Özet
Giriş ve Amaç Hasarlı damarların onarımı veya replase edilmesi kalp damar cerrahisi ile birlikte plastik cerrahi ve ortopedi gibi çeşitli cerrahi disiplinlerin de ilgi alanında olan önemli bir konudur. Önemli arteriyel yapıların hasar sonucu hedef organlara kan iletemedikleri durumlarda bu görevi üstlenecek çeşitli damar greftleri kullanılır. Bu çalışma, doku mühendisliği tekniklerini kullanarak otolog bir vasküler greft alternatifi geliştirmeyi amaçlamaktadır. Silikon kateter etrafında kapsül oluşumunu takiben gerçekleştirilen endotel hücre tohumlamasının, greft biyouyumluluğunu ve fonksiyonunu artırmadaki potansiyeli araştırılmıştır. Gereç ve Yöntem Çalışmada, 24 Sprague Dawley sıçanı rastgele üç deney grubuna ayrılmıştır. Tüm gruplarda bilateral silikon blok implantasyonu gerçekleştirilmiştir. Altı hafta sonra sağ taraftaki silikonlar çıkarılmış ve grup 1'e sadece endotel tohumlama, grup 2'ye hyaluronik asit jel ile birlikte endotel tohumlama, grup 3'e hyaluronik asit jel ve eksozom ile birlikte endotel tohumlama işlemi yapılmıştır.Her sıçanın sol tarafı kontrol grubu olarak değerlendirilmiştir.Endotelizasyon, anjiogenez ve inflamasyon oranları histolojik analizler ile değerlendirilmiştir. Bulgular Hyaluronik asit jel ve eksozom ile birlikte endotel tohumlama işlemi yapılan 3.grupta diğer gruplara kıyasla anlamlı derecede artmış endotelizasyon ve anjiogenez saptanmıştır. Endotelizasyon, ışık ve elektron mikroskobisinde gösterilmiştir. Sonuç Bu çalışma; endotelyal hücre tohumlama, hyaluronik asit ve eksozom temelli desteklerle biyomühendislik ile üretilmiş otolog vasküler greftlerin potansiyelini ortaya koymaktadır. Bulgular, bu kombinasyonun endotelizasyonu artırarak tromboz riskini azalttığını göstermektedir. Daha fazla araştırmaya ihtiyaç duyulmakla birlikte, bu sonuçlar biyomühendislik ile üretilmiş damar greftlerinin geliştirilmesi için sağlam bir temel oluşturmakta ve bu yaklaşımın vasküler cerrahi sonuçlarını iyileştirme potansiyeline sahip olduğunu göstermektedir. Introduction and Aim The repair or replacement of damaged blood vessels is a critical issue not only in cardiovascular surgery but also in various surgical disciplines such as plastic surgery and orthopedics. When major arterial structures lose their ability to deliver blood to target organs due to injury, different types of vascular grafts are utilized to restore circulation. This study aims to develop an autologous vascular graft alternative using tissue engineering techniques. Specifically, it investigates the potential of endothelial cell seeding following capsule formation around a silicone catheter to enhance graft biocompatibility and functionality. Materials and Methods In this study, 24 Sprague Dawley rats were randomly divided into three experimental groups. Bilateral silicone block implantation was performed in all groups. After six weeks, the silicone blocks on the right side were removed, and the following interventions were applied: Group 1 received only endothelial cell seeding, Group 2 received endothelial cell seeding combined with hyaluronic acid gel, and Group 3 received endothelial cell seeding combined with both hyaluronic acid gel and exosomes. The left side of each rat served as the control. Endothelialization, angiogenesis, and inflammation levels were evaluated through histological analysis. Results A significant increase in endothelialization and angiogenesis was observed in Group 3, which received endothelial cell seeding in combination with hyaluronic acid gel and exosomes, compared to the other groups. Endothelialization was confirmed using both light and electron microscopy. Conclusion This study highlights the potential of bioengineered autologous vascular grafts supported by endothelial cell seeding, hyaluronic acid, and exosomes. The findings suggest that this combination enhances endothelialization and reduces the risk of thrombosis. While further research is required, these results establish a solid foundation for the development of bioengineered vascular grafts and demonstrate the potential of this approach to improve vascular surgery outcomes.
