Özet
Mikobakteri kaynaklı akciğer enfeksiyonları insan sağlığı açısından önemli bir morbidite ve mortalite sebebidir. Son epidemiyolojik çalışmalar, Mycobacterium tuberculosis'e (MTB) ek olarak, tüberküloz dışı mikobakteri (TDM) akciğer hastalığı insidansının endişe verici bir oranda artmakta olduğunu göstermektedir. Yüksek antimikrobiyal direnç oranları nedeniyle TDM enfeksiyonlarının tedavilerinde seçenekler sınırlı kalmakta, uzun tedavi süreleri, toksisite riski ve cesaret kırıcı sonuçlar, enfeksiyon yönetimini daha da zorlaştırmaktadır. TDM'ler arasında akciğer patojeni olarak en sık bildirilen türler Mycobacterium avium kompleks (MAC) ve Mycobacterium abscessus'tur. Birçok antimikrobiyale intrinsik direnç gösteren, tedavi başarısı ve remisyon oranları en düşük tür olan M. abscessus, sıklıkla ''tedavisi olmayan kabus'' olarak anılmaktadır. Bu nedenlerle, TDM'ler için daha etkili ve güvenli tedavilerin geliştirilmesine ihtiyaç duyulmaktadır. TDM antimikrobiyal direncinde, eflüks pompalarının (EP) rolüne ilişkin kanıtlar artmaktadır. İlaç direncini azaltmak için antimikrobiyallerin hücre içi konsantrasyonunu artırabilen eflüks pompa inhibitörlerinin (EPİ) kullanımı ümit verici bir yaklaşımdır. Bu çalışmada, M. abscessus kompleks (MABC) klinik izolatlarında bedakuilin (BDQ) ve klaritromisinin (CLA), birer EPİ olan verapamil (VP) ve berberin (BER) ile kombinasyonlarında etkinliklerinin değerlendirilmesi amaçlanmıştır. Solunum yolu örneklerinden elde edilen 19 klinik izolat ve bir referans suşu canlandırılarak öncelikle BDQ ve CLA için minimum inhibitör konsantrasyon (MİK) değerleri belirlenmiş, sonrasında her bir izolat için bir EPİ ve bir antimikrobiyal kombinasyonun etkileşimleri dama tahtası yöntemi ile incelenmiştir. Değerlendirmede fraksiyon inhibitör konsantrasyon indeksi (FİKİ) hesaplanarak sinerjik, aditif, indiferan (etkisiz) ve antagonistik etkiler belirlenmiştir. Ayrıca GenoType NTM-DR (Hain Lifescience, Almanya) ile MABC alt tür tayini ve genotipik direnç tespiti yapılmıştır. Çalışmaya dahil edilen 19 MABC izolatlarının %84,2'si (16/19) M. abscessus subsp. abscessus, %10,5'i (2/19) M. abscessus subsp. massiliense, %5,26'sı (1/19) M. abscessus subsp. bolletii olarak tanımlandı. İzolatların hiçbirinde rrl veya rrs gen mutasyonu tespit edilmezken, işlevsel erm41 geni 14 izolatta (%73,6) saptandı. Klinik izolatlarda sinerjistik etkileşim oranları; BDQ/VP kombinasyonunda %84,2 (16/19), CLA/VP'de %57,9 (11/19), BDQ/BER'de %5,26 (1/19), CLA/BER'de %31,5 (6/19) olarak bulundu. Hiçbir kombinasyonda antagonist etkileşim görülmedi. BDQ/VP birlikteliğinde sinerjizm oranı BDQ/BER'e göre anlamlı derecede daha fazlaydı ve FİKİ değerleri açısından da anlamlı derecede fark tespit edildi (p<0,0005). VP/BDQ kombinasyon etkinliği, VP/CLA'dan üstün bulundu ve VP/BDQ birlikteliğinde FİKİ değerleri anlamlı derecede daha düşüktü (p<0,0005). CLA/BER kombinasyonunda sinerjistik etkileşim görülen altı suşun tamamında CLA/VP için de sinerjizm tespit edildi (p=0,018). VP ve BER'in antimikrobiyallerle kombinasyonlarında FİKİ medyan değerleri sırasıyla 0,370 ve 0,745 bulundu. VP için FİKİ değerleri anlamlı şekilde daha düşüktü (p<0,0005). VP birlikteliğinde antimikrobiyallerin kat değişimi medyan değeri 4 iken, BER için bu değer 2 olarak bulundu (p<0,0005). Çalışmamızda en etkili EPİ/antimikrobiyal kombinasyonunun VP/BDQ olduğu tespit edilmiştir. VP, tüm kombinasyonlarda BER'den etkili bulunmuştur. Bu çalışma, TDM'lerde EPİ/antimikrobiyal kombinsyonlarının sinerjistik etkilerinin incelendiği sınırlı çalışmalardan biri olup, BDQ/BER etkileşiminin incelendiği ilk çalışmadır. Çalışmamız, EPİ'lerin TDM tedavisinde sinerjistik kombinasyonlarını ortaya koyarak, bu alanda yapılacak ileri çalışmaların yolunu açacaktır.

Mycobacteria-associated lung infections are a major cause of morbidity and mortality in human health. Recent epidemiologic studies show that in addition to Mycobacterium tuberculosis (MTB), the incidence of nontuberculous mycobacteria (NTM) lung disease is increasing at an alarming rate. Due to high antimicrobial resistance rates, options for the treatment of NTM infections remain limited and long treatment durations, risk of toxicity and discouraging outcomes make infection management even more challenging. Among NTMs, Mycobacterium avium complex (MAC) and Mycobacterium abscessus are the most frequently reported lung pathogens. M. abscessus, which shows intrinsic resistance to many antimicrobials and has the lowest treatment success and remission rates, is often referred to as the“incurable nightmare”. For these reasons, there is a need to develop more effective and safe treatments for NTMs. There is increasing evidence for the role of efflux pumps (EPs) in NTM antimicrobial resistance. The use of efflux pump inhibitors (EPIs), which can increase the intracellular concentration of antimicrobials to reduce drug resistance, is a promising approach. This study aimed to evaluate the efficacy of bedaquiline (BDQ) and clarithromycin (CLA) in combination with verapamil (VP) and berberine (BER), which are EPIs, in clinical isolates of the Mycobacterium abscessus complex (MABC). Nineteen clinical isolates obtained from respiratory samples and one reference strain were revived, and the minimum inhibitory concentration (MIC) values for BDQ and CLA were determined. Subsequently, the interactions of one EPI and one antimicrobial combination for each isolate were analyzed using the checkerboard method. The fractional inhibitory concentration index (FICI) was calculated to determine synergistic, additive, indifferent (ineffective), and antagonistic effects. Additionally, the GenoType NTM-DR (Hain Lifescience, Germany) assay was used to identify MABC subspecies and detect resistance genotypes. Of the 19 MABC isolates included in the study, 84.2% (16/19) were identified as M. abscessus subsp. abscessus, 10.5% (2/19) as M. abscessus subsp. massiliense, and 5.26% (1/19) as M. abscessus subsp. bolletii. No mutations in the rrl or rrs genes were detected in any of the isolates, while the functional erm41 gene was exhibited in 14 isolates (73.6%). Synergistic interaction rates in clinical isolates were 84.2% (16/19) for BDQ/VP combination, 57.9% (11/19) for CLA/VP, 5.26% (1/19) for BDQ/BER and 31.5% (6/19) for CLA/BER. No antagonistic interactions were observed in any of the combinations. The synergy rate in the BDQ/VP combination was significantly higher than that of BDQ/BER, and a significant difference was also observed in terms of FICI values (p<0.0005). The efficacy of the VP/BDQ combination was found to be superior to that of VP/CLA, and FICI values in the VP/BDQ combination were significantly lower (p<0.0005). Synergistic interaction observed in all six strains with the CLA/BER combination was also detected with CLA/VP (p=0.018). The median FICI values for combinations of antimicrobials with VP and BER were found to be 0.370 and 0.745, respectively. FICI values were significantly lower for VP (p<0.0005). The median fold change in antimicrobials when combined with VP was 4, whereas it was 2 for BER (p<0.0005). In our study, the most effective EPI/antimicrobial combination was identified as VP/BDQ. VP was found to be more effective than BER in all combinations. This study is one of the limited studies examining the synergistic effects of EPI/antimicrobial combinations in NTMs and the first to investigate the BDQ/BER interaction. Our study highlights the synergistic potential of EPI combinations in the treatment of NTMs, paving the way for future research in this field.
