Özet
Dental implantlar, günümüzde diş eksikliklerini gidermek için uygulanan etkili tedavi yöntemlerindendir. Osseointegrasyonun sağlanmasıyla implantların klinik başarı oranı artmıştır. Ancak, estetik beklentilerin yükselmesi ve doğal diş görünümüne yakın sonuçlar elde edilmesi, implant çevresindeki yumuşak dokuların estetik açıdan değerlendirilmesini zorunlu kılmaktadır. Bu bağlamda, anterior bölgedeki tek diş implantlarının estetik başarısını ölçmek için geliştirilen Kompleks Estetik İndeksi (KEİ) önemli bir araç olarak kullanılmaktadır. KEİ, sert dokunun yumuşak doku üzerindeki etkilerini de göz önünde bulundurur. Çalışmamızın amacı, anterior bölgelerde yerleştirilen dental implantların çevresindeki yumuşak ve sert dokuları, klinik, estetik ve radyografik açıdan değerlendirerek, implant destekli restorasyonların estetik hedeflere uygunluğunu incelemektir. KEİ kullanılarak estetik başarı kriterleri objektif biçimde tanımlanmış ve klinik uygulamalar için rehber olarak sunulmuştur. Yumuşak doku konturu, vertikal konum, papil yüksekliği gibi parametreler göz önünde bulundurularak tedavi öncesi ve sonrası komplikasyonların önlenmesine yönelik stratejiler geliştirilmiştir. Çalışmaya, Gazi Üniversitesi Diş Hekimliği Fakültesi'ne başvuran ve kriterler uyan hastalar dahil edildi. Gönüllülerin tüm ağız ortalama plak indeksi (Pİ) ve gingival indeksinin (Gİ) implant çevresi Pİ ve Gİ ile ilişki anlamlı derecede tüm ağız ortalamasında daha yüksek görüldü (p<0,05). Gönüllülerin tüm ağız ortalama cep derinliğinin (CD) implant çevresi CD arasındaki ilişki anlamlı derece implantların ortalamasında daha yüksek görüldü (p<0,05). Mezial ve distal kontakt noktası ile kret tepesi arası uzaklık ile papil dolumu arasında anlamlı ilişkiler bulundu (p<0,05). Yumuşak doku vertikal konumu ile implant omzu ile Zenith noktası arasında istatistiksel olarak anlamlı bir ilişki tespit edildi (p<0,05). Bulgularımız, estetik uyumu arttırarak komplikasyon riskini azaltan klinik stratejilerin geliştirilmesine olanak sağlamaktadır.

Dental implants are among the most effective treatment modalities for tooth loss. The clinical success rate of implants has significantly increased with the achievements in achieving osseointegration. However, the increasing aesthetic expectations and obtaining results close to the appearance of natural teeth necessitate the aesthetic evaluation of the soft tissues around the implant. In this context, the Complex Aesthetic Index (CAI), developed to measure the aesthetic success of single-tooth implants in the anterior region, has become an essential tool. The CAI also considers the impact of hard tissue on soft tissue. The aim of our study was to evaluate the soft and hard tissues around dental implants placed in anterior anterior regions from clinical, aesthetic and radiographic perspectives and to examine the compliance of implant-supported restorations with aesthetic objectives. Aesthetic success criteria were objectively defined using the CAI and presented as a clinical practice guide. Strategies were developed to prevent pre- and post-treatment complications by considering parameters such as soft tissue contour, vertical positioning, and papillary height. Patients who applied to Gazi University Faculty of Dentistry and met the criteria were included in the study. The relationship between the whole mouth mean plaque index (PI) and gingival index (GI) of the volunteers and the peri-implant PI and GI was significantly higher in the whole mouth (p<0.05). The relationship between the whole mouth mean pocket depth (PD) of the volunteers and peri-implant PD was significantly higher in the mean of the implants (p<0.05). Significant relationships were found between the distance from the mesial and distal contact points to the crest and papilla filling (p<0.05). A statistically significant relationship was found between the vertical position of the soft tissue and the distance from the implant shoulder to the Zenith point (p<0.05). Our findings allow the development of clinical strategies that reduce the risk of complications by increasing aesthetic harmony.
