Özet
Amaç: Çalışmamızda amaç olarak rüptür olmamış ektopik gebeliklerin tedavisinde endikasyon dahilinde kullanılan metotreksat tedavisinin başarısını öngörmede tedavinin ilk dozunun uygulandığı günde ölçülmüş olan endometriyal kalınlık ile metotreksat tedavisinin başarısı arasındaki ilişkiyi incelemek hedeflenmiştir. Gereç ve Yöntem: Retrospektif kohort çalışma olarak planlanan çalışmamıza Ocak 2020 ile Mayıs 2024 tarihleri arasında rüptür olmamış tubaya yerleşmiş ektopik gebelik tanısı almış, Ankara Bilkent Şehir Hastanesi Jinekoloji kliniklerinde yatışı yapılarak endikasyon dahilinde metotreksat tedavisi verilen hastalar dahil edilmiştir. Hastaların ultrasonografi bulguları, laboratuvar değerleri ve hasta bilgileri hastanenin bilgi sisteminden bakılarak elde edilmiş ve kayıt altına alınmıştır. Hastaların yaşı, gebelik sayısı, doğum ve yaşayan çocuk sayısı, daha önce geçirilmiş düşük ve küretaj öyküsü, doğum şekli, kronik hastalıkları, ilaç kullanımı, geçirilmiş operasyon ve/veya ektopik gebelik öyküsü ve varsa ektopik gebeliğin hangi yöntemle tedavi edildiği, hastanın başvurusunda mevcut olan bulguları (karın ağrısı, vajinal kanama…vb), tanı anında beta HCG düzeyi, ilk doz metotreksat uygulanmadan hemen önce yapılmış olan ikinci basamak ultrasonografide endometrium çift duvar kalınlığı (EÇDK), metotreksat uygulaması sonrası birinci, dördüncü, yedinci, on birinci ve on dördüncü günlerdeki beta HCG düzeyleri, ikinci doz metoreksat gerekliliği, ikinci doz metotreksat uygulaması sonrası birinci, dördüncü, yedinci, on birinci ve on dördüncü günlerdeki beta hcg düzeyleri ve tedavi başarısına bakılmıştır. Verilerin analizinde SPSS 11.5 programından faydalanılmış olup istatistiksel anlamlılık düzeyi p<0,05 olarak alınmıştır. Bulgular: Çalışmamıza dahil edilen 136 hastanın metotreksat tedavisinin birinci gününde, tedaviden hemen önce ölçülmüş olan endometrium çift duvar kalınlığı 9,12±4,54 (Ort.±SS) olarak hesaplanmıştır. 110 hastaya (%80,9) tek doz metotreksat uygulanmış olup 26 hastaya(%19,1) ise iki doz metotreksat tedavisi uygulanmıştır. Toplamda 96 hastanın(%70,6) metotreksat tedavisi başarı ile sonuçlanırken; 40 hastanın ise (%29,4) metotreksat tedavisi başarısız olarak sonuçlanmıştır. İstatistiksel analiz ile tanı anında beta HCG düzeyi ile metotreksat uygulanan birinci gün ölçülmüş olan endometrium çift duvar kalınlığı değeri arasında pozitif yönlü orta büyüklükte, anlamlı ilişki bulunmuştur (r=0,400 ve p<0,001). Yani tanı anında beta HCG düzeyi arttıkça metotreksat 1. gün EÇDK değeri de artmaktadır. Tek doz metotreksat uygulanmış ve başarısız olarak sonuçlanmış hasta grubunda beta HCG değeri ölçülen her zamanda tedavi sonucu başarılı olan gruptan 1205,23 mIU/ml daha fazla olduğu tespit edilmiştir. Ayrıca iki doz metotreksat tedavisi almış olan ve başarısız olarak sonuçlanmış olan hasta grubunda ise beta HCG değeri ölçülen her zaman için başarılı olan gruptan 1742,76 mIU/ml daha fazla olduğu görülmüştür. Değişkenler incelendiğinde daha önce geçirilmiş ektopik gebelik öyküsü ve metotreksat birinci gün ölçülmüş olan EÇDK değeri tedavi başarısına etki eden anlamlı risk faktörleri olarak bulunmuştur (sırasıyla p=0,005 ve p<0,001). Ektopik gebelik öyküsü olan hastalarda tedavi başarısızlığı riski ektopik gebelik öyküsü olmayan hastalara oranla 4,172 kat daha fazla olarak hesaplanmıştır. Metotreksat uygulandığı 1. gün ölçülmüş olan EÇDK ortalaması tedavisi başarılı olarak sonuçlanan grupta ortalama 7,96±4,18 mm olarak bulunmuş olup, başarısız olarak sonuçlanan grupta ise 11,90±4,18 mm olarak hesaplanmış olup bu EÇDK değerleri metoreksat tedavi sonuçları başarısız olan grupta anlamlı derecede yüksek olarak bulunmuştur (p <0,001). Çalışmamızda ayrıca metotreksat 1. gün EÇDK ölçümü için tedavi sonucunun başarılı ya da başarısız olmasını belirleyen bir kesim noktası hesaplanmıştır. EÇDK için ROC eğrisi altında kalan alan 0,758 (p<0,001), bu değere ait sensitivite, spesifisite değerleri ise sırasıyla 0,800 ve 0,688 olarak bulunmuştur ve EÇDK için kesim noktası ise 9,50 olarak hesaplanmıştır. Ayrıca metotreksat 1. gün EÇDK değeri bir birim arttıkça hastalarda tedavi başarısızlığı riskinin 1,226 kat arttığı tespit edilmiştir. Sonuç: Metotreksat 1.gün ölçülmüş olan EÇDK tedavi sonucu başarılı olan grupta ortalama 7,96±4,18mm iken, tedavi sonucu başarısız olarak sonuçlanan grupta ise ortalama 11,90±4,18 mm olarak hesaplanmıştır. Ayrıca metotreksat tedavisinde başarıyı öngörmede tedavi öncesi ölçülmüş olan EÇDK için kesim noktası 9,50 mm olarak tespit edilmiştir. Metotreksat tedavisinin ilk gününde ölçülmüş olan EÇDK değeri bir birim arttıkça tedavi başarısızlığı riskinin 1,226 kat arttığı bulunmuştur. Bu bağlamda metotreksat tedavisi öncesi ölçülen EÇDK değerinin de tedavi seçiminde bir belirteç olabileceğini ve bu değerin ne kadar düşükse metotreksat tedavisine yanıtın daha etkin olacağını bulunmuştur. Ancak bu konuyla ilgili daha fazla hastanın katıldığı, daha geniş profilden hastaların bulunduğu, prospektif ve randomize kontrollü çalışmalarla bu sonuçların teyit edilmesine ve/veya geliştirilmesine ihtiyaç vardır.

Objective: The aim of our study was to investigate the relationship between endometrial thickness measured on the day of the first dose of methotrexate treatment and the success of methotrexate treatment in predicting the success of methotrexate treatment used within the indication in the treatment of unruptured ectopic pregnancies. Materials and Methods: Our study, which was planned as a retrospective cohort study, included patients who were diagnosed with ectopic pregnancy with unruptured tubal location between January 2020 and May 2024, who were hospitalized in the gynecology clinics of Ankara Bilkent City Hospital and received methotrexate treatment within the indication. Ultrasonographic findings, laboratory values and patient information of the patients were obtained and recorded from the hospital's information system. Age of the patients, gravida, parity, number of surviving children, previous history of abortion and curettage, type of delivery, chronic diseases, drug use, history of previous operation, history of previous ectopic pregnancy and the method by which the previous ectopic pregnancy was treated, symptoms present in the patient's application (abdominal pain, vaginal bleeding... etc.), beta HCG level at the time of diagnosis, endometrial double wall thickness (ECDK) in second-line ultrasonography performed just before the first dose of methotrexate, beta HCG levels on the first, fourth, seventh, eleventh and fourteenth days after methotrexate administration, the need for a second dose of methotrexate, beta HCG levels on the first, fourth, seventh, eleventh and fourteenth days after the second dose of methotrexate administration and the success of the treatment were examined. SPSS 11.5 program was used in the analysis of the data and the statistical significance level was taken as p<0.05. Results: The endometrial double wall thickness measured just before the treatment on the first day of methotrexate treatment of 136 patients included in our study was calculated as 9.12±4.54 (Mean±SD). A single dose of methotrexate was administered to 110 patients (80.9%) and two doses of methotrexate were administered to 26 patients (19.1%). In total, 96 patients (70.6%) had successful methotrexate treatment; while 40 patients (29.4%) had unsuccessful methotrexate treatment. With statistical analysis, a positive, medium-sized, significant relationship was found between the beta HCG level at the time of diagnosis and the endometrial double wall thickness value measured on the first day of methotrexate administration (r=0.400 and p<0.001). In other words, as the beta HCG level at the time of diagnosis increases, the endometrial double wall thickness on the first day of methotrexate treatment also increases. In the patient group who received a single dose of methotrexate and failed, the beta HCG value was found to be 1205.23 mIU/ml higher than the successful group at each time. In addition, in the patient group who received two doses of methotrexate treatment and failed, the beta HCG value was always 1742.76 mIU/ml higher than the successful group. When variables were examined, previous ectopic pregnancy history and endometrial double wall thickness measured on the first day of methotrexate treatment were found to be significant risk factors affecting treatment success (p=0.005 and p<0.001, respectively). The risk of treatment failure in patients with a history of ectopic pregnancy was calculated to be 4.172 times higher than in patients without a history of ectopic pregnancy. The average endometrial double wall thickness measured on the first day of methotrexate administration was found to be 7.96±4.18 mm in the group whose treatment was successful, and was calculated as 11.90±4.18 mm in the group whose treatment was unsuccessful. Thickness values were found to be significantly higher in the group whose methotrexate treatment results were unsuccessful (p <0.001). In our study, a cut-off point was also calculated for endometrial double wall thickness measurement on day 1 of methotrexate, which determines whether the treatment result is successful or unsuccessful. The area under the ROC curve for endometrial double wall thickness was found to be 0.758 (p<0.001), the sensitivity and specificity values for this value were found to be 0.800 and 0.688, respectively, and the cut-off point for endometrial double wall thickness was calculated as 9.50. In addition, it was found that as the endometrial double wall thickness value on day 1 of methotrexate increased by one unit, the risk of treatment failure in patients increased by 1.226 times. Conclusion: Methotrexate was measured on the 1st day and the mean ECDK was 7.96±4.18 mm in the group that was successful and 11.90±4.18 mm in the group that failed the treatment. In addition, the cut-off point for ECDK, which was measured before treatment, was determined as 9.50mm in predicting the success of methotrexate treatment. It was found that the risk of treatment failure increased 1,226-fold as the ECDK value measured on the first day of methotrexate treatment increased by one unit. In this context, we think that the ECDK value measured before methotrexate treatment may also be a marker in treatment selection and that the lower this value, the more effective the response to methotrexate treatment will be. However, there is a need to confirm and/or improve these results with prospective and randomized controlled trials with more patients and broader profile patients.
