Özet
AMAÇ VE HİPOTEZ: Günümüzdeki en önemli sağlık sorunlarından biri sepsis olup sepsisin neden olduğu en sık görülen sorunlardan biri de böbrek hasarına bağlı böbrek yetmezliğidir. Epifiz bezinden salgılanan ve nörohormon olan Melatonin reaktif oksijen türlerini (ROS) bir antioksidan olatak nötralize edenken, C Vitamini ise suda çözünebilen bir Vitamin olup güçlü bir antioksidan olarak ROS'u nötralize etme kapasitesine sahip biyolojik moleküllerdir. Melatonin ve C Vitaminin birlikte uygulanması ve bu etkilerinden dolayı sepsisin tedavisinde ve organ hasarlarını önlemede önemli bir teropatik ajan olabileceğine dair literatürde önemli bulgular bulunmaktadır. Ancak böbrek özelinde, litaretürde melatonin ve C Vitaminin sepsise bağlı böbrek hasarı üzerine etkisini gösteren çalışmalar oldukça sınırlıdır. Mevcut çalışmamızda, LPS ile oluşturulmuş sepsis sıçan modelinde Melatonin ve C Vitamininin birlite uygulamasının sepsise bağlı böbrek hasarı üzerindeki koruyucu etkisi araştırıldı. YÖNTEM: Çalışmamız Sprague dawley cinsi erkek sıçanlar üzerinde yapıldı. Sıçanlar rastgele 4 eşit gruba ayrıldı. Bu gruplar kontrol grubu, Melatonin-C Vitamini grubu, LPS grubu, Melatonin-C Vitamini-LPS uygulanan grubu olarak deney sistematiği oluşturuldu. Gruplarımız deneyin sonunda histolojik analiz (hematoksilen- eozin boyaması, immunohistokimya boyaması) ve biyokimyasal analiz (ELISA analizi ve BUN , kreatinin seviyeleri analizi) yapıldı. .P<0.05 istatistiksel olarak anlamlı kabul edilerek bu testler gerçekleştirildi. BULGULAR: Histolojik analizde hematoksilen-eozin boyamasında LPS-Melatonin- C Vitamini kombinasyonu uygulanan gruba ait böbrek dokuları ile LPS uygulanan grup kıyaslandığında tübulointerstisyal alanlarda inflamatuar hücrelerin miktarının daha az olduğu (p<0,05), tübüllerde vakuolizasyonun (p<0,05) ve dejenere glomerül sayısının (p<0,005) istatistiksel olarak azaldığı gözlendi. İmmünohistokimyasal bulgularında ise LPS-Melatonin-Vitamini C uygulanan grupla sadece LPS uygulanan grup karşılaştırıldığında, LPS-Melatonin-Vitamini C grubunda AQP-1, AQP-2, AQP-3 ifadesinin anlamlı derecede arttığı gözlenmiştir (p<0.05). Biyokimyasal bulgularda ELISA bulgularında MDA ve TAS düzeyleri üzerine herhangi bir etkisinin olmadığı ancak TOS düzeyleri üzerine olumlu bir etkisi olduğu tespit edilmiştir. BUN ve kreatinin seviyelerinde deney grupları arasında anlamlı bir fark gözlenmedi. SONUÇ: Mevcut çalışmada böbrek dokusunun histopatolojik ve Aquporinlerin protein seviyesindeki immünohistokimyasal sonuçları, geliştirilen sepsis sıçan modelinde Melatonin-C Vitamin sepsisin böbrek dokusu üzerinde koruyucu etkisi olduğu, moleküler seviyede ise AQ-1,2,3'lerin protein seviyelerini normal seviyelerde tutarak böbrek dokusunu koruduğu ve buna bağlı olarak sepsisin böbrek dokusunda meydana getirdiği akut zararlı etkilerden koruyarak hem böbrek fonsiyonlarının bozulmasını engellediği hem de vücut hemostasisisinin korunmasında ciddi bir katkı sağladığı belirlendi. Diğer taraftan Melatonin-C Vitamin uygulaması sepsisin böbrek dokusu üzerinde zararlı etkisini göstergesi olan kan BUN ve kreatin değerlerinin biyokimyasal inceleme sonuçları normal değerlere yakın olduğu ve bu bulguların Melatonin-C Vitamin uygulamasının vücut homeostasisini de koruduğu söylenebilir.

OBJECTIVE AND HYPOTHESIS: Sepsis is one of the most significant health problems today, and one of the most common complications associated with sepsis is kidney injury leading to renal failure. Melatonin, a neurohormone secreted by the pineal gland, acts as an antioxidant by neutralizing reactive oxygen species (ROS), whereas Vitamin C, a water-soluble Vitamin, is a potent biological molecule capable of neutralizing ROS. The combined administration of melatonin and Vitamin C has been suggested in the literature as a promising therapeutic strategy for the treatment of sepsis and the prevention of organ damage due to their antioxidant properties. However, studies specifically investigating the effects of melatonin and Vitamin C on sepsis-induced kidney injury are quite limited. In the present study, we aimed to investigate the protective effects of the combined administration of melatonin and Vitamin C on sepsis-induced kidney injury in a lipopolysaccharide (LPS)-induced rat model of sepsis. METHODS: Our study was conducted on male Sprague-Dawley rats. The rats were randomly divided into four equal groups: a control group, a melatonin–Vitamin C group, an LPS group, and a melatonin–Vitamin C–LPS group, establishing the experimental design. At the end of the experiment, histological analyses (hematoxylin-eosin staining and immunohistochemical staining) and biochemical analyses (ELISA assays and measurements of BUN and creatinine levels) were performed. A p-value of <0.05 was considered statistically significant for all tests. RESULTS: In the histological analysis, comparison of kidney tissues between the LPS–melatonin–Vitamin C group and the LPS-only group showed that the number of inflammatory cells in the tubulointerstitial areas was significantly lower (p<0.05), and that tubular vacuolization (p<0.05) and the number of degenerated glomeruli (p<0.005) were significantly reduced. In the immunohistochemical analysis, when comparing the LPS–melatonin–Vitamin C group to the LPS-only group, a significant increase in the expression levels of AQP-1, AQP-2, and AQP-3 was observed (p<0.05). In the biochemical findings, ELISA results indicated that the combined treatment had no significant effect on MDA and TAS levels but had a beneficial effect on TOS levels. No significant differences were observed among the experimental groups regarding BUN and creatinine levels. CONCLUSION: In the present study, histopathological findings of kidney tissue and immunohistochemical results of aquaporin protein levels demonstrated that the combination of melatonin and Vitamin C exerted a protective effect on renal tissue in the established rat model of sepsis. At the molecular level, melatonin and Vitamin C administration helped maintain normal protein levels of AQP-1, AQP-2, and AQP-3, thereby preserving kidney tissue and preventing the acute detrimental effects induced by sepsis. This protection contributed both to the maintenance of renal function and to the preservation of systemic homeostasis. Furthermore, biochemical analyses showed that serum BUN and creatinine levels, markers of kidney injury, remained close to normal values in the melatonin–Vitamin C treatment group, supporting the conclusion that this combination therapy also plays a role in maintaining overall homeostasis.
