı bir adjuvan tedaviye geçme gereksinimi açısından istatistiksel anlamlı fark saptanmamıştır. Literatürde MMF ve AZA'nın etkinliğini karşılaştıran ve benzer sonuçları olan çalışmalar mevcuttur. Buna karşın mikofenolat mofetil alan grupta basit enfeksiyonların daha düşük oranda görülmesi, azatioprin alan grupta hastane yatış süresinin daha uzun olması, önemli farklar olarak dikkati çekmektedir.

Objective: The aim of our study is to contribute to the literature by comparatively evaluating the clinical and serological outcomes, as well as the side effect profiles, of using adjuvant azathioprine or mycophenolate mofetil alongside corticosteroids in patients diagnosed with pemphigus vulgaris who were followed in our clinic between 1985 and 2023. Additionally, by assessing the clinical and serological characteristics and disease course of pemphigus vulgaris, in a large patient series, we aim to deepen the understanding of this condition. Materials and methods: In the Dermatology and Venereology Clinic of Istanbul Medical Faculty, the files of patients diagnosed with pemphigus vulgaris between 1985 and 2023 were retrospectively reviewed. In addition to clinical findings the diagnosis was confirmed using at least two of the histopathological and immunological methods. The demographic characteristics, comorbidities, and laboratory findings of the patients at the time of diagnosis were recorded. To evaluate the effect of adjuvant immunosuppressive therapy on the course of the disease, patients were divided into three groups based on their initial treatment: those receiving corticosteroids (CS) alone, corticosteroids with azathioprine (AZA), and corticosteroids with mycophenolate mofetil (MMF). The rates of achieving complete remission on minimal therapy and off therapy , the time to achieve remission, and the cumulative CS doses were calculated and recorded for these groups. Additionally, it was detailed whether treatment changes were made during clinical follow-up within the groups and the side effects observed due to both adjuvant therapies and corticosteroids, the number of activations, and serological follow-up results were meticulously recorded and compared. Evaluations were performed using IBM SPSS version 20. In our study, a statistical significance threshold of p<0.05 was accepted. Results: In our study, the female/male ratio among the 329 patients included was determined to be 1.43. The cases were followed for an average of 108.51 months. During their follow-up, 78.41% of our patients received adjuvant therapy at least once, while 21.58% completed their treatment with systemic corticosteroid therapy alone. Of the 329 patients 4 included in our study group, 220 received azathioprine (AZA) therapy and 97 received mycophenolate mofetil (MMF) therapy during their follow-up. The most common side effect observed in patients receiving AZA therapy was bone marrow suppression at a rate of 12.27%, whereas hyperlipidemia was the most common side effect observed in patients receiving MMF therapy at a rate of 12.37%. The effect of adjuvant immunosuppressive use on disease progression was evaluated in a subgroup of 264 patients, consisting of 96, 129, and 39 patients initially treated with corticosteroids CS alone, CS + AZA and CS + MMF, respectively. The mean follow-up duration was 104.2 months for the CS group, 105.45 months for the CS + AZA group, and 79.73 months for the CS + MMF group. No statistically significant difference was observed in follow-up durations between the groups (p > 0.05).Among the corticosteroid CS, CS+AZA, and CS+MMF treatment groups , the rates of experiencing at least one side effect related to CS, as well as the rates of minor and serious side effects, were similar (p>0.05). The rates of side effects associated with adjuvant therapy, treatment changes, and hospitalizations were similar between the groups using CS+AZA and those using CS+MMF (p>0.05). However, the number of hospitalizations in patients receiving CS+AZA was higher than in those receiving CS alone (p=0.010, p<0.05), and the length of hospital stay was longer than in patients receiving CS alone (p<0.001) and CS+MMF (p=0.017). The rates of complete remission on treatment in the treatment arms were 79.2%, 76%, and 79.5%, with durations of 9.31, 11.43, and 12.71 months, respectively, and the differences were not statistically significant (p>0.05). The cumulative corticosteroid dose was lower in the group receiving CS alone than in the groups receiving MMF (p=0.019, p<0.05) and AZA (p=0.006, p<0.01). The number of activations during the period they continued their initial treatments was found to be 1.27±2.06 in the group receiving CS alone, 1.17±1.81 in the group receiving CS and AZA, and 0.93±1.36 in the group receiving CS and MMF. Although the least activation was observed in the group receiving MMF as adjuvant therapy, the difference between the groups was not statistically significant (p>0.05). There were no statistically significant differences between the groups in terms of Dsg 1, Dsg 3 positivity, and anti-Dsg 1 IgG and anti-Dsg 3 IgG antibody levels during remission (p=0.448, p=0.215, p=0.545, p=0.072, respectively). Discussion: The demographic data and clinical and serological characteristics of the patients included in our study are consistent with previous studies. In our study, no statistically significant differences were found between the patient groups receiving AZA and MMF as adjuvant therapy regarding duration and cumulative corticosteroid dose used until remission, the incidence of side effects related to adjuvant drugs, the number of activations during the 5 period the drug was taken, and the need to switch to a different adjuvant therapy. There are studies in the literature comparing the efficacy of MMF and AZA with similar results. However, notable differences were observed, such as the lower incidence of mild infections in the group receiving mycophenolate mofetil and the longer hospital stays in the group receiving azathioprine.
