Özet
AMAÇ: Bu çalışmada adenoidektomi, tonsillektomi ve adenotonsillektomi operasyonları yapılan hastalarda operasyonun ses kalitesi ve nazal rezonans üzerindeki etkilerini objektif ve subjektif analizlerle araştırmak amaçlanmıştır. GEREÇ VE YÖNTEM: Bu çalışma kliniğimizde takip ve tedavisi gerçekleştirilen 201 hastanın (45 adenoidektomi, 46 tonsillektomi, 45 adenotonsillektomi, 65 kontrol) incelendiği prospektif ve gözlemsel bir çalışmadır. Hastalardan (preoperatif, postoperatif 1. ve 3. aylarda) ve kontrol grubundan nazalans analizi (oral ve nazal dil birimlerinin nazalansları), subjektif analizler (VHI, GRBAS, CAPE-V) ve ses akustiği analizi (temel frekans (F0), Jitter, Shimmer, gürültü-harmonik oranı (NHR), harmonik-gürültü oranı (HNR), kepstral tepe çıkıntısı (CPP)) veri kayıtları ve ölçümleri gerçekleştirilmiş; hasta grupları ölçüm sonuçları hem kendi içinde hem birbirleri arasında hem de kontrol grupları ile karşılaştırılmıştır. Çalışma kapsamında nazalans analizinde, akustik analizde ve subjektif analizde kullanılmak amacıyla 473 nazalans kaydı ve 473 ses kaydı (kapsama dahil edilen) verisi toplanılmış, 946 grup kayıt verisi analiz edilerek, ölçümler 10 farklı analiz türü ve 20 farklı analizle (11 farklı dil biriminin nazalans analizi, temel frekans, jitter, shimmer, NHR, HNR, CPP, VHI, GRBAS, CAPE-V) 9460 analiz sonucu değeri ve 473 anket verisi istatiksel olarak tekrar analiz edilmiş ve değerlendirilmiştir. BULGULAR: Preoperatif ve postoperatif dönemde elde edilen bulgular, operasyon türüne göre farklılıklar göstermiştir. Tonsillektomi ve Adenotonsillektomi hasta gruplarında akustik analiz parametrelerinden CPP değerleri, preoperatif dönemde kontrol grubundan düşük saptanmıştır (p<0,05); bu iki hasta grubunda CPP değerlerinin preoperatif, postoperatif 1.ay ve 3.ay ölçümlerinde kontrol grubuna göre artış saptanmıştır (p<0,05). Diğer akustik analiz parametrelerindeki değişim hasta gruplarına göre değişkendir. Adenoidektomi hasta grubunun nazal dil birimlerinin nazalansları, Tonsillektomi hasta grubunun oral dil birimlerinin nazalansları, Adenotonsillektomi hasta grubunun bazı oral ve nazal dil birimlerinin nazalansları preoperatif dönemde hiponazaldır; postoperatif 1. ve 3. ayda normonazal olarak izlenmiştir. Subjektif ses analizlerinden VHI-10 sonuçları, tüm hasta gruplarında kontrol gruplarından yüksektir (p<0,001) ve postoperatif dönem ölçümlerinde azalma izlenmiştir (p<0,001). GRBAS ve CAPE-V değerleri birbirlerine korele olarak adenoidektomi cerrahi prosedürünü içeren hasta gruplarında postoperatif 1. ayda artış gösterse de postoperatif 3. ayda azalmıştır. SONUÇ: Bu çalışmada adenoidektomi, tonsillektomi, adenotonsillektomi sonrasında ses kalitesinde artış olduğu saptanmıştır. Sesteki minimal değişiklikleri saptamada CPP, diğer akustik analiz parametrelerinden üstün ve VHI-10 ile ters koreledir. GRBAS ve CAPE-V testleri sonuçları bakımından birbirlerine korele testlerdir.

AIM: This study aimed to investigate the effects of adenoidectomy, tonsillectomy and adenotonsillectomy operations on voice quality and nasal resonance in patients with objective and subjective analyses. MATERIALS AND METHODS: This study is a prospective and observational study in which 201 patients (45 adenoidectomy, 46 tonsillectomy, 45 adenotonsillectomy, 65 controls) who were followed up and treated in our clinic were examined. Nasalance analysis (nasalances of oral and nasal language units), subjective analyses (VHI, GRBAS, CAPE-V) and voice acoustic analysis (fundamental frequency (F0), Jitter, Shimmer, noise-harmonic ratio (NHR), harmonic- to-noise ratio (HNR), cepstral peak protrusion (CPP)) data were recorded and measured from the patients (preoperatively, at 1st and 3rd months postoperatively) and the control group; the measurement results of the patient groups were compared within themselves, among each other and with the control groups. Within the scope of the study, 473 nasalance recordings and 473 voice recordings (included in the scope) data were collected for use in nasalance analysis, acoustic analysis and subjective analysis, 946 group recording data were analyzed, measurements were made with 10 different analysis types and 20 different analyses (nasalance analysis of 11 different language units, fundamental frequency, jitter, shimmer, NHR, HNR, CPP, VHI, GRBAS, CAPE-V) 9460 analysis result values and 473 survey data were statistically re-analyzed and evaluated. RESULTS: Findings obtained in the preoperative and postoperative periods showed differences according to the type of operation. CPP values of the acoustic analysis parameters in the tonsillectomy and adenotonsillectomy patient groups were found to be lower than the control group in the preoperative period (p<0.05); an increase was found in the preoperative, postoperative 1st and 3rd month measurements of CPP values in these two patient groups (p<0.05) .Changes in other acoustic analysis parameters varied according to the patient groups. The nasalences of the nasal tongue units in the adenoidectomy patient group,the nasalences of the oral tongue units in the tonsillectomy patient group, and the nasalences of some oral and nasal tongue units in the adenotonsillectomy patient group were hyponasal in the preoperative period; they were observed to be normonasal in the postoperative 1st and 3rd months. VHI-10 results from subjective voice analyses were higher than the control groups in all patient groups (p<0.001) and a decrease was observed in time-dependent measurements (p<0.001). Although GRBAS and CAPE-V values increased in the patient groups that had adenoidectomy surgery in the first postoperative month, they decreased in the third postoperative month. CONCLUSION: This study revealed that voice quality increased after adenoidectomy, tonsillectomy, adenotonsillectomy. CPP is superior to other acoustic analysis parameters in detecting minimal changes in voice and is inversely correlated with VHI-10. GRBAS and CAPE-V tests are correlated tests in terms of their results.
