Özet

Obsesif kompulsif bozukluk (OKB), bireylerin günlük yaşamını, işini ve sosyal etkileşimlerini etkileyen, yoğun sıkıntıya yol açan obsesyonlar ve/veya kompulsiyonlarla tanımlanan bir psikiyatrik bozukluktur. Epigenetik mekanizmalar, genetik ve çevresel faktörler arasındaki bağlantıyı sağlayarak bu tür psikiyatrik bozuklukların gelişimine aracı olur. Glutamat sinyalizasyonu OKB'nin gelişiminde önemli bir rol oynar, serotonin de bu bozuklukla ilişkili bir diğer nörotransmitterdir. Bu bağlamda, glutamaterjik ve serotonerjik sistemde rol oynayan genler, OKB'nin epigenetik temellerinin incelenmesi için uygun adaylar olarak görülmektedir. Bununla birlikte, OKB'li bireylerde metabolik sendromun görülme sıklığının genel popülasyona göre daha yüksek olduğu bildirilmiştir. Leptin, bu bağlamda dikkat çeken bir metabolik hormondur. Leptin ve leptin reseptör genlerinin incelenmesi, OKB'li bireylerin olası metabolik bozukluklar ve bu bozukluklarla ilişkili komplikasyonlara yatkınlıklarını değerlendirmek açısından önem taşımaktadır. Bu çalışmada, glutamaterjik (SLC1A1, GRID1) ve serotonerjik (SLC6A4, HTR2A) sistem genlerinin, ayrıca leptin (LEP) ve leptin reseptör (LEPR) genlerinin genetik-epigenetik düzeyde incelenmesi, ek olarak kandaki glutamat, serotonin ve leptin seviyelerinin ölçülerek hasta ve kontrol grupları arasında karşılaştırılması amaçlanmıştır. Araştırmada, DNA metilasyon düzeylerinin MSP ve MS-HRM yöntemi ile belirlenmesi, kandaki glutamat, serotonin ve leptin seviyelerinin ELISA yöntemi ile tespiti ve ilgili gen varyantlarının PCR-RFLP yöntemi ile tayin edilmesi gerçekleştirilmiştir. Elde edilen veriler ışığında, OKB kan glutamat ve serotonin düzeyleri kontrole kıyasla anlamlı seviyede düşük bulunurken kan leptin seviyesi OKB'lilerde yüksek bulunur (sırasıyla; p<0,001, p<0,001, p=0,002). Leptin geni rs7799039 ile Leptin reseptör geni rs1137101 polimorfizmleri ve OKB arasında önemli bir ilişki saptanmamıştır (p> 0,05). LEP geninin promoter bölgesindeki -51 nt ve -31 nt CpG bölgelerinin metilasyon durumları da analiz edilmiş ve her iki bölgenin de tüm örneklerde metile olduğu saptanmıştır. OKB ve kontrol gruplarında SLC1A1 geni metilasyon yüzdeleri açısında da anlamlı bir farklılık saptanmamıştır (p=0,371). Ayrıca elde edilen genetik ve epigenetik verilerin klinik parametreler açısından değerlendirilmesi yapılmış ancak anlamlı bir ilişki saptanmamıştır. Sonuç olarak, OKB'de glutamat ve serotonin düzeylerinin azalırken leptin seviyesinin arttığı belirlenmiş, ancak leptin ve leptin reseptör gen polimorfizmleri ile LEP ve SLC1A1 genlerinin metilasyon durumlarının OKB ile anlamlı bir ilişkisi olmadığı gözlemlenmiştir.

Obsessive-compulsive disorder (OCD) is a psychiatric disorder characterized by obsessions and/or compulsions that cause significant distress, affecting individuals' daily lives, work, and social interactions. Epigenetic mechanisms act as a bridge between genetic and environmental factors, contributing to the development of such psychiatric disorders. Glutamate signaling plays a crucial role in the pathogenesis of OCD, while serotonin is another neurotransmitter associated with this disorder. In this context, genes involved in the glutamatergic and serotonergic systems are considered suitable candidates for investigating the epigenetic basis of OCD. Moreover, studies have reported a higher prevalence of metabolic syndrome in individuals with OCD compared to the general population. Leptin, a metabolic hormone, has gained attention in this regard. Examining leptin and leptin receptor genes is essential for assessing the susceptibility of individuals with OCD to potential metabolic disorders and their associated complications. This study aimed to investigate the genetic and epigenetic aspects of glutamatergic (SLC1A1, GRID1) and serotonergic (SLC6A4, HTR2A) system genes, as well as leptin (LEP) and leptin receptor (LEPR) genes. Additionally, it aimed to measure blood levels of glutamate, serotonin, and leptin and compare them between OCD patients and control groups. In the study, DNA methylation levels were determined using MSP and MS-HRM methods, while blood glutamate, serotonin, and leptin levels were measured using the ELISA method. Genotypic variations of the relevant genes were identified via the PCR-RFLP method. According to the findings, blood glutamate and serotonin levels in the OCD group were significantly lower compared to the control group, whereas blood leptin levels were significantly higher (p<0.001, p<0.001, and p=0.002, respectively). No significant association was found between leptin gene rs7799039 and leptin receptor gene rs1137101 polymorphisms and OCD (p>0.05). Additionally, the methylation status of the -51 nt and -31 nt CpG regions in the promoter region of the LEP gene was analyzed, revealing that both regions were methylated in all samples. No significant difference was observed in the methylation percentages of the SLC1A1 gene between the OCD and control groups (p=0.371). Furthermore, genetic and epigenetic data were evaluated in relation to clinical parameters, but no significant associations were detected. In conclusion, this study determined that glutamate and serotonin levels decreased while leptin levels increased in OCD. However, no significant association was observed between OCD and the methylation status of the LEP and SLC1A1 genes or the polymorphisms of the leptin and leptin receptor genes.
